Depression

Everyone knows what it feels like to get the blues once in a while. But depression is a serious illness that is more severe than a bad day and lasts much longer. Symptoms of depression stop a person from being able to function and enjoy daily activities for weeks or months at a time. It can happen to anyone, and it isn't something that people can control by force of will or "snap out of it."

Some common symptoms of depression include:

Feeling sad, guilty, empty or hopeless
Fatigue and a lack of energy and motivation
A loss of pleasure in activities that were previously enjoyed
Unusual sleep or eating habits
Mental fog -- trouble concentrating, remembering, or making decisions
Physical aches and pains
Suicidal thoughts or a preoccupation with death

We don't yet know what causes depression, but it's thought that it is a combination of genetic, biological, environmental, and social influences. Because of this, the most effective treatments for depression combine medication with psychotherapy. Therapy, especially cognitive-behavioral therapy, can be extremely helpful in resolving the negative thoughts and feelings that come with depression. It gives patients new tools that they can use themselves to cope when their depression is making them feel down.

Some of the common medications used to treat depression include antidepressants such as SSRI's (Prozac, Paxil, Zoloft) or atypical antidepressants (Cymbalta, Wellbutrin). It's important to remember that these medications have different effects on everybody, and no one medication works right for everyone. Patients may have to try a couple before finding the one that works just right for them. If the first medication they try doesn't work, they should talk to their doctor about trying something else. In extreme cases where medication is not enough, electro-convulsive therapy and hospitalization may be the answer to keeping a severely depressed person safe.

Depression is a difficult illness to deal with, but it is more common than believed and there are many people who can help. With the right treatment, individuals with depression can get back to fully enjoying life again.

Hemophilia

Hemophilia is a bleeding disorder in which blood cannot clot normally. After an injury, people who have hemophilia bleed longer because they lack certain blood proteins that are responsible for clotting, called clotting factors. Some hemophiliacs are mildly deficient in this protein, so they bleed only when injured or after surgery. Those who are severely deficient, however, may experience symptoms of spontaneous bleeding, including the following:

Presence of blood in stool or urine
Frequent nosebleeds or large bruises
Unusual bleeding after being vaccinated
Pain, swelling, or tightness in joint

Most people with hemophilia have a family history of the disorder, though about 30% do not. The most common forms are hemophilia A (classic hemophilia) and hemophilia B (Christmas disease). Hemophilia A and B share certain characteristics, but different genes are involved. Acquired hemophilia is very rare and has been linked to autoimmune conditions, cancer, and multiple sclerosis.

Hemophilia may be diagnosed using a blood test. If there is family history of the disorder, testing may be done prenatally. Treatment involves regular intravenous replacement of clotting factors, which may be produced in a laboratory (recombinant) or come from blood donors. For people with milder forms of the disease, a hormone called desmopressin may stimulate the body to produce clotting factors. Other drug therapies include medications that help slow the breakdown of blood clots and medications that are applied directly to the wound to promote clotting.

Living with hemophilia can be challenging, but simple things may help prevent excessive bleeding. For example, choosing activities like walking or swimming over contact sports can prevent injuries and strengthen muscles while protecting the joints. Chances for bleeding may also be reduced by making sure that children wear helmets and elbow pads during activities like bicycle rides.

Thyroid Problems

Thyroid problems are a group of conditions that alter hormone regulation and metabolism. The thyroid is a small, butterfly-shaped gland located in the lower neck, just below the Adam's apple (laryngeal prominence) and larynx (voice box). Despite its small size, the thyroid has an outsized effect on the body by regulating hormones that control metabolism. Metabolism is the body's process of converting consumed food into fuel to power the entire body. Heart rate, weight, cholesterol, body temperature, and even physical growth and development are all affected by the metabolism.

When the thyroid malfunctions, the body is unable to properly process and allocate energy. Thyroid problems, such as hyperthyroidism (overactive thyroid) and hypothyroidism (underactive thyroid), cause irregular metabolism. In some cases, the body produces antibodies that attack the thyroid cells for unknown reasons. The damaged thyroid responds by releasing either too little or too much T3 and T4 thyroid hormones. Thyroid malfunctions may also arise from genetic causes or iodine deficiency. Thyroid problems may include:

Hashimoto's thyroiditis, an autoimmune disease that causes hypothyroidism
Graves' disease, an autoimmune disease that causes hyperthyroidism
Goiter
Thyroid nodule
Thyroid cancer

Hashimoto's is the leading cause of hypothyroidism, which is when the thyroid does not produce enough hormones to stimulate adequate metabolism. The condition is diagnosed by blood tests that measure TSH (thyroid stimulating hormone) and detect Hashimoto's antibodies. Symptoms of hypothyroidism include sluggish metabolism, inflammation, fatigue, weight gain, brittle hair, dry skin, and sensitivity to cold. To supplement impaired thyroid function, patients with Hashimoto's are typically prescribed synthetic thyroid hormone pills. Such patients may need to consume synthetic thyroids for the rest of their lives. Other treatments may include dietary changes, such as the avoidance of certain foods and medication to reduce inflammation. Frequent blood tests can inform patients if their TSH levels are within a normal range.

