Autoimmune Diseases

An autoimmune disorder happens when the immune system mistakenly attacks the tissues of its own body, causing symptoms of illness. There are more than 80 different types of autoimmune disorders. While some are very rare, others are fairly common. Combined, autoimmune disorders are one of the leading causes of death and disability in the United States, affecting approximately 24 million people.

A properly working immune system identifies foreign substances in the body that might cause illness, such as bacteria and viruses. The immune system then creates antibodies which attack the foreign substances, neutralizing them and keeping the body safe. In people with autoimmune disorders, something goes wrong with this process. For reasons that are not fully understand, the immune system creates antibodies to attack the patient's own tissues.

Symptoms of an autoimmune disorder depend on which tissue is being attacked by the immune system, but common symptoms of autoimmune disease include fever, fatigue, and a general feeling of just not being well. Autoimmune disorders are more common in women than in men, and they may run in families. Autoimmune disorders can affect various parts of the body such as blood vessels, connective tissue, endocrine glands, joints, muscles, red blood cells, skin, and many others.

It is common to have more than one autoimmune disorder at a time. Most are chronic, or life-long illnesses, although they may come and go in flares. Treatment for autoimmune disorders depends on which part of the body is being attacked. For example:

A type 1 diabetic whose pancreas has been damaged will need insulin.
A person with Hashimoto's whose thyroid has been damaged will need replacement thyroid hormones.
Someone with Sjogren's syndrome will need eye drops and mouth rinses to replace tears and saliva.

Many autoimmune disorders of all kinds are treated with immune-suppressing medications, such as corticosteroids (e.g. prednisone) to reduce the effect of the immune system.

Heart Attack

A heart attack is a serious medical event that happens when the blood supplying oxygen to the heart is cut off, usually because of a blockage in an artery. This results in damage or death of part of the heart muscle. The heart can heal, but like any part of the body that was injured it leaves a scar. This scar tissue does not contract effectively, so the ability of the heart to pump is lowered after a heart attack.

A heart attack, sometimes called a myocardial infarction, can happen without warning. However there are certain factors that raise the risk of heart attack. Some of these are:

Age
Gender (most heart attack sufferers are male)
A family history of heart attacks
Smoking
Obesity
High cholesterol
High blood pressure
Diabetes
Unhealthy lifestyle choices (lack of exercise, poor diet, too much alcohol or stress)

Heart attacks typically feel like pain or pressure in the chest that can radiate out to the back, arm, or jaw. Some other symptoms can include fatigue or weakness, shortness of breath, sweating, and nausea.

Treatment aims to increase blood flow to the heart muscle by opening up blocked blood vessels. Medications are sometimes used to dissolve clots in the arteries. Aspirin and other medications such as Plavix can thin the blood and make clots less likely to form. Dilators can widen blood vessels to make room for more blood to flow. Doctors can also use a procedure called catheterization to look at the blood vessels around the heart directly, place stents, or widen vessels using angioplasty. In extreme cases bypass surgery can be used to help blood flow better. After recovery, it is important to lead a healthy lifestyle and take any prescribed medications in order to protect the heart.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

My cardiologist and his team are always professional, kind and concerned about me as a patient and as a person. They take care to be clear and thorough in whatever tasks the appointment involves. One of their strongest measures is the way each appointments tasks (i.e. Blood pressures, EKG's, Echocardiograms, weight, written summaries, etc.) are carried out evenly and equally by different members of the team. In this united but diverse effort, they are stellar and efficient. The only task that could use some improvement is the checking in process on Mondays. That day seems to be more cumbersome, chaotic and slow from the very first check-in downstairs at the main desk; which takes an average of 15 minutes longer than any other day of the week. Apparently, this is the day for new trainees. Wednesday through Fridays, in the afternoons are the smoothest and run like clock work. Once you get upstairs to Cardiology, their staff is quick and competent Thank you for every effort that is made to make SMMC a more cohesive echelon of care. A job well done !

