Headache

Headaches are dull or sharp pains that occur in regions of the head and face. Headaches occur in many different forms and vary in location, severity, and duration. They are not necessarily a sign of an underlying illness and often resolve on their own. However, headaches can present significant day-to-day discomfort. The most common forms of headaches include:

  • Migraines: one of the most prevalent types of headache, migraines typically last from two hours to three days, causing a throbbing pain on one side of the head. Nausea, sensitivity to bright lights, fatigue, and blurred vision all may accompany migraines. While migraines have no known cure, medications and lifestyle modifications may ease symptoms. Avoiding behaviors that may trigger migraines such as lack of sleep or loud, repetitive noises can improve headaches.
  • Tension headaches: also known as stress headaches, these headaches are characterized by pain in the upper top part of the head and behind the eyes. Tension headaches are common and have no exact cause; however, certain practices may increase the likelihood of developing them. For example, wearing one's hair up in a tight hairstyle or wearing a restrictive hat can put pressure on the scalp, leading to a tension headache. Other causes of tension headaches may include alcohol or nicotine use, caffeine, teeth grinding, stress, overexertion, and eye strain. Some people find posture improvement, alternating sleeping positions, and neck exercise can alleviate tension headaches. Over-the-counter pain relievers such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve) may also reduce tension-headache pain.
  • Sinus headaches: this type of headache causes pain in the sinuses, but may progress to pain and pressure in the eyes, forehead, and cheeks. These headaches are often accompanied by nasal congestion, fatigue, and ache in the upper teeth and may or may not be caused by sinus infections or sinusitis (sinus inflammation). Some sinus headaches are allergic headaches, which arise from allergic triggers such as seasonal allergies (pollen allergies) or hay fever, causing sinus pressure and facial pain. Other symptoms of allergic headaches include nasal congestion, irritated or watery eyes, runny nose, and sneezing. Allergic headaches may be treated with antihistamine medications such as cetirizine (Zyrtec). Allergic headaches are common during pollen season, which is usually around springtime.
  • Cluster headaches: these headaches occur suddenly and cause stabbing, sharp pain from one eye down to the one side of the neck. They are often referred to as the most aggravating and painful type of headaches. Other symptoms of cluster headaches include nasal congestion, droopy eyelid, and enlarged pupil on one side of the face. In the past, it was widely believed that cluster headaches were caused by a bevy or "cluster" of multiple types of headaches, such as a combination of an allergic headache and a tension headache. Medical experts now recognize that cluster headaches are a separate and unique form of headache. Cluster headaches last for around twenty minutes to two hours. They may recur throughout the period of a day or several weeks, but usually dissipate within a month. Treatment for cluster headaches includes over-the-counter pain medications such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve). Prescription Prednisone can help with swelling and Verapamil may reduce pain by relaxing the blood vessels.

Living with headaches is challenging; headaches can prevent people from fully enjoying life. Medical treatment and care in avoiding certain triggers can help those with headaches begin to feel normal once again.

Mononucleosis (Mono)

Mononucleosis, frequently called ''mono,'' is a viral infection that causes fatigue, fever, swollen lymph glands, and sore throat. The Epstein-Barr virus (EBV), spread through saliva, is the primary virus that leads to mono. Mono is most common in teenagers and young adults, where EBV is often transmitted through kissing or sharing drinks. For this reason, mono is colloquially referred to as ''the kissing disease.'' Mononucleosis usually resolves on its own; however, recovery from mono can take from several days to several weeks. Some young adults may continue to experience extreme fatigue from mono for months, often requiring a semester home from school.

Mononucleosis happens in three stages:

During the initial stage, those with mono may experience few symptoms as EBV replicates in the body. Symptoms such as headaches and body aches are often misattributed to allergies or stress. People with mono may continue performing their daily activities and attending school, leading to increased transmission of the virus to others. The initial stage lasts for about five to fourteen days.

During the acute stage, mono symptoms intensify and may include the enlargement of the spleen, a sore throat, fatigue, liver issues, and skin rashes. The mono infection is at its worst in this stage.

Lastly, the convalescent stage is when recovery begins. This period of healing can last from two to six months.

Mono can be detected by a physical exam; physicians can diagnose mono by checking for inflamed lymph nodes, tonsils, or spleens. Blood antibody tests may also help diagnose mono by revealing antibodies in the blood that were formed as a response to the Epstein-Barr virus. Elevated levels of white blood cells can also suggest that a patient has mono.

