Autoimmune Diseases

An autoimmune disorder happens when the immune system mistakenly attacks the tissues of its own body, causing symptoms of illness. There are more than 80 different types of autoimmune disorders. While some are very rare, others are fairly common. Combined, autoimmune disorders are one of the leading causes of death and disability in the United States, affecting approximately 24 million people.

A properly working immune system identifies foreign substances in the body that might cause illness, such as bacteria and viruses. The immune system then creates antibodies which attack the foreign substances, neutralizing them and keeping the body safe. In people with autoimmune disorders, something goes wrong with this process. For reasons that are not fully understand, the immune system creates antibodies to attack the patient's own tissues.

Symptoms of an autoimmune disorder depend on which tissue is being attacked by the immune system, but common symptoms of autoimmune disease include fever, fatigue, and a general feeling of just not being well. Autoimmune disorders are more common in women than in men, and they may run in families. Autoimmune disorders can affect various parts of the body such as blood vessels, connective tissue, endocrine glands, joints, muscles, red blood cells, skin, and many others.

It is common to have more than one autoimmune disorder at a time. Most are chronic, or life-long illnesses, although they may come and go in flares. Treatment for autoimmune disorders depends on which part of the body is being attacked. For example:

A type 1 diabetic whose pancreas has been damaged will need insulin.
A person with Hashimoto's whose thyroid has been damaged will need replacement thyroid hormones.
Someone with Sjogren's syndrome will need eye drops and mouth rinses to replace tears and saliva.

Many autoimmune disorders of all kinds are treated with immune-suppressing medications, such as corticosteroids (e.g. prednisone) to reduce the effect of the immune system.

Heart Attack

A heart attack is a serious medical event that happens when the blood supplying oxygen to the heart is cut off, usually because of a blockage in an artery. This results in damage or death of part of the heart muscle. The heart can heal, but like any part of the body that was injured it leaves a scar. This scar tissue does not contract effectively, so the ability of the heart to pump is lowered after a heart attack.

A heart attack, sometimes called a myocardial infarction, can happen without warning. However there are certain factors that raise the risk of heart attack. Some of these are:

Age
Gender (most heart attack sufferers are male)
A family history of heart attacks
Smoking
Obesity
High cholesterol
High blood pressure
Diabetes
Unhealthy lifestyle choices (lack of exercise, poor diet, too much alcohol or stress)

Heart attacks typically feel like pain or pressure in the chest that can radiate out to the back, arm, or jaw. Some other symptoms can include fatigue or weakness, shortness of breath, sweating, and nausea.

Treatment aims to increase blood flow to the heart muscle by opening up blocked blood vessels. Medications are sometimes used to dissolve clots in the arteries. Aspirin and other medications such as Plavix can thin the blood and make clots less likely to form. Dilators can widen blood vessels to make room for more blood to flow. Doctors can also use a procedure called catheterization to look at the blood vessels around the heart directly, place stents, or widen vessels using angioplasty. In extreme cases bypass surgery can be used to help blood flow better. After recovery, it is important to lead a healthy lifestyle and take any prescribed medications in order to protect the heart.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

Doctor is very focused upon providing me, the patient, with excellent care. I have learned to understand his particular bed side manners...and have learned that I, as a patient, need to provide him my full attention during my visit....and when he has finished addressing all the issues/ care he feels necessary to tell me about my health, test and exam results, changing/ and new medications etc. I then can ask him questions. At that point during my visit, he is attentive and answers all of my questions which I appreciate. Thank you.

I had a good first impression when I met this provider for the first time. He answered all of my questions. I was a human person instead of a number, or just a female. I saw that he was a 5 star rated provider which made me feel so very comfortable.. I love the Asian Dr's as their intellect is very methodical and has this coverage inside that is different from so many others.. Very smart. Direct, concise and reflective. It's a very amiable quality that tends to keep my trust.

Dr Chang is awesome. I would love for him to be my PCP and not just cardiologist. He is so caring and gentle with the way he treats me (even when he is telling me something I don't want to hear). It's generally difficult to get appointments with him because he is so busy. I am so very grateful that his office was able to get me an appointment quickly because of the recommendations from my other doctors and tests I had. I would be very sad to lose him as my cardiologist.

This was my first visit with Dr Chang. I was referred to him by my primary care physician to check and make sure that my cholesterol medication and dosage was adequate and that my heart is in good shape. Medication and dosage were found to be fine, as is my heart. No follow up needed. I found Dr Chang to be friendly, compassionate and caring. If I ever have a cardiac issue in the future, he is the doctor I would like to see.

All good. Appreciated Dr. Chang's courtesy, knowledge, friendliness, sense of humor, & respect. Ditto for attending staff. They & the front desk folks do seem somewhat stressed from time management issues, but no different in any other superbusy clinic. I did have 2 questions the assistant was going to have Dr Chang respond to, but there hasn't been in follow up to those just yet. Not too worried, tho. Thanks!

I am very grateful for Dr. Chang and his team. Very professional, and I feel that I am in good hands with Dr. Chang, and how things are changing with my health, and I do understand it. I do everything that I can to comply with everything told to me. I would not want any other heart Dr because of the trust I feel towards Dr Chang, and the knowledge that this Dr is very, very intelligent with my care.

Dr Chang is great. Shared knowledge, listened carefully.Was lots of confusion setting up appointment. Office changed several times. Each time I made other arrangements based on their dates, then things were changed. Was supposed to see doctor for Oh annual visit then told I couldn't this year I would need to wait till next year. When I pushed back I was rescheduled with the doctor.

I like Dr Chang very much. He is easy to talk to, listens to my concerns or comments but doesn't hesitate to tell me if I need to change my habits or do something differently. Back on an earlier question I said he was not the dr to care for my day-to-day concerns, he's not my GP but being my cardiologist, he is definitely who I turn to with those questions and concerns.

He checked my heart status and encouraged my ongoing self-care plans, while also offering additional ideas to handle grief. I am very grateful for his caring attitude.

Dr. Chang listens very well and explains very well. He is very patient. He provided the best advice that I have ever received regarding my need to stay physically fit to protect my heart and my health as I pass 80 years of age. He was very specific about the amount of exercise I need to do weekly In order for my heart to be strong.
