I have been a patient for several years and had some problems that were being watched because of family history.  When I first went I saw Dr. Dakil but have not seen him for at least a couple of years (going in to check PSA every 6 months).  When I requested to see the Dr. instead of a PA I was flatly told he would not see me.  I am paying the same for an office visit to see a PA, not a specialist.  When I demanded to see Dr. Dakil on the next visit with the understanding I would move to another urologist if he would not I received a phone call from his office telling me to find another urologist.  What a slap in the face!  He's  too busy doing surgeries and follow-ups to see me.  Yet he still charges the same for an office visit to see a PA.  I'm almost 60 and am trying to stay on top of things.  Beware of you want to see the doctor to get his expertise!  Guess you'll have to schedule a surgery.
