Highly recommend NOT using this doctor/office.  They do not call in/request scans/ultrasounds/physical therapy when they say they're going to do so.  They schedule follow up appointments to see how physical therapy went when they never even sent in the request for the therapy.  The doctor/staff do not follow through on what they say they're going to do for you.
