Kidney Stones

Kidney stones are hard deposits that form in the kidneys, made up of minerals that are normally present in urine. They can vary in size, from as small as a grain of sand to as large as a nickel, occasionally even larger. Sometimes they lodge in the kidney, and sometimes they break free and make their way out through the urinary tract, which can be extremely painful.

Kidney stones can be smooth or jagged and are yellow to brown in color. They are mostly comprised of the minerals calcium, oxalate, and phosphorus. Examining the stones to see what they are made of can show what caused the stone to be formed in the first place. For example, a stone made of mostly calcium, which is the most common type, can happen any time the urine becomes too concentrated due to dehydration or a blockage in the kidney. A uric acid stone forms when acid levels in the urine get too high, usually due to excessive consumption of animal protein such as meat and fish. A struvite stone is a sign of certain infections, and a cystine stone can be due to a genetic disorder that raises the risk of kidney stones.

The most common symptom of kidney stones is pain, either in the back or lower abdomen, or severe pain when urinating. There may also be blood in the urine. Treatment for kidney stones depends on how large the stone is. Very small stones can pass out of the body on their own, and they do not require treatment other than drinking adequate water and taking pain killers. Larger stones need to be broken apart and removed. The main treatment options are:

  • Shock wave lithotripsy, which uses sound wave vibrations to break apart the stone
  • Percutaneous nephrolithotomy, or the use of a very tiny tool (like a wire inserted through the back) to break apart and remove the stone
  • Ureteroscopy, a thin tube inserted through the urethra and bladder to the stone, where tiny tools can grasp the stone and remove

People who have had one kidney stone are at risk of developing another. To reduce this risk, patients are given instructions specific to the type of stone they developed. Generally the instructions will include drinking more water to dilute the urine, but it may also involve lowering sodium intake or eating less meat.

Pelvic Organ Prolapse

Pelvic organ prolapse is the bulging of pelvic organs into the vaginal canal due to a weak pelvic floor, causing symptoms like discomfort, pain, urinary problems, and constipation. The pelvic floor is a group of muscles that support the pelvic organs, which are made up of the bladder, rectum, small bowel, uterus, and vagina. Vaginal childbirth, menopause, pelvic surgery, radiation treatments, or being extremely overweight may cause the pelvic floor to weaken. Pelvic organ prolapse is a very common disorder among women, particularly for those who are over 50.

The choice of treatment for pelvic organ prolapse depends on the severity of the condition. Mild to moderate prolapse may be managed by doing pelvic floor strengthening exercises, taking hormone replacement therapy, or using pessaries. Pessaries are small devices inserted into the vagina to help support the pelvic organs. Pessary fitting is a quick procedure done in a doctor's office. For severe prolapse, surgical treatment may be necessary. Pelvic organ prolapse surgery repairs the following:

  • Cystocele or bladder prolapse, where the bladder sags into the vagina. The surgeon repairs a cystocele by strengthening the front wall of the vagina using stitches so that the bladder will no longer sag.
  • Enterocele or small bowel prolapse, which occurs when the small bowel descends towards the vagina. To correct an enterocele, the tissues between the small bowel and the vagina are sewn together, allowing them to provide more support.
  • Rectocele or posterior prolapse, which is the bulging of the front wall of the rectum into the back wall of the vagina. Surgery to repair a rectocele removes the excess stretched tissue between the rectum and vagina.
  • Uterine prolapse, which occurs when the uterus slips down to the vagina. To treat a uterine prolapse, the surgeon may secure the neck of the uterus (called cervix) to a ligament in the pelvis using stitches. Another option for this type of prolapse is a vaginal hysterectomy, which is the removal of the uterus.

Each of these procedures is performed through a vaginal incision, but an enterocele repair may use the abdominal approach as an alternative. If two or more pelvic organs have prolapsed, a combination of any of these procedures may be performed during the same surgical session.

For women who no longer wish to have intercourse, an operation called colpocleisis

may be a surgical option as well. By closing the vaginal canal, colpocleisis treats pelvic organ prolapse and eliminates any risk of its recurrence.

In most instances, pelvic organ prolapse surgery only requires a hospital stay of one day. Patients may experience vaginal bleeding for the first few weeks following surgery. If this happens, patients should use sanitary pads rather than tampons, as a higher risk of infection is associated with the use of tampons. To further help with recovery, patients should begin doing gentle pelvic floor strengthening exercises a few days after surgery. Doing these exercises at least three times a day as a routine tightens the pelvic floor muscles and prevents the recurrence of prolapse.