I have eight years of care from Dr. Wong. He always had time to spend with me, always made me feel as if I was his only patient. Always came prepared with my history. On this visit he was in a big hurry, seemed not totally focused, and did not come prepared as he always was before. Only spent about 8 minutes and then rushed out. I really like Dr. Wong. At this visit I left with the impression that he was under a lot of pressure, seemed overwhelmed, and just didn't have enough time to spend with me. He literally rushed out. As he was rushing out he said he hoped I did not feel like he was ignoring me. I am a retired corporate executive. He showed all the signs of being overwhelmed with work to do. He had appropriate tests recommended for me, but it appeared he did not have time to give me the quality care time that he had on my visits.

Dr. Wong is kind and caring. I know everyone Is busy, and that I have a very long and complicated chart. It would be helpful, however, if people--including the doctor--were to review my chart before my visits so that procedures and tests are fresh in the mind for us all. It seems things were forgotten and had to be quickly looked up during my visit. I have done everything ordered within the best of my abilities, but wasn't aware I needed fasting bloodwork done before my appointment. It may have been on my after-visit paperwork some time ago, but it would have been nice to have been reminded of the bloodwork before the appointment, as I cannot always find the paperwork.

My visit was scheduled with a different provider however, she called in sick and at first I was upset because I came all the way from Chicago just for this appointment. I received a text message a few hours earlier saying that they're excited to see me and look forward to our visit. So, when the staff heard all this they told Dr. WONG and he said that he would go ahead and see me. I was happy to hear this and appreciated his commitment to his patient. I would've been very disappointed if I was turned away without seeing anyone, especially when I cut my vacation short and traveled on a train for 2 days. All went well and I went home a happy camper. Thank you!

13 years ago I moved to Central Oregon High Desert. It is at least 3 hours from my home to Dr. WONG'S OFFICE. Dr. Wong Is an excellent doctor. He listens, he explains things clearly and it is very obvious that he care greatly for each patient. Respect and professionalism is always a part of his work. Because he is so trustworthy I have chosen to continue retaining Dr. Wong as my heart specialist. Because of this choice, I have driven to Walla Walla, WA, for all of my appointments and follow-ups. Each trip has required a total of 6 hours for traveling. I'd do it again in a heart beat.A huge Thanks and Bravo for your top notch Doctor.

The appointments for any of the providers I saw were easy to make, but the wait time until the physicians were extended.Dr. Wonsuwong was very nice & SEEMED to listen well to my responses to his questions, but some of what I said was opposite of what he put in the chart. I sent a message about this in My Chart & I received a message back that he noted my comment. I hope this means he corrected the errors. Otherwise, my visit to him was very pleasant & informative. He was very patient & reviewed with me the parts of my charted history appropriate to this visit. All of my questions were answered.

I appreciate Dr. WONG so very much. He has: high ethical standards, very personable, if a problem arises he studies the problem, studies it out, then resolves the problem. I have high respect and trust in Dr. Wong. Because of his medical stadards, since I moved from Walla Walla 12 years ago, I still choose to drive 3 hours one way and then back home 3 hours just because I know he does his job right. I am most thankful for Dr. WONG. I would, and do, trust my life in his outstanding medical care. Thanks is not a sufficient word.

This was a required preop visit that was needed for scheduled surgery. I felt very fortunate to be able to get in as fast as I could. Dr wong and his staff were thorough, complete and interested in my care. His RN in particular was most helpful, sharing info about meds, schedules, plan for care abd she followed up w a phone call to report lab findings particular to newly prescribed medication. The whole visit was the most positive and helpful to my personal care that I've experienced.

This visit, I was a new patient in a specialty area. It took a while for an available appointment for a new patient to open, but I was called as soon as one opened. I let the scheduler know I was retired and available early if they had an earlier cancelation, which is what happened. I was called and was able to come in two weeks earlier. At the appt. Dr. Wong answered all my questions patiently, as well as him asking many of his own. I found him very caring and very knowledgeable.

I was very pleased when Dr Wong offered to write a letter to medical team regarding my fitness for major surgery next week from his cardiology perspective. Both anesthesiologist & surgical teams read over his notes from this visit and felt reassured about what they proposed for their treatment. It was timely, detailed & succinct; and everybody was amused (commented on) that he referred to my regular pickleball play/practice.
