Thyroid Problems

Thyroid problems are a group of conditions that alter hormone regulation and metabolism. The thyroid is a small, butterfly-shaped gland located in the lower neck, just below the Adam's apple (laryngeal prominence) and larynx (voice box). Despite its small size, the thyroid has an outsized effect on the body by regulating hormones that control metabolism. Metabolism is the body's process of converting consumed food into fuel to power the entire body. Heart rate, weight, cholesterol, body temperature, and even physical growth and development are all affected by the metabolism.

When the thyroid malfunctions, the body is unable to properly process and allocate energy. Thyroid problems, such as hyperthyroidism (overactive thyroid) and hypothyroidism (underactive thyroid), cause irregular metabolism. In some cases, the body produces antibodies that attack the thyroid cells for unknown reasons. The damaged thyroid responds by releasing either too little or too much T3 and T4 thyroid hormones. Thyroid malfunctions may also arise from genetic causes or iodine deficiency. Thyroid problems may include:

Hashimoto's thyroiditis, an autoimmune disease that causes hypothyroidism
Graves' disease, an autoimmune disease that causes hyperthyroidism
Goiter
Thyroid nodule
Thyroid cancer

Hashimoto's is the leading cause of hypothyroidism, which is when the thyroid does not produce enough hormones to stimulate adequate metabolism. The condition is diagnosed by blood tests that measure TSH (thyroid stimulating hormone) and detect Hashimoto's antibodies. Symptoms of hypothyroidism include sluggish metabolism, inflammation, fatigue, weight gain, brittle hair, dry skin, and sensitivity to cold. To supplement impaired thyroid function, patients with Hashimoto's are typically prescribed synthetic thyroid hormone pills. Such patients may need to consume synthetic thyroids for the rest of their lives. Other treatments may include dietary changes, such as the avoidance of certain foods and medication to reduce inflammation. Frequent blood tests can inform patients if their TSH levels are within a normal range.

Grave's disease is most frequently the cause of hyperthyroidism, which is when the thyroid produces too many hormones, overstimulating one's metabolism. The condition is also diagnosed by blood tests that measure TSH and detect Grave's antibodies. Elevated heart rate, high blood pressure, sweating, bulging eyes, and weight loss are common symptoms of hyperthyroidism. Treatments for hyperthyroidism include oral radioactive iodine, which slowly shrinks the thyroid, slowing the production of excess thyroid hormone. Other anti-thyroid medications such as methimazole (Tapazole) also reduce the production of thyroid hormones. Blood pressure medications known as beta blockers can ease the increased heart rate caused by hyperthyroidism. If treatments are unsuccessful, patients may be prescribed a thyroidectomy, or surgery to remove part or all of the thyroid. Some patients with hyperthyroidism may require ophthalmological (eye medicine) treatments such as eye drops, prednisone, and surgery to reduce eye swelling and bulging.

Untreated thyroid disorders frequently lead to goiters or thyroid nodules. A thyroid goiter is a large lump that can be felt near the base of the neck and is made of swollen thyroid tissue or nodules (excess cell growth). Nodules and goiters are diagnosed by CT scan or ultrasound. Obstructive goiters can cause pain, coughing, and abnormal breathing. The most common cause of goiters is hyperthyroidism (overactive thyroid). However, Hashimoto's is also known to cause goiters or nodules. Goiters develop from continued damage from antibodies to the thyroid over time. At times, a goiter can result from a cancerous thyroid tumor. Most goiters and nodules are themselves benign.

The exact causes of thyroid cancer are unknown. People who develop thyroid cancer may or may not have another thyroid problem such as Hashimoto's or Grave's disease. Symptoms of thyroid cancer include a nodule that can be felt on the neck, hoarseness, difficulty swallowing, swelling in the neck, and neck and throat pain. CT and MRI scans are used to diagnose thyroid cancer. There are four types of thyroid cancer which vary in intensity. Thyroidectomy (thyroid removal surgery) is usually prescribed to treat thyroid cancer.

Thyroid problems are often long-term conditions that require some form of treatment. With regular monitoring and medication, many people with thyroid problems are able to lead active and fulfilling lives.

