Cochlear Implant Surgery

A cochlear implant is an electronic device that helps people with severe hearing loss. Although it does not treat deafness or restore hearing, it can help its users to better perceive sounds. For adults, a cochlear implant is typically recommended when traditional hearing aids are unhelpful. Children may also benefit from a cochlear implant if they are born deaf or do not fully develop auditory skills. Surgery to implant the device is called cochlear implant surgery.

Implantation begins with an incision behind the ear. The surgeon will make an opening in the portion of the ear responsible for hearing, called the cochlea. The internal portion of the implant is then placed under the skin and attached to the skull, and the incision is closed. Part of the device is placed externally, behind the ear, and connected to the internal portion by a magnet.

Cochlear implant surgery may take between one and four hours. Most patients may return home the same day or the following morning. After surgery, patients will need to follow up with their doctor and specialists for fitting and programming of the device.

Eardrum Repair

Eardrum repair is a surgical procedure performed to treat ruptured eardrums. Ruptures are holes in the eardrum, or tympanic membrane, that may occur due to ear infections or injury. Often, a ruptured eardrum may heal on its own. However, if it does not, the hole may increase in size. To avoid complications such as hearing loss, surgery is required to repair the hole.

There are two types of eardrum repair surgeries: myringoplasty and tympanoplasty. Both procedures use a graft to cover the hole, or perforation. Grafts may be composed of tissue taken from the body, such as skin, or they may be paper or gel-like materials. The difference between the procedures is that during a myringoplasty, surgery is limited to the eardrum. Myringoplasty is typically performed for smaller perforations and for conditions in which only the eardrum is affected.

In contrast, tympanoplasty is used for larger perforations. It may also target other areas of the ear to treat infection, remove cysts, or address other issues. Common types of tympanoplasty include:

  • Tympanoossiculoplasty, which combines tympanoplasty with ossicular chain reconstruction (ossiculoplasty). The ossicular chain in the middle ear is composed of three bones, called ossicles or middle ear bones, which are responsible for transmitting sound to the inner ear. Ossiculoplasty repairs damaged ossicles to treat or prevent hearing loss.
  • Tympanomastoidectomy, which combines tympanoplasty and mastoidectomy. A mastoidectomy removes the bone behind the ear (mastoid), to treat damage or infection.

Depending on the type of eardrum repair, surgery can last for a half hour (myringoplasty) or for several hours (tympanoplasty). The surgeon will make an incision to access the eardrum. Infected tissue and broken bones will be removed, and a graft will be placed to seal the perforation. The ear will heal over four to six weeks. Patients may return home the same day following surgery.

Enteroscopy

Enteroscopy visually examines the small bowel and identifies such issues as bleeding, tumors, polyps, ulcers, and swollen lymph nodes. Common reasons this exam may be ordered are abnormal X-ray results and unexplained diarrhea.

Enteroscopies may be used alone or in combination with other procedures. For example, if a doctor suspects malignancy (cancer) in the small bowel, she may use an enteroscope to take a sample of tissue from the small bowel and have it examined in a laboratory. If there is a blockage in the small intestine that cannot be surgically removed, an enteroscopy-guided stent placement may help relieve some of the symptoms associated with the obstruction. Enteroscopes may also be used to treat intestinal bleeding and remove abnormal growths like tumors and polyps, as well as foreign objects that might have found their way into the small bowel.

There are several ways to perform an enteroscopy:

  • Push enteroscopy involves the use of a long tube with a camera, called an enteroscope. Depending on the location of the problem, the enteroscope is either passed through the mouth or rectum. This technique is often used to detect and stop bleeding in the upper small intestine.
  • Balloon enteroscopy uses scopes that have tiny balloons at the tip. The balloons inflate and deflate to help make the passage of the scope into the small bowel easier. This method is used when there is a need to access and treat certain parts of the small intestine that push enteroscopy cannot reach.
  • Capsule enteroscopy involves swallowing a capsule with a tiny camera inside it. As the capsule travels through the small intestine, the camera records and sends images that are then downloaded and analyzed by the doctor. The capsule eventually leaves the body during bowel movement. If the doctor needs to visualize the entire small intestine, capsule enteroscopy is the only option. The only drawback to this technique is that it cannot be combined with any treatment.

Enteroscopy is typically performed at the doctor's office and takes about 20 to 30 minutes, but may take longer if combined with another procedure. Patients may have a sore throat for a few hours if the scope was inserted through the mouth. Normal activities may be resumed about 24 hours following enteroscopy.