Urodynamics (Bladder and Urethra Function Test)

Urodynamics are tests that assess how well the lower urinary tract is functioning. Specifically, they evaluate its ability to hold and release urine, as well as show if any blockages or leaks are present. The lower urinary tract is made up of the following:

  • The bladder, which stores urine.
  • The urinary sphincters, which are muscles around the opening of the bladder that hold urine in.
  • The urethra, which is the tube where urine flows out of the body.

Urodynamics tests are normally performed together as one series, but depending on patients' symptoms, only one or some of them may be necessary. The most common tests are:

  • Cystometrogram, which measures the amount of urine the bladder can store and how full it is before someone has the urge to urinate. Doctors may order this test if she suspects their patient has a weak bladder.
  • Pressure flow study, which measures the pressure in the bladder while urinating. This test helps locate a blockage in the bladder.
  • Uroflowmetry, which measures how much and how fast urine flows out of the body. This test is usually recommended when patients have difficulty urinating or they have a slow stream.
  • Urethral sphincter electromyography, which measures how strong the muscles and nerves in the bladder and sphincters are. Doctors may request this test if the problem they are experiencing is linked to muscle or nerve damage in the lower urinary tract.
  • Urethral pressure profile, which measures the pressure along the urethra while the bladder is gradually filled with fluid. Low pressure could mean someone has a weak urethra, a contributing factor to urinary incontinence (loss of bladder control).

For a few hours following a urodynamics study, patients will likely feel some soreness or discomfort. It is recommended to drink at least one glass of water every half hour for two hours to relieve these symptoms. Holding a warm, moistened washcloth over the area where discomfort is felt or taking a warm bath may also help. Results for cystrometrograms and uroflowmetry are typically available immediately after the test, but results for the other tests may take a few days. When doctors have reviewed a patient's test results, he or she will discuss them with the patient, along with any treatments the patient may need.

My visit to the urologist was very good. However, I was not aware that I had to call to make an appoint after my referral was sent by my PCP. I was told to wait until the urology Dr office call for the appointment. However, the urology office had never called me. So, I made sure the referral was sent to them on time and I received the name and phone number of the urology Office from my PCP's office in order for me to make an appointment after waiting for long time. I was hopping to get the care sooner due to my condition and insurance purpose, but it didn't happened as I desired.

I have seen Dr. Cheng multiple times over the years for treatment with kidney stones, December of 22 my primary Dr. Did a PSA test which increased from 1.1 in 2021 to 7.5 in dec 22. It took 3 months to schedule an initial consultation for that then an additional 4 months to schedule a prostrate biopsy. Out side of (in my personal opinion) an exceeding long time to be seen on a potential cancer diagnosis. She is a patient Dr. And has a great " bedside" manner, explains things well and demonstrates confidence in her ability.

Dr. Cheng is easy going and a joy to have as my specialty care physician. She listens to my concerns, and explains things at my level of understanding. She explains treatments and conditions from both good and bad outcomes. She is open to, within reason, modifying treatment and waiting to see the outcome. Not like a previous physician( not of this med group) that, it was their way and t"their way" was the only way.She is a exceptional doctor in my book.

I am an RN that worked with my husband, an Orthopaedic surgeon, in his office. I was amazed at how efficient, thorough and knowledgeable Dr.Cheng was about the types and outcomes of gynecological and urological procedures I have had in the last 50 yrs. Including what was available to me now. It gave me utmost confidence in her ability to treat me now.

Dr. Cheng consistently goes above and beyond routine patient care. I had a traumatic injury, her attitude and demeanor made me feel confident in my recovery.Her attentiveness and compassion were not only important for me, but necessary to build trust. Although I wished my injury never occurred, I'm relieved that Dr. Cheng was part of the care team.

However I feel I should be listend to with some attention not condescension and that past health history be taken under scrutiny in order to better understand my present condition. I'm thinking that Dr. Cheng is a very competent urologist. IF I was her coach, I'd encourage her to review patient files before seeing them, provide time to listent to the patient early on in the visit, share her advice in a manner that does not preach, and then check for understading by the patient.

Good: Relatively painless procedure due to care provider's skill, experience, professionalism. Excellent care and preparation for procedure by care provider's assistant. Five-minute procedure.Bad: Extensive wait (3 1/2 months) for 5-minute procedure to be performed. Due primarily to staff shortages.

I feel my doctor I feel my doctor it's very qualified and she makes me feel very safe. I have had a lot of problems with my bladder and I've had a couple of surgeries and she is just been so good about it. She explains everything to me and I really makes me feel safe and decreases my anxiety.

I had a very bad experience with a provider outside of Providence. After this I asked within the Providence group for a second opinion from a Urologist. This entire experience has been one of professionalism and expertise that is very appreciated. I would recommend them to any of my friends.
