Cesarean Section (C-Section)

A Cesarean section (more often known as "C-Section") is a surgery performed during labor and delivery. A C-Section is a procedure that delivers a baby through an incision in the lower abdomen and uterus. In the United States, about a third of all births are performed by Cesarean section. Most often, a C-section is done by an obstetrician/gynecologist (OB-GYN), or in certain regions, a general surgeon or family physician.

C-section incisions can be up-and-down (vertical), from belly button to lower abdomen or side-to-side (horizontal), across the lower abdomen. Side-to-side incisions are more frequently used. After the exterior incisions are made, the doctor then cuts into the uterus and amniotic sac and delivers the infant and cuts the baby's umbilical cord. The mother receives medicine to expel the placenta and the doctor helps to safely remove it. Finally, the doctor uses sutures to seal the uterus and tissue incisions. The initial C-section skin incision is also closed with sutures or staples.

C-sections are typically performed with regional anesthesia, which inhibits pain in the lower half of the body, but allows mothers to be awake. However, some mothers may receive general anesthesia, where they are put to sleep during the surgical procedure.

C-sections may be planned or unplanned, depending on the circumstances of the birth.

Pre-existing conditions like heart disease, high blood pressure, herpes, and HIV may prompt a doctor to prescribe a C-section. Other situations for C-section include:

  • A large baby or birth to multiple children
  • Pelvic fracture or limited birth canal
  • Baby in the "breech position," where a baby is positioned feet or bottom first in the uterus
  • Baby with a developmental disorder
  • Irregular fetal heartbeat
  • Mother with a history of previous C-sections

Other mothers may require a C-section due to complications arising during delivery. These unplanned, emergency C-sections allow for a healthy birth even when complex complications are present. Problems during labor which may necessitate a C-section include:

  • Labor not progressing
  • Fetus switching position during labor
  • Distressed infant
  • Blockage of the cervical opening by the placenta
  • Prolapsed umbilical cord

Mothers should expect about a six-week recovery period following a C-section, including an initial hospital stay of two to three days. While in the hospital, mothers may spend time with their baby and begin breastfeeding. After returning home, mothers should limit strenuous activities such as lifting, reaching, and exercise, and should make sure to get plenty of rest.

Cryotherapy

Cryotherapy is the targeted use of extreme cold to destroy unhealthy or unwanted tissue. The doctor applies the cold on the skin or injects it through a needle to reach the inside of the body, where it freezes away the problem tissue. Cryotherapy is often used to remove minor skin problems such as warts or skin tags, and it is also used to treat some small tumors.

To achieve the cold temperatures needed for cryotherapy, liquid nitrogen or argon gas is passed through a tube, which becomes cold enough to freeze cells on contact. Alternatively, liquid nitrogen can be swabbed directly onto the skin or via a spray, which applies only a specific, small amount to the skin at a time.

Because the area treated is typically very small, cryotherapy is not usually painful. There may be a sensation of cold and possibly some slight discomfort. Cryotherapy is very fast and can easily be performed in a doctor's office. It also doesn't require stitches, because there is no cutting involved. These benefits make cryotherapy a good choice for many issues, including:

  • Common warts
  • Genital warts
  • Skin tags
  • Age spots
  • Some skin cancers
  • Cervical cancer
  • Prostate cancer
  • Retinoblastoma, cancer that occurs in the eye

Depending on the procedure, it usually takes three to four weeks to heal after cryotherapy. If it was done internally, patients may feel sore or have a small amount of swelling the first day afterwards. If cryotherapy was performed on the skin, patients will have a scab where their tissue was removed. Keep it clean and dry so it can heal.

Hysteroscopy

A hysteroscopy is a diagnostic procedure performed to examine the inside of the uterus. A thin tube with a camera and light at the end, called a hysteroscope, is inserted through the vagina. It allows the physician to view the cervix and uterine walls. Sometimes, gas or liquid is used to expand the uterus and allow a better view. If necessary, tiny instruments can also be passed through the tube and used for certain procedures, such as taking a biopsy or removing a polyp.

Hysteroscopy is often done to figure out the cause of abnormal menstrual bleeding. However, it can also be used to:

  • Identify the cause of recurrent miscarriage
  • Remove polyps or fibroids, non-cancerous growths in the uterus
  • Remove adhesions, scar tissue within the uterus
  • Identify uterine septums, a wall of tissue dividing the uterus
  • Perform ablation, the removal of excess uterine lining

If surgery is necessary, hysteroscopy is often used along with laparoscopy, a kind of surgery performed through small incisions in the abdomen. Tiny tools are passed through the incisions, and the hysteroscope guides the doctors while surgery is performed.