Dr. Kunkle has always been on top of my conditions and every adjustment has worked well. He also has seen my youngest son who is now 21 years old, after seeing several different doctors since he was 15 years old. He had gotten to the point that he wouldn't leave the house except for school. After graduation from high school my son got to the point he wouldn't leave his room and struggled with suicidal thoughts. I suggested he see Dr. Kunkle because he listens carefully and understands the body and development better than any endocrinologist I've ever seen. Dr. Kunkle, on the first visit ordered a blood test that diagnosed my son with adrenal hyperplasia. Since putting my son on medication he is a totally different young man. His condition usually is caught at birth but for some reason wasn't, also every other doctor he'd seen over the previous eight years missed this diagnosis. Not only with me, but also more importantly my son, Dr. Kunkle has made the difference in our quality of life. In my sons life he most likely saved him from suicide as he was at the end of his preverbal rope.

Dr Kunkel is my Endocrinologist, who this survey initially was intended for. He's a very good endocrinologist, and works well with my thought process. I only go to him for this condition of thyroid and pre-diabetes..Dr. Kim is my primary care doctor. She's a very good primary care doctor. I only go to her for primary care and I was placed with her from other primary care doctors who could not properly treat me according to the clinic, so I was changed. She is an internist and works well with my thought process. I am well aware of the fact that I can be a challenging patien questions and need to understand the thought process of any doctor that treats me. I have a recent degree in medical records with research, so I am very well aware of what I don't know and look to my doctor to tell me and explain to me through what they do know.

Dr. Kunkle is an outstanding extraordinary Dr. One of the best that I've ever had the pleasure to visit. He listens with compassion and concerns. Reviewed my chart thoroughly before my first visit as well as test results each time. He Listens Intently and answers questions in a way that is highly understandable. His wisdom and desire in treating causes instead of symptoms is appreciated. I wished more Dr's had this believe. My only regrets is that he or someone just like him was my PCP. He's that type of Dr that you look forward to coming to as he is trustworthy and thorough with the best possible health care. And he is kind..He has helped wirh my diagnosis and longterm care.

I've been extremely ill since October 2024, likely due to the massive amount of drugs I was prescribed during hospital visit. My PCP did not help with my concerns. I was lucky Dr. Kunkle had an unexpected opening. (I had originally been scheduled for March 2025). I was so sick I could barely stand, leave the house, breathe... Dr. Kunkle listened to me and changed my meds (mostly heart related). I felt better almost immediately. I feel like I lost 3 months of my life before he helped me. Still have to try to get a cardiologist to see me, but for now I'm a functional person.

Dr.Kunkel was absolutely amazing! It was my first visit with him. He was friendly, empathetic, sympathetic and highly knowledgeable. He had reviewed my information from the other Dr. Surgeon and test results. He explained things very well. He also followed up with the speed of lightning to my bloodwork test results getting ahold of me. He also immediately responded in my K chart to a couple questions I had in the evening. He is truly amazing! All Drs. Should be like him. He deserves some sort of award for patient care and knowledge.

Dr Kunkel and staff are one reason why I stay with Kadlec rn. If it wasn't for him trying to push for testing that he can order, as he ran into roadblocks trying to order genetic testing (that I've needed since 2001 when your OBGYN department completely dropped the ball!) I'd be an even more limbo than I am right now as your patient, Kadlec. Dr Kunkel is a caring, passionate, and very knowledgeable doctor that I appreciate very much the extent he tries to help his patients!

I see Dr. Kunkel every 3 months for my thyroid, his office is my favorite Kadlec Clinic the front desk are always cheerful, MA's are wonderful and personable, Dr. Kunkel is very knowledgeable and always answers my questions and listens to my progress and informs me of what steps we are next or staying with current thyroid management plan we are currently on. As far as scheduling goes I am scheduled by the MA before leaving and I keep that appointment

I adjust, started taking my insulin with pens and I was having a bit of difficulty. Dr. Kunkel took his time and explain how to use them so I get the full benefit and able to get a good nights sleep because he carefully explained what both of my insulin do with my blood sugar, and I am not to continue taking sugar drinks as often as I usually do. That was a big help, and I made a big change towards the bed because of the time he spent with me.

I have seen Dr Kunkel a few times now. He is a nice guy. He is responsive to messages very quickly. However, his ability to investigate issues is lacking. He will listen when I am telling him my symptoms, but does not ask any follow up questions. He just jumps to a response. When I messaged him about my labwork, he informed me I was having an inflammatory response. Then when I got to the clinic to follow up, he told me I was just getting old.

Dr. Kunkel and his staff always treat me with respect and patience...I was able to explain to them what was going on in my life regarding a critically ill daughter-in-law and how that has impacted my stress levels, etc. He llstened to me and gave me good advice. I would recommend Dr. Kunkel to anyone . He assists me with diabetes as well as thyroid issues following thyroid cancer. I feel extremely fortunate that he is my doctor.
