Heart Attack

A heart attack is a serious medical event that happens when the blood supplying oxygen to the heart is cut off, usually because of a blockage in an artery. This results in damage or death of part of the heart muscle. The heart can heal, but like any part of the body that was injured it leaves a scar. This scar tissue does not contract effectively, so the ability of the heart to pump is lowered after a heart attack.

A heart attack, sometimes called a myocardial infarction, can happen without warning. However there are certain factors that raise the risk of heart attack. Some of these are:

  • Age
  • Gender (most heart attack sufferers are male)
  • A family history of heart attacks
  • Smoking
  • Obesity
  • High cholesterol
  • High blood pressure
  • Diabetes
  • Unhealthy lifestyle choices (lack of exercise, poor diet, too much alcohol or stress)

Heart attacks typically feel like pain or pressure in the chest that can radiate out to the back, arm, or jaw. Some other symptoms can include fatigue or weakness, shortness of breath, sweating, and nausea.

Treatment aims to increase blood flow to the heart muscle by opening up blocked blood vessels. Medications are sometimes used to dissolve clots in the arteries. Aspirin and other medications such as Plavix can thin the blood and make clots less likely to form. Dilators can widen blood vessels to make room for more blood to flow. Doctors can also use a procedure called catheterization to look at the blood vessels around the heart directly, place stents, or widen vessels using angioplasty. In extreme cases bypass surgery can be used to help blood flow better. After recovery, it is important to lead a healthy lifestyle and take any prescribed medications in order to protect the heart.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

Percutaneous Left Atrial Appendage Closure (LAAC)

Percutaneous left atrial appendage closure (LAAC) is a device implantation procedure that reduces the risk of stroke for patients who have atrial fibrillation, a condition in which the heart beats out of rhythm. Atrial fibrillation causes blood to collect in the left atrial appendage, a small sac located in the top left chamber of the heart. The blood that collects in this appendage can form clots and cause stroke when pumped out of the heart.

To implant the device, a long, flexible tube, called a catheter, is inserted into a large vein in the groin and advanced to the heart. Once the tube reaches the left side of the heart, X-ray is used to guide a thinner catheter into the left atrial appendage. The tiny device is then passed through the tube and into the appendage. When the doctor has made sure it is in the right place, she takes the catheter out, and the procedure is completed.

This minimally invasive procedure usually requires a hospital stay of at least one day. Normal activities may be resumed within a few days. About 45 days following implantation, a test will be done to determine whether the device has closed the left atrial appendage. Check-ups have to be performed every year to make sure the device is in place.

Dr. Keiko Aikawa is an EXCEPTIONAL cardiologist!!!!! She Is smart, extremely friendly and nice, listens to me, works with me (I have some difficulty tolerating medications and she is always ready to try something else or tweak an existing medication in a different way). You need to add more numbers to your evaluation because I would give her not just a 10, but a 100+!! As I say every year--I wish you could clone her!!!!! Her assistant is also fabulous. I am always nervous when I come in (I have heart disease, after all) but they are a great team!!

I met Dr. Aikawa in 2018 when my primary care doc advised me to have an EKG after I reported I was having new issues walking up hills in my neighborhood. A result of that appointment showed my heart lacked a pacing function, which signaled a need to correction. Pacemaker installed and Dr. Aikawa's office followed the issues with my heart, learned my aortic valve wasn't functioning correctly, which led to my first surgery. She has been my trusted advisor and supportive doctor since that time; I believe that discovery saved my life.

She listened carefully and asked relevant questions. I think she is knowledgeable and will provide excellent advice. I only wish that I could get the tests she ordered in a more timely manner. When facing cardiac problems it's hard to wait over a month to get diagnostic tests done and it will be 2 months from the date of my incident before I will be able to see her again after the tests. That being said, she has been very responsive and has read tests results over the weekend and night and sent notes. I really appreciate that.

This doctor seemed rushed and was not the cardiologist I have seen before for my yearly check up since he has left the practice. I was glad to get a quick appointment but she did not get some of my information correct that had bearing on my questions. This was my first time seeing this doctor, and I believe she sort of squeezed me in since I needed to see a cardiologist to be able to proceed with knee surgery. Sadly, with the holidays and the late notice that I would need this visit, even though I felt the appointment was timely, my surgery date had to be pushed out. But I am very happy to now have a cardiologist to call my own... Ha! I would be very happy to see this doctor again.

Doctor has always helped me. When I first started seeing her, I needed her help. She cared about me, and she had sent me to a specialist when the medication she tried on me that didn't work. She had been conferring with him all the time before she sent me to see him. The medication they did decided on was best for me has worked for many years. This visit was my yearly check up.

Doctor's special assistant was especially helpful also. She explained well, as she went about her duties. Personable too as we talked of similarities in her Son's excellent athlete activities and others. One of the Doctor's Directives was to increase health giving cardiovascular athlete activities. Her special assistant augmented this. Most Excellent Team.

Excellent. Had a concern about blood pressure/pulse/heart. Was concerned about stroke or other heart issue. Was able to get into the doctor quickly. She's is knowledgeable about me and my situation. Was able to ask intelligent and as near as I can determine, not being a physician myself, relevant questions. Excellent attention.

I did appreciate that she took the time to see me and I really did feel like she saw and heard me. I have much anxiety about doctors and she really did seem to understand that and I felt that she was trying hard to ease my anxiety during that visit. I am very grateful that she is able to help me with my health concerns.

Dr. Aikawa usually is friendly and respectful. This time she appeared stressed and hurried and yelled at me when I asked her what my last two blood pressure readings were. I did not feel respected by her and she seemed irritated by everything about my visit. I am considering limiting my visits to her in the future.
