Anemia

Anemia is the most common blood disorder. It occurs when the body does not have enough red blood cells to carry oxygen to the tissues. Blood loss, insufficient red blood cell production, and red blood cell destruction are the three main causes of anemia. These issues may arise from such conditions as iron deficiency, heavy periods, ulcers, kidney disease, as well as some forms of cancer.

An individual with anemia will have the following symptoms: shortness of breath, weakness, dizziness, cold hands or feet, paleness, and irritability. Anemia is diagnosed when blood tests show low counts for hemoglobin, which is the protein responsible for carrying oxygen to different tissues throughout the body.

Treatments for anemia are dependent on the kind of anemia an individual has. For example, iron supplementation may correct iron deficiency anemia. For anemia caused by chronic kidney failure, chemotherapy treatments, and HIV/AIDS, a group of medications called erythropoiesis-stimulating agents may be prescribed.

Musculoskeletal Problems

The musculoskeletal system refers collectively to the bones, joints, ligaments, muscles, and nerves. It is what gives the body structure and movement. Because this system encompasses so much of the body, musculoskeletal problems are extremely varied and can happen almost anywhere. There are musculoskeletal problems that affect only the joints, those that affect the bones, those that affect tendons and ligaments, and those that can happen in any area of the body but cause pain and numbness.

Joint problems include bursitis and arthritis. Bursitis is the inflammation of a fluid-filled sac cushion on the outside of a joint, causing symptoms including pain and swelling. Arthritis is the inflammation and damage of a joint due to wear and tear or disease. There are several treatments for arthritis, but if the damage progresses far enough, joint replacement may be necessary. In joint replacement, a metal or plastic implant is surgically placed within the joint to make movement easier.

Bone problems include scoliosis, fractures, and osteoporosis. Scoliosis is a curvature of the spine, making the normally straight spine look like a 'C' or 'S.' It is treated with braces or surgery. Osteoporosis is a disease that causes bones to weaken and break easily. It's most common in women, especially older women. Fully half of all women over 65 have osteoporosis. Fractures are any break in the bone. They can be a simple crack or a severe and complicated shatter. Osteoporosis causes fractures in people who have it, but other causes are trauma (such as a fall) or overuse.

Tendon and ligament problems most often result from injury or overuse. Two good examples are ligament tears and tendonitis. The most common ligament tear is the ACL tear in the knee. This ligament supports and stabilizes the knee and is most often torn during sports activities. Tendonitis happens when a tendon, which connects muscles and bones together, becomes irritated and inflamed. This happens most often in older patients who push their bodies too far, leading to pain and swelling. Treatment for both tendon and ligament problems usually includes rest, ice, and supporting the area to let it heal.

Pain and numbness problems can be caused by overuse, disease, an injury, or a compressed nerve. Lower back pain and carpal tunnel syndrome are two of the most common conditions patients encounter. Lower back pain may be caused by sore muscles that have been overworked, or an injury to the disks separating the vertebrae in the spine. Lower back pain usually goes away on its own within a few days, but it may require medical treatment depending on the cause. Carpal tunnel syndrome occurs when the nerve at the base of the palm becomes trapped or pinched. Symptoms include tingling, pain, numbness, or weakness in the hand. Treatment involves rest and sometimes steroid injections.

Because the musculoskeletal system involves so much of the body, problems here can be incredibly varied in their type and severity. Some musculoskeletal problems will go away on their own or only require rest, while others may require medication, physical therapy, or even surgery.

Thrombocytopenia (Low Platelet Count)

Thrombocytopenia is a disorder where people have too few platelets. With too few platelets in the bloodstream, the blood is unable to properly clot. People with thrombocytopenia may experience prolonged external and internal bleeding. Large purple bruises (ecchymoses) and small red skin spots (petechiae) in the skin and mouth are common in those with thrombocytopenia. Other symptoms of the disorder include bleeding gums, blood in bodily fluids, and fatigue. Thrombocytopenia most often arises from a different condition or treatment, such as alcoholism, autoimmune disease, cancer treatments, viruses (such as Hepatitis C and HIV), bone marrow diseases (such as leukemia and aplastic anemia), enlarged spleen, and exposure to toxic chemicals. In rare cases, thrombocytopenia is inherited.

