Sacral Nerve Stimulation (SNS)

Sacral nerve stimulation (SNS), also called sacral neuromodulation, is a treatment for bladder and bowel problems. Using thin, insulated wires (electrodes) and a device called a neurostimulator, sacral nerve stimulation can deliver electrical signals to correct communication issues between the brain and the bladder. The electrical signals reach the sacral nerve, which is responsible for muscles that control the bladder and rectal sphincter. By targeting this nerve, sacral nerve stimulation can influence the activity of pelvic muscles to treat overactive bladder, fecal (bowel) incontinence, and chronic constipation. When diet or lifestyle changes, medication, and other conservative options fail to work, neurostimulation is an option.

SNS surgery has two parts. The first part is an evaluation phase, during which it is determined if neurostimulation is effective treatment option for patients. Patients lie on their stomachs while a temporary electrode lead, through which electrical signals will travel, is inserted into their lower back. A permanent lead may also be used, and it would remain in place should the trial period prove successful. The lead is connected to an external neurostimulator. The procedure takes approximately one hour and may be performed in a doctor's office, hospital, or surgical center. After the procedure, patients are required to keep a log of their toilet habits over approximately two weeks, and they may need to limit their activities during this time. If patients' symptoms do not improve after the trial, they may repeat the test phase or discuss other treatment options with their doctors.

If the trial stimulator is effective, patients will undergo the second phase of SNS surgery, which is permanent implantation. In the second stage of surgery, the temporary lead (if used) will be replaced with a permanent lead. Then the sacral nerve stimulator will be implanted subcutaneously (under the skin) in the upper buttock. Patients should be able to return home the day of their procedure.

If patients are sore after surgery, their doctors may give them medication. The sacral nerve stimulator will be programmed after the procedure, and when it is turned on, patients should feel a sensation similar to pulling, tapping, tingling, or pulsing. Patients will need to work with their doctors during follow-up appointments to determine the most effective settings for the stimulator. Patients will be given instructions on how to adjust it themselves at home. The stimulator is powered by a battery that will last approximately five years, after which it can be replaced.

Spinal Cord Injury

Spinal cord injury medicine is a branch of medicine that treats damage to the spinal cord from an injury or a nontraumatic myelopathy, which is any disorder that affects the spinal area. The spinal cord is a section of nerve tissue protected by the vertebrae (spinal bones). Trauma to the spinal cord, typically caused by compression or bent vertebrae, prevents the body and brain from communicating. This can result in pain, loss of sensation, and impaired physical function and movement. Acute spinal cord injuries (SCIs) are a leading cause of permanent disability. Spinal cord injuries may develop from accidents or myelopathies, disorders that provoke spinal compression. Traumatic incidents such as falls, sports injuries, vehicle accidents, and bullet or stab wounds can cause an acute spinal cord injury.

Spinal cord injury medicine seeks to stabilize the spine and to alleviate the symptoms of spine damage. Practitioners of spinal cord injury medicine are called spinal cord injury specialists. Spinal cord injury medicine is an interdisciplinary field, meaning physicians of various specialties may be trained to treat spinal cord injuries.

Damage to the spinal cord may initially be diagnosed through X-ray, MRI, or CT scans. Patients can also undergo spinal exams to evaluate their sensory ability and strength. Injuries to the spinal cord can be complete (no feeling or sensation) or incomplete (some feeling or sensation remains). Conditions spinal cord injury medicine specialists may treat include:

Paralysis, such as paraplegia (loss of function in the legs), or quadriplegia (loss of function from the neck down)
Spine fracture or trauma, which can lead to impairment and/or significant pain
Spinal infections
Myelopathies, such as herniated disks, spinal tumors, and rheumatoid arthritis
Inflammatory diseases such as ankylosing spondylosis, and degenerative conditions like Lou Gehrig's disease (ALS)

Treatment for spinal cord injuries often involves surgery, either directly after an injury occurs or at a later date. Surgery for spine injuries is intended to ease spinal compression and stabilize the spine. Surgical procedures can involve shifting vertebrae, removing bone, or altering spinal placement with implantable devices. Patients with significant spinal trauma may require urgent surgical intervention.

Surgical treatments can be complemented by rehabilitative physical therapy to improve mobility. Spinal cord injury specialists also treat complications arising from a spinal injury, such as respiratory or bladder conditions. Some patients with spinal cord injuries may require lifelong treatment. Spinal cord injury specialists aim to help these patients successfully adjust and lead an improved quality of life. An emerging treatment for spinal cord injuries is neural prosthetics, which replicate patients' lost nerve function. Neural prostheses may be used as artificial body parts or assistive devices that patients may cognitively control. Other assistive devices include wheelchairs and scooters.

Spinal cord injury specialists may collaborate with physical therapists, radiologists, neurologists, urologists, and orthopedists.

Urodynamics (Bladder and Urethra Function Test)

Urodynamics are tests that assess how well the lower urinary tract is functioning. Specifically, they evaluate its ability to hold and release urine, as well as show if any blockages or leaks are present. The lower urinary tract is made up of the following:

The bladder, which stores urine.
The urinary sphincters, which are muscles around the opening of the bladder that hold urine in.
The urethra, which is the tube where urine flows out of the body.

Urodynamics tests are normally performed together as one series, but depending on patients' symptoms, only one or some of them may be necessary. The most common tests are:

Cystometrogram, which measures the amount of urine the bladder can store and how full it is before someone has the urge to urinate. Doctors may order this test if she suspects their patient has a weak bladder.
Pressure flow study, which measures the pressure in the bladder while urinating. This test helps locate a blockage in the bladder.
Uroflowmetry, which measures how much and how fast urine flows out of the body. This test is usually recommended when patients have difficulty urinating or they have a slow stream.
Urethral sphincter electromyography, which measures how strong the muscles and nerves in the bladder and sphincters are. Doctors may request this test if the problem they are experiencing is linked to muscle or nerve damage in the lower urinary tract.
Urethral pressure profile, which measures the pressure along the urethra while the bladder is gradually filled with fluid. Low pressure could mean someone has a weak urethra, a contributing factor to urinary incontinence (loss of bladder control).

For a few hours following a urodynamics study, patients will likely feel some soreness or discomfort. It is recommended to drink at least one glass of water every half hour for two hours to relieve these symptoms. Holding a warm, moistened washcloth over the area where discomfort is felt or taking a warm bath may also help. Results for cystrometrograms and uroflowmetry are typically available immediately after the test, but results for the other tests may take a few days. When doctors have reviewed a patient's test results, he or she will discuss them with the patient, along with any treatments the patient may need.

Dr was knowledgeable, explained so I could understand, knew not only urology but gyne issues & referred me for fol up w himself for urology issues & to gyne for issue that fits gyne better. He explained additional tests not available to me done on urine specimen showed no blood. Dr was gentle during exam, did not hurt me, was cognizant of my embarassment as body was checked.
