Dacryocystorhinostomy (DCR)

Dacryocystorhinostomy, or DCR, is a surgical procedure used to bypass a blocked tear duct.

Normally, the tears that constantly bathe the eyes are drained through two tiny holes in the eyelid, where they run through a duct into the nose. This tear duct can become clogged or blocked, causing tears to back up and leading to watery eyes. Common causes of tear duct blockages include trauma, a facial tumor, and inflammation due to certain medical conditions, but usually the cause is not known. Clogged tear ducts are especially common in young children. If the blockage is permanent and located lower in the duct, near the nose, DCR may be performed to create a bypass and allow tears to drain freely again.

During DCR surgery, a small hole is made in the bone and tissue between the upper part of the tear duct and the nasal passage. Usually, a tiny plastic tube called a stent is placed into the opening to protect it and keep it open during healing. DCR may be performed through an open incision on the upper cheek near the nose, or it may be performed endoscopically using tiny instruments that are passed through the nostril. Open surgery is more common, and has a slightly higher success rate, although endoscopic surgery causes less pain and bleeding during recovery. DCR is able to correct the blockage and watering eyes in nearly 95% of cases.

The surgery itself takes only about half an hour. It is usually done under general anesthesia, which may require a short hospital stay while patients recover. Small amounts of bleeding from the nose are not uncommon for the first 24 hours, and bruising and swelling are expected for the first two weeks. Don't be alarmed if the eye continues to water at first. Swelling in the area due to surgery can cause the eye to water, but this should go away as patients heal.

Facial Paralysis

Facial paralysis is an inability to move the muscles in the face. It often affects only one side, and it can happen quite suddenly. Facial paralysis may make one's face feel frozen or numb.

Facial paralysis can happen when there is inflammation or damage to either the part of the brain that controls the facial muscles or the nerve that carries signals from the brain to the face. Some common causes include:

  • Stroke, which can affect the part of the brain controlling the face and prevent those muscles from working
  • Bell's palsy, an inflammation of the main facial nerve
  • Lyme disease
  • Birth trauma or difficult birth, which can cause facial paralysis in babies (usually temporary)
  • Head trauma in which nerves are damaged

Facial paralysis is a frustrating and frightening experience, but many cases are temporary and go away on their own. Even with more difficult cases, there are treatment options available, including rehabilitation, pain management, and surgery.

Nasal Endoscopy

Nasal endoscopy is the use of a thin, flexible tube (called a 'scope') that is inserted into the nostril to examine the structures of the nose. The scope contains a fiber optic light and a camera that allow a physician to see much more of the nasal passages, airways, and sinuses than is possible with a standard nasal exam.

A nasal endoscopy is typically performed by an otolaryngologist (or ENT) in their office. It may be performed for a variety of reasons, but some common ones include:

  • Sinus infections
  • Nasal polyps
  • Frequent nosebleeds
  • Persistent stuffiness
  • Obstruction in the nose
  • Suspected tumor in the nasal passages

In most cases, a medication containing a decongestant and anesthetic is sprayed into the nose before the scope is inserted. The doctor uses the scope to examine any problem areas. If necessary, most scopes allow for tiny tools to be inserted and used to removed tissue or take samples. Nasal endoscopy is a very safe and relatively easy procedure.

Dr. Daneshrad is super helpful at helping with my sinuses. He has not pushed any products or services, but offers what can help and is very realistic with cost. While some things are expensive, he worked with me and gave me some great advice for the balloon procedure I am looking into this year. He also helped my friend who could not smell and she speaks so highly of him after Dr. Daneshrad was the only doctor to get her sense of smell working again and she is very happy.
