Autoimmune Diseases

An autoimmune disorder happens when the immune system mistakenly attacks the tissues of its own body, causing symptoms of illness. There are more than 80 different types of autoimmune disorders. While some are very rare, others are fairly common. Combined, autoimmune disorders are one of the leading causes of death and disability in the United States, affecting approximately 24 million people.

A properly working immune system identifies foreign substances in the body that might cause illness, such as bacteria and viruses. The immune system then creates antibodies which attack the foreign substances, neutralizing them and keeping the body safe. In people with autoimmune disorders, something goes wrong with this process. For reasons that are not fully understand, the immune system creates antibodies to attack the patient's own tissues.

Symptoms of an autoimmune disorder depend on which tissue is being attacked by the immune system, but common symptoms of autoimmune disease include fever, fatigue, and a general feeling of just not being well. Autoimmune disorders are more common in women than in men, and they may run in families. Autoimmune disorders can affect various parts of the body such as blood vessels, connective tissue, endocrine glands, joints, muscles, red blood cells, skin, and many others.

It is common to have more than one autoimmune disorder at a time. Most are chronic, or life-long illnesses, although they may come and go in flares. Treatment for autoimmune disorders depends on which part of the body is being attacked. For example:

  • A type 1 diabetic whose pancreas has been damaged will need insulin.
  • A person with Hashimoto's whose thyroid has been damaged will need replacement thyroid hormones.
  • Someone with Sjogren's syndrome will need eye drops and mouth rinses to replace tears and saliva.

Many autoimmune disorders of all kinds are treated with immune-suppressing medications, such as corticosteroids (e.g. prednisone) to reduce the effect of the immune system.

Heart Attack

A heart attack is a serious medical event that happens when the blood supplying oxygen to the heart is cut off, usually because of a blockage in an artery. This results in damage or death of part of the heart muscle. The heart can heal, but like any part of the body that was injured it leaves a scar. This scar tissue does not contract effectively, so the ability of the heart to pump is lowered after a heart attack.

A heart attack, sometimes called a myocardial infarction, can happen without warning. However there are certain factors that raise the risk of heart attack. Some of these are:

  • Age
  • Gender (most heart attack sufferers are male)
  • A family history of heart attacks
  • Smoking
  • Obesity
  • High cholesterol
  • High blood pressure
  • Diabetes
  • Unhealthy lifestyle choices (lack of exercise, poor diet, too much alcohol or stress)

Heart attacks typically feel like pain or pressure in the chest that can radiate out to the back, arm, or jaw. Some other symptoms can include fatigue or weakness, shortness of breath, sweating, and nausea.

Treatment aims to increase blood flow to the heart muscle by opening up blocked blood vessels. Medications are sometimes used to dissolve clots in the arteries. Aspirin and other medications such as Plavix can thin the blood and make clots less likely to form. Dilators can widen blood vessels to make room for more blood to flow. Doctors can also use a procedure called catheterization to look at the blood vessels around the heart directly, place stents, or widen vessels using angioplasty. In extreme cases bypass surgery can be used to help blood flow better. After recovery, it is important to lead a healthy lifestyle and take any prescribed medications in order to protect the heart.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

Dr. Daniel Goodwin has been my Cardiologist for several years, beginning when I was hospitalized with a severe infection that had affected my heart and other internal organs. With his expert care my heart functioning has vastly improved, although I still have some issues, but they are age related, since I will be 82 in a few months. He continues to provide excellent care, and he recently prescribed a relatively new heart medication that he believes will be beneficial for my condition. Based on my personal experience, I would highly recommend Dr. Goodwin to anyone seeking the care of a Cardiologist.

The nurses at the Providence Heart Clinic - Bridgeport are fantastic. I have messaged Dr. Goodwin's team numerous times (through My Chart) seeking help with lapsing prescriptions, providing health updates and asking for medical advice, and seeking help with referrals. They have been incredibly responsive and helpful. I can't say enough about the professionalism of the nurse team at the Heart Clinic. Thank you!

This doctor is not my regular care doctor but my cardiologist and he is very thorough and patient. He let me talk a lot and he listened and then told me all the things to do. He changed my meds and is waiting for e-rays ordered and set another appointment after he gets the result of the x-rays. He is very helpful and even called in a new prescription before I left the office. I am very pleased to have him.

This was my 3rd visit with my cardiologist in the past 2 yrs. All visits were excellent due to the careful explanation of my condition, options of follow up care, including my input in the health plans and completeness of explaining my options and potential consequences of these options. I always feel as though my concerns are taken seriously and have never had a bad experience with any staff member I have encountered, from the front desk personnel, the care nurses, physician assistants, and the physician I am seeing. They do get a little carried away with all the "have you experienced" questions, but I understand the necessity.

I really like Dr. Goodwin and he is very Informed and compassionate. The appointment itself was very helpful. It was hard getting a time to see him and I waited several months (it was not an emergency but I wanted his opinion on my meds and treatment). Obviously the wait was not his fault at all and I am grateful for his time and care.
