Pelvic Organ Prolapse

Pelvic organ prolapse is the bulging of pelvic organs into the vaginal canal due to a weak pelvic floor, causing symptoms like discomfort, pain, urinary problems, and constipation. The pelvic floor is a group of muscles that support the pelvic organs, which are made up of the bladder, rectum, small bowel, uterus, and vagina. Vaginal childbirth, menopause, pelvic surgery, radiation treatments, or being extremely overweight may cause the pelvic floor to weaken. Pelvic organ prolapse is a very common disorder among women, particularly for those who are over 50.

The choice of treatment for pelvic organ prolapse depends on the severity of the condition. Mild to moderate prolapse may be managed by doing pelvic floor strengthening exercises, taking hormone replacement therapy, or using pessaries. Pessaries are small devices inserted into the vagina to help support the pelvic organs. Pessary fitting is a quick procedure done in a doctor's office. For severe prolapse, surgical treatment may be necessary. Pelvic organ prolapse surgery repairs the following:

Cystocele or bladder prolapse, where the bladder sags into the vagina. The surgeon repairs a cystocele by strengthening the front wall of the vagina using stitches so that the bladder will no longer sag.
Enterocele or small bowel prolapse, which occurs when the small bowel descends towards the vagina. To correct an enterocele, the tissues between the small bowel and the vagina are sewn together, allowing them to provide more support.
Rectocele or posterior prolapse, which is the bulging of the front wall of the rectum into the back wall of the vagina. Surgery to repair a rectocele removes the excess stretched tissue between the rectum and vagina.
Uterine prolapse, which occurs when the uterus slips down to the vagina. To treat a uterine prolapse, the surgeon may secure the neck of the uterus (called cervix) to a ligament in the pelvis using stitches. Another option for this type of prolapse is a vaginal hysterectomy, which is the removal of the uterus.

Each of these procedures is performed through a vaginal incision, but an enterocele repair may use the abdominal approach as an alternative. If two or more pelvic organs have prolapsed, a combination of any of these procedures may be performed during the same surgical session.

For women who no longer wish to have intercourse, an operation called colpocleisis

may be a surgical option as well. By closing the vaginal canal, colpocleisis treats pelvic organ prolapse and eliminates any risk of its recurrence.

In most instances, pelvic organ prolapse surgery only requires a hospital stay of one day. Patients may experience vaginal bleeding for the first few weeks following surgery. If this happens, patients should use sanitary pads rather than tampons, as a higher risk of infection is associated with the use of tampons. To further help with recovery, patients should begin doing gentle pelvic floor strengthening exercises a few days after surgery. Doing these exercises at least three times a day as a routine tightens the pelvic floor muscles and prevents the recurrence of prolapse.

I chose this doctor because I saw she had multiple raving reviews. Here was my experience and impressions of my visit. While I chose her because she had raving reviews about her holistic approach this turned out to be a big mistake in my opinion. I don’t feel she took into consideration that I might know my body a bit more than she does. When trying to explain why I disagreed that exercising would improve my symptoms I got the condescending response of “do you know everything? Isn’t it possible that a doctor might know a bit more”? Or something along those lines. She did have some great techniques on relaxation that I’m sure would have been helpful for someone else. She gave me a flyer. I got emotional towards the end of my visit and cried. I think overall I just didn’t feel heard and i was disappointed that once again there was no solution in sight. I was genuinely very upset. To which she responded that I probably released some energy from the “vaginal exam” she did . Which in hindsight irritated me as well. I felt that she was so stuck in the “holistic” way that she was missing the bigger picture. It’s more likely I released energy than that I am frustrated and upset? Anyways her mind seemed made up I had an overactive bladder and here was a flyer of exercises I could do. I don’t want to say she was reluctant to order an ultrasound for me but it definitely came off that she was skeptical of my symptoms (bilateral flank pain). It came off as well I really think these exercises will be the solution but since you claim to have bilateral flank plain I’ll order this ultrasound to rule anything else out. About a week later I call the office regarding the ultrasound. Someone from her office calls me back the next day saying that according to the ultrasound I have “a few fibroids” and to follow up with GYN. I log into patient portal reviewed the ultrasound myself and there was a HUGE GIGANTIC fibroid. Clearly labeled, very obvious and everything! Thankfully I’m a nurse and showed one of the surgeons I worked with and was worked up pretty quickly. Like really you didn’t think to call and mention the GIANT fibroid could at least maybe compressing my bladder. Not very holistic! Super disappointed! And a little irritated. I feel bad for the people who are not in healthcare and don’t advocate for themselves! They didnt know I was a nurse. But I figured it shouldn’t have made a difference! Got a MRI after that reported moderate bladder compression. I’m not a doctor and I don’t know everything but even I could infer from size of the fibroid that it might be compressing my bladder.

Since I’ve provided why i am displeased with this experience here are some positives. I know there is a process and people need to be worked up. So it’s likely (I hope) at least that was the plan. I know she has multiple patients and I appreciate the ATTEMPT to connect and not appear inpatient or rushed. I could tell that she tried to appear like she cared. It just didn’t actually feeeeeel like she cared. I just wished after all that, talking down to me, telling her I don’t think the exercises would work, getting emotional and crying. That she would have at least called and said something like “hey I see on your ultrasound you have a large fibroid. It’s possibly that could be causing your urinary symptoms, you should continue doing those exercises we talked about but in the meantime go see your gyn”. instead of trying to gaslight me then dumping me! 😏

Dr Fleishman answered my questions and provided reasons for the issue I had with the implant. She told me we.could leave the issue for the moment but if it bothered me in the future she could fix it. I was happy with that response as the issue wasn't causing me any pain or discomfort.