There are no medications specifically for mono or the Epstein-Barr virus, and patients are advised to rest and avoid strenuous exercise. Drinking plenty of fluids, eating a healthy diet, and taking anti-inflammatory ibuprofen (motrin) may also help people who are afflicted with mono feel better.

People with mono should take precautions to avoid spreading the condition to others. They can prevent infection by not sharing glasses, food, and utensils with others until they fully recover.

Thyroid Problems

Thyroid problems are a group of conditions that alter hormone regulation and metabolism. The thyroid is a small, butterfly-shaped gland located in the lower neck, just below the Adam's apple (laryngeal prominence) and larynx (voice box). Despite its small size, the thyroid has an outsized effect on the body by regulating hormones that control metabolism. Metabolism is the body's process of converting consumed food into fuel to power the entire body. Heart rate, weight, cholesterol, body temperature, and even physical growth and development are all affected by the metabolism.

When the thyroid malfunctions, the body is unable to properly process and allocate energy. Thyroid problems, such as hyperthyroidism (overactive thyroid) and hypothyroidism (underactive thyroid), cause irregular metabolism. In some cases, the body produces antibodies that attack the thyroid cells for unknown reasons. The damaged thyroid responds by releasing either too little or too much T3 and T4 thyroid hormones. Thyroid malfunctions may also arise from genetic causes or iodine deficiency. Thyroid problems may include:

  • Hashimoto's thyroiditis, an autoimmune disease that causes hypothyroidism
  • Graves' disease, an autoimmune disease that causes hyperthyroidism
  • Goiter
  • Thyroid nodule
  • Thyroid cancer

Hashimoto's is the leading cause of hypothyroidism, which is when the thyroid does not produce enough hormones to stimulate adequate metabolism. The condition is diagnosed by blood tests that measure TSH (thyroid stimulating hormone) and detect Hashimoto's antibodies. Symptoms of hypothyroidism include sluggish metabolism, inflammation, fatigue, weight gain, brittle hair, dry skin, and sensitivity to cold. To supplement impaired thyroid function, patients with Hashimoto's are typically prescribed synthetic thyroid hormone pills. Such patients may need to consume synthetic thyroids for the rest of their lives. Other treatments may include dietary changes, such as the avoidance of certain foods and medication to reduce inflammation. Frequent blood tests can inform patients if their TSH levels are within a normal range.

Grave's disease is most frequently the cause of hyperthyroidism, which is when the thyroid produces too many hormones, overstimulating one's metabolism. The condition is also diagnosed by blood tests that measure TSH and detect Grave's antibodies. Elevated heart rate, high blood pressure, sweating, bulging eyes, and weight loss are common symptoms of hyperthyroidism. Treatments for hyperthyroidism include oral radioactive iodine, which slowly shrinks the thyroid, slowing the production of excess thyroid hormone. Other anti-thyroid medications such as methimazole (Tapazole) also reduce the production of thyroid hormones. Blood pressure medications known as beta blockers can ease the increased heart rate caused by hyperthyroidism. If treatments are unsuccessful, patients may be prescribed a thyroidectomy, or surgery to remove part or all of the thyroid. Some patients with hyperthyroidism may require ophthalmological (eye medicine) treatments such as eye drops, prednisone, and surgery to reduce eye swelling and bulging.

Untreated thyroid disorders frequently lead to goiters or thyroid nodules. A thyroid goiter is a large lump that can be felt near the base of the neck and is made of swollen thyroid tissue or nodules (excess cell growth). Nodules and goiters are diagnosed by CT scan or ultrasound. Obstructive goiters can cause pain, coughing, and abnormal breathing. The most common cause of goiters is hyperthyroidism (overactive thyroid). However, Hashimoto's is also known to cause goiters or nodules. Goiters develop from continued damage from antibodies to the thyroid over time. At times, a goiter can result from a cancerous thyroid tumor. Most goiters and nodules are themselves benign.

The exact causes of thyroid cancer are unknown. People who develop thyroid cancer may or may not have another thyroid problem such as Hashimoto's or Grave's disease. Symptoms of thyroid cancer include a nodule that can be felt on the neck, hoarseness, difficulty swallowing, swelling in the neck, and neck and throat pain. CT and MRI scans are used to diagnose thyroid cancer. There are four types of thyroid cancer which vary in intensity. Thyroidectomy (thyroid removal surgery) is usually prescribed to treat thyroid cancer.

