Koval was consistently late to the 4 appointments I had with him 20+ minutes, including telehealth appointments. He falsified information in my chart including lying about prescribing two medications he noted in the chart that he sent to my pharmacy. After reviewing my chart 3 months after our initial appointment, I discovered a number of medical mistakes. I unfortunately had to file a claim to the medical board for the degree of negligence I experienced. His bedside is one of the worst I have ever encountered, and he continually would dangle the carrot of refills so that I would return for additional appointments. Just a truly unethical person.
