Asthma

Asthma is a chronic lung disease where the tissue and airways of the lungs become extremely sensitive to certain substances. When these substances are breathed in, the lungs become inflamed. The muscles around the airways tighten and squeeze the lungs, and passages within the lungs swell and tighten. The airways themselves produce mucus, which further clogs the tightened, swollen airways. A person having an asthma attack finds it very difficult to breathe, and a severe attack can even be fatal.

Asthma affects people of all ages, but people with asthma are most often diagnosed as children. Symptoms can include wheezing, a tight feeling in the chest, shortness of breath, and coughing. Some people have mild symptoms all the time, and some people have no symptoms at all, but everyone with asthma is susceptible to occasional severe attacks or flare-ups of symptoms when they are exposed to triggers. Triggers vary widely but can include:

  • Allergens (dust mites, pet fur, mold, pollen, grass)
  • Irritants (smoke, pollution, dust, chemicals)
  • Viral infections
  • Physical activity, especially outdoors
  • Certain medications or chemicals in foods (aspirin, sulfites)

Treatment for most asthma patients involves three steps. Patients learn what their asthma triggers are and avoid them. They take a daily control medication, usually an inhaled corticosteroid, to reduce inflammation in the lungs. Also, they have rescue medication with them at all times to take in case of a severe attack. These quick-acting inhaled medications relax the muscles around the airways and allow the lungs to open up for air.

There is no cure for asthma, but with treatment it should not interfere with daily life and activities.

Subcutaneous Allergen Immunotherapy (Allergy Shots)

Also known as allergy shots, subcutaneous allergen immunotherapy is a treatment in which gradually increasing doses of an allergen are injected under the skin. An allergen is a substance that can bring about an allergic reaction. Pollen, dust mites, and pet dander are the most common examples of these substances. When patients receive an allergy shot, their body produces "blocking" antibodies that decrease the immune system's sensitivity to an allergen, thus reducing their symptoms.

Before treatment is begun, patients and their doctors first have to identify different factors that trigger their symptoms. Then, skin tests are done to confirm which substances cause allergic reactions. Blood tests may be performed as well, but they are typically not necessary.

There are two phases to subcutaneous allergen immunotherapy: a build-up phase and a maintenance phase. The build-up phase lasts three to six months, during which patients receive shots with increasing amounts of the allergens once or twice a week. When the effective dose is reached, the maintenance phase begins and may continue for up to five years.

Although allergy shots are the longest-lasting and most effective allergy treatment available, they are not for everyone. If patients have heart problems, high blood pressure, or severe asthma, they should not be given allergy shots. The injections are generally safe, but they may have a little swelling at the injection site, which is normal. Rarely, severe shock-like reactions, called anaphylaxis, can also occur. Patients will likely be required by their doctor to stay for half an hour after their shot, so in the event of an anaphylactic reaction, it can be treated immediately.

Dr. Peng and team are outstanding. I feel extremely lucky to have grabbed an appointment - after only a few months of calling back to check cancellations. It's a combination of her astounding expertise, diligence, and care. She made rapid and impressive use of our time to review a complicated, 6-month, very acute history across multiple PCPs/labs that was in the middle of a steroids "rescue course." It was clear she has deep experience with not just allergy but immunology. One of the kindest things to me was a willingness not just to put down a clear diagnosis and plan going forward, but listening to the vague/med side effect/systemic symptoms that can also exist and make me feel less "crazy" about them. Almost as important - unlike others in town - the front office never made me feel unwelcome or silly for calling in to check cancellations. So far the team seems adapt and confident at managing insurance for biologics, which is huge. A credit to Swedish - the only allergy/immunology group there I think - so hang on to em!

While the wait time for a new patient was extremely long, understandable for a specialist, I was seen immediately, she was well informed of the issue I came in for and was prepared with the 3 potential diagnosis and tests at the ready. Everybody at the office was kind, courteous, empathetic and prompt. She updated my results in mychart with understandable comments immediately when lab results became available and checked in on results for me that were taking too long. Someone from the office came with me and was very patient, to make sure labs were ordered properly after a mix up with the lab system.

Dr. Peng was wonderful with my 11 year old daughter. This was her first visit for an allergy evaluation. My daughter is scared of needles and was really scared about getting allergy testing. Dr. Peng was kind, compassionate about the situation and provided reassurance to my daughter that it would be quick and explained it would feel like a poke but it would be over quickly. She was able to put my daugther at east.

I've always been pleased with my experiences with Dr. Peng. I wish there were more flexibility with getting the monthly shot appointments.I don't know what these questions were addressing, but it seems like it would have saved quite a few questions if you had a category for annual appointments with specialists. They are rarely for anything emergent and most of your questions were about more new issues.

I've been getting coordinate care among several Swedish providers following a serious injury I sustained in early May. I feel so incredibly blessed about the quality of care I've been getting at Swedish and its associates. I've always felt well informed and consulted about my care and diagnoses. The providers have all been top-notch and have gone out their way to accommodate my needs.

Dr. Peng was a lovely provider who listened and gathered a strong medical history from me. She explained all the testing she recommended with rational while also making it feel like a collaborative approach to my healthcare putting me in the drivers seat. While it was frustrating to wait about 7 months to get an appointment, Dr. Peng was worth the wait.

My primary care doctor asked me to send Dr. Peng details about my history and examples. I was impressed that she read it in advance so we could focus on her questions and mine, instead of me repeating my history. I left feeling informed and like she and I are a team working together to solve my allergy issues. Awesome experience.

Top notch care! I just thought waiting for over a year to be seen is too long. While the condition wasn't pressing, I do think it's too long to wait to see a specialist. But once I was seen, I thought Dr. Peng was thorough and did a great job listening and explaining techniques to deal with a yellow jacket sting.

My original appointment was a 4 month wait which was very concerning. I waitlisted myself at multiple locations. Luckily I got a call after only a week. I'm so glad I did, Marlene Peng is an absolute gem and I'm so happy to have her as part of my care team now!