Went to the ER for a ruptured cyst and wasn’t getting any better so I decided to call my (so I thought) obgy provider for guidance since I was suppose to leave in two days for two weeks. I called three times, left three messages and finally received one call back but I missed the call. Didn’t hear back until four days later in mychart email portal to follow up. Her main nurse was so rude, not helpful and said I needed an appointment aka money to receive any guidance and yet the next available time was in two weeks!? Unacceptable the time it took to respond, and then her response lacked any respect and empathy. I think the doctor and staff are correlated. 

Side note prior - the doctor wasn't the friendliest, very dry and also told me to come back in a year even with high risk hpv.

Worst gynecologist I've ever been to. I made an appointment for my annual well woman visit and they set me up with Dr. Desai.  She asked why I was there and if I had any concerns. I answered her and she said she now had to charge me for asking a question.  When I asked for clarification on that she said we are already past the point since I already asked.  So don't ask any questions or tell her of any concerns.  She also told me I don't need a well woman visit every year, but every 5 years.  Totally incorrect.

This appointment was followed by up for MRI result which could be just some notes under MRI results but she didn't explain there. I scheduled virtual due to my work schedule, a day before appointment I got a call to come in person due to insurance issues! I arrived late because of work schedule! The provider didn't spend enough time because I was late! The verbiage she used was so professional and I wouldn't understand. I ended up reading the MRI result myself. To me this appointment did not help me to understand the result at all. It could have been very easier. I have to monitor my condition every year and I beed her to order me ultrasound otherwise I would have not go to her anymore. I feel they just wanted to charge me for a visit which was not necessary at all. All if this could be a note under the results, since it was no further action needed due to result!

The Provider failed to establish a connection with me, which left me feeling uncomfortable as they incessantly stared me down and fixated on my wedding rings. Furthermore, during our interaction, she interrupted me before I could fully respond to her initial question, only to ask me the same question again merely moments later. The entire visit felt careless, rushed and lacked any sense of warmth or personal connection. Adding to this, her humor was noticeably absent, which contributed to a rather tense atmosphere. Moreover, she did not present me with many options to consider, and her straightforward approach did not align with my preferences. I most certainly do not desire to have any future appointments with this provider.

I felt like this provider was unnecessarily brusque with the assisting nurse, and she also seemed to expect my procedure to fail despite my having gone through it several times previously without incident. Also, as a note I didn't find the provider's slow, incremental approach to be less uncomfortable than the quick method, it just was uncomfortable for longer. Overall, I was glad the team reached out to me to schedule the appointment, but I was confused because in mychart it just said new patient appointment and didn't mention the specific purpose of the appointment. There were no pre-appointment instructions which would have been helpful, which my PCP has provided in the past.

Overall, excellent experience. I trust the provider with my care and my surgery. My surgery is two full months out, unfortunately, but I am willing to wait to work with the provider. The one piece of feedback I have is regarding compassion and trauma-informed care: when someone expresses co cern for previous doctors' care, try to understand and respond with compassion. Minimizing patient experiences can have grave consequences for the patient. I, in particular, need reassurance, not judgement. So not defend other doctors out of loyalty over patient care. Period.

I'm fairly new to PacMed. I've been very impressed with the care team and all the physicians that are caring for me. They've made me feel comfortable at all my visits. My only concern is the staffing shortage for laboratory, but it is understandable, and I can work with it. I think I am a little more understanding than about these types of situations than the normal patient because I work within the organization. My overall experience has exceeded my expectations and I feel proud to recommend PacMed to family and friends seeking care.

My visit with Dr. Dasai lasted 3 minutes. She denied my request, which I understand, but my request for alternative was basically ignored. She suggested I see a dermatologist for something unrelated to dermatology. It was a gynecological question. I left with no good lead on where to turn. Plus it was difficult to make the appointment. Gyns are scarce. I had to wait about 8 weeks to get in for an appointment. Total waste of my time. Fortunately the doctor only had to waste 3 minutes.

This was a first time visit for a specific problem/ a referral . . . I was frustrated to have to wait so long to obtain an appointment to see her . . . I was also frustrated to not be able to contact her or her staff PRIOR to the appointment because I was a new patient, I had a question that I was not able to pursue . . .That said, once my appointment time came I was extremely pleased with the Doctor and the visit. Wonderful manner, professionalism and knowledge.

Dr. Desai demonstrated great compassion during my appointment. When my appointment was wrapping up another question came to mind and she sat back down to hear my question and respond. While this is a simple gesture, I appreciated it as I felt she was genuinely interested in my question and willing to give me her undivided attention, making sure all my concerns were addressed