Thyroid problems are often long-term conditions that require some form of treatment. With regular monitoring and medication, many people with thyroid problems are able to lead active and fulfilling lives.

I have been a patient of Dr. Iqbal for over 20 years, and I truly can't imagine trusting my care to anyone else. His professionalism and thoroughness set him apart as an all-around exceptional doctor. No matter how busy his schedule may be, I have never once felt rushed, ignored, or unheard.Dr. Iqbal takes the time to listen carefully to my concerns, explain every aspect of my care, and ensure I feel comfortable with any decisions about my health. He has an incredible ability to combine deep medical expertise with a warm, compassionate approach that puts patients at ease.His staff is just as wonderful-always responsive, helpful, and welcoming. It's clear they share his commitment to patient care and respect.When it comes to health insurance, I am genuinely hesitant to make any changes out of concern I could lose Dr. Iqbal as my primary physician. I will always make sure he is in my network because having him as my doctor is that important to me.If you are looking for a physician who is not only highly skilled but also caring and dedicated, I can't recommend Dr. Iqbal enough. I feel very fortunate to have my current medical professional, Dr Iqbal. He always listens when I explain my medical concerns and factors that into his assessment of my health. He is always very professional and respectful yet strategic in approach to collecting additional data needed to better understand my health status and develop an appropriate action plan. I do appreciate, when I have a health concern that requires immediate attention, that the office staff try to schedule an appointment at the earliest availability. However, the office is quite busy and that may mean there is a several day wait to be seen. I do, when possible, wait to be seen by Dr. Iqbal due to his nature, knowledge, professionalism, and his willingness to listen to what information I have to share as his patient.

Dr Iqbal is the best. I have been going to him for about 12 years now. He is always been so professional and understanding everything about my health. This appointment my health had declined a tiny bit and he was very thorough and explaining the changes from my last blood test to this blood test. We talked about what was going on that was impacting it and I explained my circumstances to him. He was very sincere and I knew I was cared for. Your office and he in particular are the best. A lot of people ask me why I don't go to a PPO and I say I don't need to because I have the best doctor. Thank you for sending surveys to ensure that your network is providing the best care possible. I value Dr Iqbal's medical proficiency, patient empathy, and communication skills very highly. I have the highest level of trust that Dr Iqbal is invested in my well-being.However, this survey seems to have completely missed capturing the value of the integration of My Chart as a conduit for direct connection to (in my case) Dr Iqbal and his team. I have come to rely on the messaging capabilities within MyChart, and have always received quick responses from Dr Iqbal and his team. Having this capability allows us (my doctor and I) to set an agenda for upcoming visits to make best use of the time available.

Dr. Iqbal is BY FAR the best primary care doctor I have ever seen! I was blown away by the amount of time he spent with me, the thoroughness with which he gathered my information and connected with other providers to ensure that he, as my primary provider, was aware of everything that was happening to me medically. I was also amazed with his kind manner and concern for me, even though I know how incredibly busy he is. I am SO HAPPY that I found Dr. Iqbal.

This was my 1st visit with this doctor, and I was a little nervous. The second the doctor said hello to me, all my nerves melted away! He was very friendly and very easy to talk to! He LISTENED to every word I said asked me if I had any questions for him. He obviously cares about what he does, showed respect for me, and it was clear he cared about me and my health, even though he'd just met me! By the end of the appt I knew I had just met my new doctor!

Dr Iqbal has been the best Physician I have ever been to. His team is well versed in taking care of patients and I truly appreciate how Dr Iqbal Is available to be between appointments if I have any questions. The front desk women are exceptional and making an appointment is so easy and they ladies remember me. It makes this feel like a small family office instead of a large corporation. I love my Doctor and his staff. I wish they able to book appointments in lesser days no to way too long to see my doctor. Or get sooner appointments for medical reason. Also I recommend that if we out of state for vacation o visiting a family member, we are able to see a doctor or other providers or urgent care or have the ability to see your doctor or a different doctor via virtual call without any hassle. Otherwise everything us wonderful.

Dr. Iqbal is one of only a handful of doctors these days that actually listens, is friendly and takes time to explain everything. He is a credit to the profession. You should modify questions such as (Has anyone from the office contacted you about blood test results etc in the past 3 months). I am able to check that on my own through My Chart. There should be a question re: the use of My Chart for some of those questions posed.