Grave's disease is most frequently the cause of hyperthyroidism, which is when the thyroid produces too many hormones, overstimulating one's metabolism. The condition is also diagnosed by blood tests that measure TSH and detect Grave's antibodies. Elevated heart rate, high blood pressure, sweating, bulging eyes, and weight loss are common symptoms of hyperthyroidism. Treatments for hyperthyroidism include oral radioactive iodine, which slowly shrinks the thyroid, slowing the production of excess thyroid hormone. Other anti-thyroid medications such as methimazole (Tapazole) also reduce the production of thyroid hormones. Blood pressure medications known as beta blockers can ease the increased heart rate caused by hyperthyroidism. If treatments are unsuccessful, patients may be prescribed a thyroidectomy, or surgery to remove part or all of the thyroid. Some patients with hyperthyroidism may require ophthalmological (eye medicine) treatments such as eye drops, prednisone, and surgery to reduce eye swelling and bulging.

Untreated thyroid disorders frequently lead to goiters or thyroid nodules. A thyroid goiter is a large lump that can be felt near the base of the neck and is made of swollen thyroid tissue or nodules (excess cell growth). Nodules and goiters are diagnosed by CT scan or ultrasound. Obstructive goiters can cause pain, coughing, and abnormal breathing. The most common cause of goiters is hyperthyroidism (overactive thyroid). However, Hashimoto's is also known to cause goiters or nodules. Goiters develop from continued damage from antibodies to the thyroid over time. At times, a goiter can result from a cancerous thyroid tumor. Most goiters and nodules are themselves benign.

The exact causes of thyroid cancer are unknown. People who develop thyroid cancer may or may not have another thyroid problem such as Hashimoto's or Grave's disease. Symptoms of thyroid cancer include a nodule that can be felt on the neck, hoarseness, difficulty swallowing, swelling in the neck, and neck and throat pain. CT and MRI scans are used to diagnose thyroid cancer. There are four types of thyroid cancer which vary in intensity. Thyroidectomy (thyroid removal surgery) is usually prescribed to treat thyroid cancer.

Thyroid problems are often long-term conditions that require some form of treatment. With regular monitoring and medication, many people with thyroid problems are able to lead active and fulfilling lives.

All of my experiences with Dr. Vogt have been outstanding. He is always respectful, listens well, offers sound and understandable advice, seems caring and delivers exceptional care to his patients. During Covid, so many providers went a little crazy and cracked under the pressure (understandably), many became rude and disrespectful and generally underperformed...but not Dr. Vogt. Somehow he really kept it together...that is pretty amazing considering the circumstances that he was likely working under. I have tremendous respect and gratitude for him.

I have to say Doctor vote is the most caring, thoughtful, and intelligent. Doctor I have ever seen. He always talk to me about every blood test or procedure. I get and sometimes even draws a sketch to explain. When I leave the office, I am amazed of the care, I get from his team. This doctor goes out of his way to make you feel good about your test results in comforting if there is a situation that demands more attention. On a scale of one to 10 I give him a 20. Great doctor and I want to keep him for myself .

Dr Vogt and staff are amazing. I have received the best care and you can feel that they actually care. Dr Vogt paved the way for my additional tests and specialists appts so that we could work out what was going on. His support staff is a+. This was a life threatening issue-non Covid and we were all scared. The compassion I received from both Dr Vogt and his staff has been amazing. In my almost 60 years -I have never felt so reassured-I have complete trust in him.

I got my medical care in a nearby community, until about 15 years ago. Doctors came and left like clockwork, albeit, a busted clock. I had seven different doctors over 35 years, then, somehow ended up with the one I started with. Some good doctors among them. Plus a fair number of locum tenens, nurse practitioners, and physicians assistants. Since that, I've seen Doctor Vogt. Best doc ever.

Upon arrival at the doctor's office I was not feeling well. My BP and pulse were checked and an immediate EKG was done. I was in tachycardia and short of breath. Dr. Vogt & I briefly talked. My husband, who was waiting for me, drove me to the ER. Dr. Vogt alerted the Dr.on call that I would be arriving. Everyone did a good job taking care of me.

Every person I encounter at that office from the minute I walk in the door until I leave are all very polite and have upbeat personalities. It's honestly something I look forward to. It's like seeing old friends every time I go. The uplifting attitude there is contagious!!
