Adrenalectomy (Adrenal Surgery)

Adrenalectomy is a surgical procedure to remove the adrenal glands, which lie above each kidney. The adrenal glands are responsible for producing hormones that are necessary for bodily functions, such as the regulation of blood pressure and metabolism. If the adrenal glands overproduce certain hormones, or if a tumor grows on them, removal of the glands is a treatment option.

Adrenal surgery can be open or laparoscopic. Laparoscopic adrenalectomy is a minimally invasive procedure that inserts a thin tube with a camera (laparoscope) through several small incisions on the abdomen. The surgeon uses the laparoscope to guide the procedure and is able to access and remove adrenal glands through the small incisions. Laparoscopic adrenal surgery lasts approximately 1-2 hours.

Laparoscopic adrenalectomy is associated with shorter recovery periods, reduced risk of infection, and smaller scars. Although these factors may be desirable, the location of the tumor, its size, and the severity of the condition may require that an individual instead undergo open surgery. An open adrenalectomy is typically done for larger adrenal tumors and requires a single, large incision on the abdomen. Open adrenal surgery takes around 2-4 hours.

Following the operation, patients will remain in the hospital for 1-2 days for a laparoscopic adrenalectomy or 3-5 days for an open adrenalectomy. Patients will likely have a liquid diet immediately after their surgery, but should be able to return to solid foods after a day. If both of the glands are removed, patients will be given drug therapy to replace the hormones they produced.

Hysterectomy (Uterus Removal)

A hysterectomy is an extremely common surgery performed to remove the uterus. Sometimes the fallopian tubes, ovaries, or cervix will be removed as well. A woman who has had a hysterectomy will no longer have periods and cannot get pregnant.

There are a variety of reasons that women have hysterectomies, including:

  • Heavy bleeding or pain, such as that due to fibroids, endometriosis, or adenomyosis
  • Cancer
  • Prolapsed uterus, where the pelvic muscles can no longer completely hold the uterus in place

It is important to note that having any of these conditions does not necessarily mean that a hysterectomy is necessary. There are other available treatment options in most cases.

Although the uterus is responsible for a period, it is the ovaries that control the hormonal changes that women go through every month. So if a woman has a hysterectomy but keeps her ovaries, she might still experience hormonal swings every month even if she no longer has a period. Alternately, if a younger woman has a hysterectomy where her ovaries as removed, she will essentially be in immediate menopause.

A hysterectomy can be performed traditionally through one large cut in the abdomen, laparoscopically using tiny incisions and small tools, or through the vagina. Full recovery may take four to six weeks. After a hysterectomy, patients might experience sexual changes such as vaginal dryness or a change in libido. It is common to experience strong emotions after a hysterectomy, including both grief and relief. If a patient's ovaries were removed, they may be at higher risk for certain diseases, such as heart disease and osteoporosis. Taking hormonal birth control might reduce this risk.

Mastectomy (Breast Removal Surgery)

Mastectomy is the surgical removal of all or part of the breasts, most frequently associated with breast cancer treatment (for both male and female breast cancer patients). Breast cancer patients who receive a mastectomy typically have undergone other treatments, such as chemotherapy, radiation, and less invasive breast-conserving surgery (lumpectomy). If these treatments are unsuccessful, oncologists may eventually prescribe a mastectomy. Other conditions that may prompt a mastectomy include:

  • Two or more cancerous regions in the same breast
  • Inability to receive chemotherapy or radiation therapy
  • High likelihood of recurring breast cancer
  • Large tumor that is bigger than two inches across
  • Pregnancy that prohibits radiation therapy
  • Preexisting conditions like lupus or scleroderma

Notably, some patients may elect to undergo mastectomy without a breast cancer diagnosis. Preventative (prophylactic) mastectomy is performed on those with a high risk of breast cancer, such as having a family history of the disease or carrying the breast cancer BRCA1 or BRCA2 gene. Preventative mastectomy substantially reduces one's chances of developing breast cancer in the future, but it cannot wholly prevent it.

Surgical oncologists perform several types of mastectomies, depending upon the severity of the breast cancer (including factors such as its location and spread), as well as the patients' own aesthetic and personal choices. The six main types of mastectomy include:

  • Total mastectomy, where the surgical oncologist removes an entire breast, including parts like the pectoral chest muscle, skin, and nipple.
  • Radical mastectomy, where the surgical oncologist removes the entire breast, underarm lymph nodes, pectoral chest muscles, and outer skin and nipple. One of the most invasive forms of mastectomy, radical mastectomy is infrequently performed.
  • Modified radical mastectomy, where the surgical oncologist completes a total mastectomy and also removes the underarm lymph nodes. This procedure is slightly less invasive than radical mastectomy.
  • Skin-sparing mastectomy, where the surgical oncologist removes cancerous breast tissue and nipple, but leaves most of breast skin. This method is used in conjunction with breast reconstruction, as implants can be added below the skin. Skin-sparing mastectomy often requires a treatment team with a surgical oncologist and a plastic surgeon experienced in breast procedures.
  • Nipple-sparing mastectomy, where the surgical oncologist removes cancerous breast tissue and leaves the nipple and most of the breast skin. This method is only available to patients in the early stages of breast cancer. Similar to skin-sparing mastectomy, this procedure is frequently performed with breast reconstruction.

Any of these procedures can be done on both breasts, in which case the procedure would be known as a double mastectomy. For example, a surgical oncologist might perform a total or nipple-sparing mastectomy on both breasts. Double mastectomies are often used as a preventive surgery for those who are at risk of developing breast cancer later on in life.

Patients have one or two days of hospital stay before returning home from their mastectomy procedures. Full recovery can take several months, but some patients find they are able to resume normal activity within one month of their mastectomy. Patients are advised to limit arm and chest movements whenever possible.

After surgery, patients must also adjust to changes in their appearance. While some patients choose to receive breast reconstruction, other patients may decide to wear prosthetic breasts or will simply embrace their new chest.

Mastectomies permit patients to conquer cancer and lead healthy, thriving lives as breast cancer survivors.
