Benign Prostatic Hyperplasia (Enlarged Prostate)

Benign prostatic hyperplasia (BPH) is a noncancerous condition in which the prostate gland becomes enlarged. BPH affects about half of men between the ages of 50 and 60, and approximately 80% of men over 80. As the prostate grows in size, it can press down on the tube where urine flows out of the body (the urethra) and cause urinary problems.

Medication can relieve mild to moderate symptoms of BPH, such as frequent urination, incomplete bladder emptying, a weak urine stream, and straining while urinating. However, other forms of treatment may be more appropriate if an individual has pain with urination, frequent urinary tract infections, or is unable to urinate. The size of the prostate and the severity of their symptoms will determine the type of treatment they need. If the prostate is not very large, doctors will likely recommend a transurethral procedure. This minimally invasive technique involves the insertion of a scope into the urethra. The most common transurethral procedures for BPH are:

Transurethral incision of the prostate or TUIP, where the surgeon uses the scope to make small cuts in the area of the prostate that meets the bladder. These cuts will open up the pathway for urine and allow it to flow with more ease.
Transurethral needle ablation or TUNA, where needles are inserted through the scope and into the prostate. The needles use radiofrequency waves to destroy excess prostate tissue.
Laser prostate ablation, where lasers pass through the scope to melt away the part of the prostate causing urine blockage.
Transurethral resection of the prostate or TURP, where the inside of the prostate is trimmed and removed one tiny piece at a time using the scope.

If the prostate is too big for a transurethral procedure, an individual may need a simple prostatectomy, which can be done in one of three ways: laparoscopic, robotic, or open. During a laparoscopic simple prostatectomy, a surgeon makes several small incisions on the belly. Then they insert a long tube with a camera into one of the cuts and surgical instruments into the others. Using the camera to see inside the belly, they carefully removes the enlarged part of the prostate. Robotic simple prostatectomy uses the same techniques as the laparoscopic method, but the surgery is done with the help of a robot. For men with very large prostates, open simple prostatectomy may be the best treatment option. This surgery differs from the other approaches in that it requires a much larger incision.

Most transurethral treatments for BPH, like TUIP, TUNA, and laser prostate ablation, are done in the doctor's office or outpatient surgery center. TURP and simple prostatectomy, however, need to be performed in the hospital and require an average stay of one to three days. Patients should wait a week before doing any strenuous activities after a TUIP, TUNA, or laser prostate ablation, and about four to six weeks after a TURP or simple prostatectomy. Although these treatments improve BPH symptoms for most patients, it is important to be aware of the risks involved, such as urine control issues, tightening of the urethra, and erectile dysfunction.

Pelvic Organ Prolapse

Pelvic organ prolapse is the bulging of pelvic organs into the vaginal canal due to a weak pelvic floor, causing symptoms like discomfort, pain, urinary problems, and constipation. The pelvic floor is a group of muscles that support the pelvic organs, which are made up of the bladder, rectum, small bowel, uterus, and vagina. Vaginal childbirth, menopause, pelvic surgery, radiation treatments, or being extremely overweight may cause the pelvic floor to weaken. Pelvic organ prolapse is a very common disorder among women, particularly for those who are over 50.

The choice of treatment for pelvic organ prolapse depends on the severity of the condition. Mild to moderate prolapse may be managed by doing pelvic floor strengthening exercises, taking hormone replacement therapy, or using pessaries. Pessaries are small devices inserted into the vagina to help support the pelvic organs. Pessary fitting is a quick procedure done in a doctor's office. For severe prolapse, surgical treatment may be necessary. Pelvic organ prolapse surgery repairs the following:

Cystocele or bladder prolapse, where the bladder sags into the vagina. The surgeon repairs a cystocele by strengthening the front wall of the vagina using stitches so that the bladder will no longer sag.
Enterocele or small bowel prolapse, which occurs when the small bowel descends towards the vagina. To correct an enterocele, the tissues between the small bowel and the vagina are sewn together, allowing them to provide more support.
Rectocele or posterior prolapse, which is the bulging of the front wall of the rectum into the back wall of the vagina. Surgery to repair a rectocele removes the excess stretched tissue between the rectum and vagina.
Uterine prolapse, which occurs when the uterus slips down to the vagina. To treat a uterine prolapse, the surgeon may secure the neck of the uterus (called cervix) to a ligament in the pelvis using stitches. Another option for this type of prolapse is a vaginal hysterectomy, which is the removal of the uterus.

Each of these procedures is performed through a vaginal incision, but an enterocele repair may use the abdominal approach as an alternative. If two or more pelvic organs have prolapsed, a combination of any of these procedures may be performed during the same surgical session.

For women who no longer wish to have intercourse, an operation called colpocleisis

may be a surgical option as well. By closing the vaginal canal, colpocleisis treats pelvic organ prolapse and eliminates any risk of its recurrence.

In most instances, pelvic organ prolapse surgery only requires a hospital stay of one day. Patients may experience vaginal bleeding for the first few weeks following surgery. If this happens, patients should use sanitary pads rather than tampons, as a higher risk of infection is associated with the use of tampons. To further help with recovery, patients should begin doing gentle pelvic floor strengthening exercises a few days after surgery. Doing these exercises at least three times a day as a routine tightens the pelvic floor muscles and prevents the recurrence of prolapse.

Shock Wave Lithotripsy

Stones are formed when mineral deposits build up and harden in an organ or gland. For example, urine contains minerals that can form crystals and result in kidney or bladder stones. Stones can also occur in other parts of the body, like the mouth. Salivary gland stones form when the chemicals in the saliva harden and get stuck in the tiny ducts of the salivary glands. Pain and swelling are the most common symptoms associated with stones.

Lithotripsy is a procedure used to break up small stones. Shock wave lithotripsy, the most common type of lithotripsy and least invasive way of treating stones, does this using high-powered sound waves. This procedure can treat many types of stones in the body, but it is most frequently used for urinary stones, such as kidney and bladder stones.

Shock wave lithotripsy can be done in one of two ways. Conventionally, this procedure is performed when someone is sitting in a tub of water, and the sound waves travel through the water to their body. A more recent alternative is to send the shock waves through cushions while a patient is lying on a table. Although shock wave lithotripsy is not surgery and requires no cutting, the patient will be given anesthesia before the procedure. Their body should be able to pass the broken-up urinary stones naturally in their urine after treatment. For other types of stones, another minimally invasive procedure may be required to remove stone fragments left by shock wave lithotripsy.

Wasted my time. Excessive billing. Had to pay for a full Dr visit to get a copy of my test results. Test results that I already paid for, after I specifically told the office on several occasions I did not need the Dr to interpret them for me since these tests had already been run by pcp and he had the results, spears wanted to run them himself. I did not need the same results interpreted twice. 5-7 minutes of actual time with Dr in office, literally read results off of page and handed them to me.
