Autoimmune Diseases

An autoimmune disorder happens when the immune system mistakenly attacks the tissues of its own body, causing symptoms of illness. There are more than 80 different types of autoimmune disorders. While some are very rare, others are fairly common. Combined, autoimmune disorders are one of the leading causes of death and disability in the United States, affecting approximately 24 million people.

A properly working immune system identifies foreign substances in the body that might cause illness, such as bacteria and viruses. The immune system then creates antibodies which attack the foreign substances, neutralizing them and keeping the body safe. In people with autoimmune disorders, something goes wrong with this process. For reasons that are not fully understand, the immune system creates antibodies to attack the patient's own tissues.

Symptoms of an autoimmune disorder depend on which tissue is being attacked by the immune system, but common symptoms of autoimmune disease include fever, fatigue, and a general feeling of just not being well. Autoimmune disorders are more common in women than in men, and they may run in families. Autoimmune disorders can affect various parts of the body such as blood vessels, connective tissue, endocrine glands, joints, muscles, red blood cells, skin, and many others.

It is common to have more than one autoimmune disorder at a time. Most are chronic, or life-long illnesses, although they may come and go in flares. Treatment for autoimmune disorders depends on which part of the body is being attacked. For example:

A type 1 diabetic whose pancreas has been damaged will need insulin.
A person with Hashimoto's whose thyroid has been damaged will need replacement thyroid hormones.
Someone with Sjogren's syndrome will need eye drops and mouth rinses to replace tears and saliva.

Many autoimmune disorders of all kinds are treated with immune-suppressing medications, such as corticosteroids (e.g. prednisone) to reduce the effect of the immune system.

Heart Attack

A heart attack is a serious medical event that happens when the blood supplying oxygen to the heart is cut off, usually because of a blockage in an artery. This results in damage or death of part of the heart muscle. The heart can heal, but like any part of the body that was injured it leaves a scar. This scar tissue does not contract effectively, so the ability of the heart to pump is lowered after a heart attack.

A heart attack, sometimes called a myocardial infarction, can happen without warning. However there are certain factors that raise the risk of heart attack. Some of these are:

Age
Gender (most heart attack sufferers are male)
A family history of heart attacks
Smoking
Obesity
High cholesterol
High blood pressure
Diabetes
Unhealthy lifestyle choices (lack of exercise, poor diet, too much alcohol or stress)

Heart attacks typically feel like pain or pressure in the chest that can radiate out to the back, arm, or jaw. Some other symptoms can include fatigue or weakness, shortness of breath, sweating, and nausea.

Treatment aims to increase blood flow to the heart muscle by opening up blocked blood vessels. Medications are sometimes used to dissolve clots in the arteries. Aspirin and other medications such as Plavix can thin the blood and make clots less likely to form. Dilators can widen blood vessels to make room for more blood to flow. Doctors can also use a procedure called catheterization to look at the blood vessels around the heart directly, place stents, or widen vessels using angioplasty. In extreme cases bypass surgery can be used to help blood flow better. After recovery, it is important to lead a healthy lifestyle and take any prescribed medications in order to protect the heart.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

I think Dr Siecke is a good Dr. However his bedside manner could use a big improvement. By that I meant his personality and the way he talks with patients. He's not warm friendly, hardly smiles, is abrupt and short at times and sometimes seems in a hurry to leave the room without saying much of anything. I said "thank you Dr and have a good holiday ", he just walked out without saying anything!! That certainly didn't leave me with a very good feeling! But overall I think he's much better than my previous cardiologist and his nurse is always quick to reply to my questions and needs so that's good. Sometimes he's vague about his answers, didn't really give me a good answer to my question.

From start to finish this visit was an example of how good a Doctor's visit can go!! Staff were pleasant and efficient and the Doctor was "my sort of Doctor excellent"! After checking my records and updating same, based on my answers, he addressed me about concerns he had. It was professional and un-rushed. That "un-rushed" part is especially important, not just to me, ALL of us!!! Kudos all around.

I had a heart related issue at the ER who suggested I needed to see a cardiologist in the next two weeks. My health system was basically silent in lining up my needs. I contacted Swedish and they were able to fit me in at their earliest convenience. I applaud the response by Swedish, their staff, nurses, the doctor for their professional efforts. A solid team.

A good medical team as always. The Doctor did seem a bit distracted but still answered questions, etc. He did order a test which we hadn't discussed but it is a test done on a fairly reqular basis. I had seen a PA from the EP clinic earlier this year and my Doctor didn't discusss the notes and comments the PA had provided during that visit.

Siecke is brilliant, caring, and refreshingly relaxed. He suggests lifestyle changes that makes me want to try. Never judgemental but encouraging and always takes time to actually listen without having me feel like he's thinking about his busy schedule that's waiting. A doctor and also a friend. That's why he really is the best!

I think Dr Siecke walks on water! He is smart, kind, friendly, cares, great communicator so I can understand, motivates, takes time as needed, he goes above and beyond expectations to make sure his patients get what they need in a timely manner and follows up.Yes, Neil is amazing. If he ever leaves Swedish I leave too!

In general we are happy with the service. What may be a bit hard with the heart condition is that the process seems a bit long -- that is, long between appointments. I realize that a process has to be worked out, but my feeling is that things don't seem to move forward so quickly....

This office of Dr. Neil Siecke and his staff are wonderful. They've gone out of their way to help me and I'm sure they've probably done so with other patients. I've never had a bad experience at this office. Wish all Doctors offices were run as well and cared as much!

Dr. Sicke Is an excellent provider. He has detailed plan for my care. I trust his knowledge and training. He cured my high blood pressure. His bedside manners are excellent. He answers all my questions and I won't go to any other provider. I appreciate his care!
