Attention Deficit Disorder (ADD/ADHD)

Attention Deficit Hyperactivity Disorder, or ADHD, is one of the most common disorders diagnosed in children. Nine percent of children in the USA have been diagnosed with ADHD. Symptoms, which include hyperactivity and difficulty maintaining focus or paying attention, can last into adulthood for some patients. Boys are four times as likely to be diagnosed as girls, although experts don't know why this is.

There are three main types of ADHD:

  • Predominantly hyperactive-impulsive types show symptoms such as constant fidgeting, talking non-stop, touching everything, having trouble sitting still and waiting their turn, and interrupting others.
  • Predominantly inattentive types tend to be distracted, get bored and wander off during tasks, forget things or instructions, daydream, and appear not to be listening. This type is sometimes misdiagnosed because they are not hyperactive and there are fewer behavior issues.
  • Combined types show symptoms from both categories. This is actually the most common type of ADHD.

When diagnosing ADHD, it is important to rule out other issues that may be causing the symptoms. Seizure disorders, hearing loss, anxiety, and domestic problems are some examples of problems that can cause behaviors similar to those seen with ADHD.

Treatment can include stimulant medications, behavioral therapy to teach patients ways to navigate their world and control symptoms better, and accommodations at school or work. A structured environment (with lots of organization and well-defined rules) seems to help most kids with ADHD function at their best.

ADHD is mainly thought of as a disorder that affects children, but symptoms can last into adulthood for a third to half of those diagnosed. In adults, ADHD has similar symptoms as when it is seen in children: impulsive behavior, difficulty maintaining focus, being easily distracted, or a tendency to fidget. These symptoms can cause problems in a patient's careers and relationships. Additionally, adults with ADHD are at increased risk for substance abuse. Medications can be an important part of treatment for adults, as well as stress reduction techniques and organization skills training. Specialized ADHD coaches can help adults with ADHD manage their symptoms and succeed in all areas of their life.

Benign Prostatic Hyperplasia (Enlarged Prostate)

Benign prostatic hyperplasia (BPH) is a noncancerous condition in which the prostate gland becomes enlarged. BPH affects about half of men between the ages of 50 and 60, and approximately 80% of men over 80. As the prostate grows in size, it can press down on the tube where urine flows out of the body (the urethra) and cause urinary problems.

Medication can relieve mild to moderate symptoms of BPH, such as frequent urination, incomplete bladder emptying, a weak urine stream, and straining while urinating. However, other forms of treatment may be more appropriate if an individual has pain with urination, frequent urinary tract infections, or is unable to urinate. The size of the prostate and the severity of their symptoms will determine the type of treatment they need. If the prostate is not very large, doctors will likely recommend a transurethral procedure. This minimally invasive technique involves the insertion of a scope into the urethra. The most common transurethral procedures for BPH are:

  • Transurethral incision of the prostate or TUIP, where the surgeon uses the scope to make small cuts in the area of the prostate that meets the bladder. These cuts will open up the pathway for urine and allow it to flow with more ease.
  • Transurethral needle ablation or TUNA, where needles are inserted through the scope and into the prostate. The needles use radiofrequency waves to destroy excess prostate tissue.
  • Laser prostate ablation, where lasers pass through the scope to melt away the part of the prostate causing urine blockage.
  • Transurethral resection of the prostate or TURP, where the inside of the prostate is trimmed and removed one tiny piece at a time using the scope.

If the prostate is too big for a transurethral procedure, an individual may need a simple prostatectomy, which can be done in one of three ways: laparoscopic, robotic, or open. During a laparoscopic simple prostatectomy, a surgeon makes several small incisions on the belly. Then they insert a long tube with a camera into one of the cuts and surgical instruments into the others. Using the camera to see inside the belly, they carefully removes the enlarged part of the prostate. Robotic simple prostatectomy uses the same techniques as the laparoscopic method, but the surgery is done with the help of a robot. For men with very large prostates, open simple prostatectomy may be the best treatment option. This surgery differs from the other approaches in that it requires a much larger incision.

Most transurethral treatments for BPH, like TUIP, TUNA, and laser prostate ablation, are done in the doctor's office or outpatient surgery center. TURP and simple prostatectomy, however, need to be performed in the hospital and require an average stay of one to three days. Patients should wait a week before doing any strenuous activities after a TUIP, TUNA, or laser prostate ablation, and about four to six weeks after a TURP or simple prostatectomy. Although these treatments improve BPH symptoms for most patients, it is important to be aware of the risks involved, such as urine control issues, tightening of the urethra, and erectile dysfunction.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

The staff at this office do not pass on urgent messages or tell you when you have credit on your account. Dr Khorrami referred me for surgery where one of the people in the operating room were out-of-network, despite me explicitly checking before the surgery took place. I received incorrect bills for over $30,000 which Dr Khorrami did not help with. Once those were resolved, I received two bills for over $1000 each for the out-of-network care that he had assured me I would not be subject to. These bills did not come from the surgery center, but directly from a collections company because they had not been given my address. It took from March 2019 to December 2019 to actually get office staff or Dr Khorrami to acknowledge the fault after leaving upward of 10 voicemails and being assured I would get a call back. I was asked to fax copies of my bills, but their fax machine was off and they refused to send an email address. I ended up booking a Dr appointment to see him face-to-face, at which point he called the surgery center and we discovered they had simply entered my insurance ID incorrectly, so this issue could have been 100% avoided and yet I had paid a huge amount of money. Not until January 2020 did I receive a partial reimbursement for the enormous out-of-pocket costs that I paid to collections to save from impacting my credit score, and I only received this because I chased all members concerned, taking time off work and getting advice from consultants and legal professionals. Despite Dr Khorrami insisting that his staff would put me straight through to him when I next called, I again had messages not passed on to him, and once I finally got him on the phone, he transferred me to a helpline and hung up. I am honestly appalled by the disregard shown to me by all members of Dr Khorrami's staff. Doctors do not need to understand how health insurance works, but to neglect these details to this extent and then refuse to help despite making explicit promises to fix the issue is extremely disrespectful. I have lost complete respect for Dr Khorrami, and am considering legal action.