If a doctor suspects a patient has a thrombocytopenia, a complete blood count (CBC) or platelet count test helps determine the concentration of platelets in a person's blood at a given time. Thrombocytopenia can be diagnosed by hematologists using advanced tests on platelet samples. Flow cytometry tests detect abnormalities by evaluating the characteristics of a platelet sample using targeted lasers and lights.

Treatments for low platelet counts include steroids like Prednisone, that can assist with platelet production. If a patient's low platelet count is caused by spleen problems, spleen removal (splenectomy) may be recommended. To treat the prolonged bleeding caused by thrombocytopenia, doctors frequently prescribe Dempressin, a hormone that promotes blood clotting. Antifibrinolytic medications such as tranexamic acid may also aid in the promotion of clotting and the prevention of bleeding. For patients with severe platelet disorders (either low platelet count and dysfunctional platelets), blood transfusions may be prescribed. Platelet blood transfusions deliver donated platelets to patients intravenously, replenishing the volume of functioning platelets.

Patients can also modify their lifestyle to better manage their disorder and minimize complications. Patients with low blood counts should avoid blood-thinning medications, alcohol, and activities that could lead to injury. Many people with thrombocytopenia can live healthy and fulfilling lives with proper treatment and precautions.

Precise and professional yet warm and personable, Dr. Ahmed is the oncological embodiment of the proverb If all you have is a hammer, all you see are nails: When you walk into her office she has you in her tractor beam and she doesn't release you until she has surfaced every last, errant cell. She is a cancer hammer, laser-focused on finding every nail in your body. So whether you are facing cancer or you are gratefully putting it in your rearview mirror, you can have no more dedicated, thorough oncologist in your corner. But did I mention that she is just the loveliest, warmest person? So perhaps we should call her a Velvet Hammer.

+I am extremely satisfied with the care/treatment and counsel I receive through PacMed. I trust the doctors and nurses explicitly. My only concern is in the administrative area. I have experienced some disturbing situations such as scheduling of covid screening prior to my open heart surgery and inconsistent availability of INR testing staff. I have no problem with the doctors and nurses. They and their work are fantastic. The organizational system of PacMed could use a comprehensive review and upgrade.

I think Dr. Sharmila is an excellent care provider - she is very much invested in my well-being and listens to my concerns and helps me understand how I'm doing with the treatment plan. Dr. Sharmila is very knowledgeable about Breast cancer treatment and responds to my questions In a respectful manner which Is much appreciated. I realize that she may be late to my appt time, but I know it's because she is providing her patients the same respect she gives me when I'm with her.

Dr Ahmed is extremely thorough, clear, and kind. She is always up to date on all the latest technology related to cancer detection and shares these resources, their pros and cons, and their availability. She also is very straightforward about where I stand in my journey as a post-cancer patient and goes through the entire history each time. She is a truly top-notch provider.

Dr. Ahmed is my oncologist. From our first meeting I felt completely comfortable with her and her professional skills in treating cancer.I interviewed her with my boyfriend and female friend in the medical field. We all agreed she was a good match and we did not feel the need to look for another specialist. I have appreciate Dr. Ahmed for her bedside manner and care.

Dr. Sharmila Ahmed is an excellent provider. She is thorough and caring and has an excellent bedside manner. She listens and develops a plan of care with you and follows up on labs and changes in medications. She gives excellent homework on how to better overall health, including various exercises and activities to challenge my brain such as art, music, and cooking

Dr. Ahmed seems to really listen and take into account all the things you're going through. And she's literally only seeing me for my blood clot mystery. I really have appreciated her attention to detail when I bring up other medical concerns, how they might affect my overall health, and her upbeat personality.

Dr. Ahmed and her team are fabulous. I am 7 years out from my cancer and in remission. However, she continues to see me yearly and is prompt with my follow-up schedule. She always puts me at ease and I always feel safe and secure with her.I use MyChart to ask questions and her team always responds promptly.
