Cesarean Section (C-Section)

A Cesarean section (more often known as "C-Section") is a surgery performed during labor and delivery. A C-Section is a procedure that delivers a baby through an incision in the lower abdomen and uterus. In the United States, about a third of all births are performed by Cesarean section. Most often, a C-section is done by an obstetrician/gynecologist (OB-GYN), or in certain regions, a general surgeon or family physician.

C-section incisions can be up-and-down (vertical), from belly button to lower abdomen or side-to-side (horizontal), across the lower abdomen. Side-to-side incisions are more frequently used. After the exterior incisions are made, the doctor then cuts into the uterus and amniotic sac and delivers the infant and cuts the baby's umbilical cord. The mother receives medicine to expel the placenta and the doctor helps to safely remove it. Finally, the doctor uses sutures to seal the uterus and tissue incisions. The initial C-section skin incision is also closed with sutures or staples.

C-sections are typically performed with regional anesthesia, which inhibits pain in the lower half of the body, but allows mothers to be awake. However, some mothers may receive general anesthesia, where they are put to sleep during the surgical procedure.

C-sections may be planned or unplanned, depending on the circumstances of the birth.

Pre-existing conditions like heart disease, high blood pressure, herpes, and HIV may prompt a doctor to prescribe a C-section. Other situations for C-section include:

A large baby or birth to multiple children
Pelvic fracture or limited birth canal
Baby in the "breech position," where a baby is positioned feet or bottom first in the uterus
Baby with a developmental disorder
Irregular fetal heartbeat
Mother with a history of previous C-sections

Other mothers may require a C-section due to complications arising during delivery. These unplanned, emergency C-sections allow for a healthy birth even when complex complications are present. Problems during labor which may necessitate a C-section include:

Labor not progressing
Fetus switching position during labor
Distressed infant
Blockage of the cervical opening by the placenta
Prolapsed umbilical cord

Mothers should expect about a six-week recovery period following a C-section, including an initial hospital stay of two to three days. While in the hospital, mothers may spend time with their baby and begin breastfeeding. After returning home, mothers should limit strenuous activities such as lifting, reaching, and exercise, and should make sure to get plenty of rest.

Cryotherapy

Cryotherapy is the targeted use of extreme cold to destroy unhealthy or unwanted tissue. The doctor applies the cold on the skin or injects it through a needle to reach the inside of the body, where it freezes away the problem tissue. Cryotherapy is often used to remove minor skin problems such as warts or skin tags, and it is also used to treat some small tumors.

To achieve the cold temperatures needed for cryotherapy, liquid nitrogen or argon gas is passed through a tube, which becomes cold enough to freeze cells on contact. Alternatively, liquid nitrogen can be swabbed directly onto the skin or via a spray, which applies only a specific, small amount to the skin at a time.

Because the area treated is typically very small, cryotherapy is not usually painful. There may be a sensation of cold and possibly some slight discomfort. Cryotherapy is very fast and can easily be performed in a doctor's office. It also doesn't require stitches, because there is no cutting involved. These benefits make cryotherapy a good choice for many issues, including:

Common warts
Genital warts
Skin tags
Age spots
Some skin cancers
Cervical cancer
Prostate cancer
Retinoblastoma, cancer that occurs in the eye

Depending on the procedure, it usually takes three to four weeks to heal after cryotherapy. If it was done internally, patients may feel sore or have a small amount of swelling the first day afterwards. If cryotherapy was performed on the skin, patients will have a scab where their tissue was removed. Keep it clean and dry so it can heal.

Pessary

A pessary is a device that is inserted into the vagina to relieve symptoms associated with mild to moderate pelvic organ prolapse, a condition in which the womb, bladder, or rectum bulges into the vaginal canal. This device is made of either rubber or plastic and is a safe, nonsurgical way to provide support for prolapsed organs.

Pessaries come in different shapes and sizes, and finding the right one can take some trial and error. Before a pessary fitting, patients will be examined and evaluated based on prolapse-related symptoms they may be experiencing, which may include discomfort, urine leakage when sneezing or coughing, and trouble keeping tampons in.

The following tips can help guide the process of finding the right fit:

Once in place, patients should not be able to feel the device at all. If there is even slight pressure, patients should let their healthcare provider know, and she will find a pessary of a different size or shape.
The pessary should not fall out with sneezing, coughing, jumping, or squatting. While getting fitted, patients should try jumping or squatting to see if the device will fall out. If it does, a different pessary will be inserted until they find one that stays in place.
Patients and their healthcare providers should make sure the device is improving symptoms by having a check-up 5 to 15 days after it is inserted. If there is no improvement, patients may have to be re-fitted with a different pessary, or patients may be asked to consider surgical options.

The device should be kept clean to avoid issues like vaginal odor, discharge, or irritation. To clean a pessary, it must be taken out and washed with soap and water. Healthcare providers will instruct patients on how to remove and reinsert the device. If patients are unable to do this on their own, they will need to get it done at the doctor's office at least once every two months.

During my last couple of visits I very much felt like a number. Dr. Vogel came in, threw on gloves, asked basic questions, smeared and left. Didn't let me get a word in on what had came in to talk about besides my annual check up and was very rude about my desire not to use prescription contraceptives.

I've been a patient of Klara Vogel for at least a decade. I love her and her office! Since becoming members of Providence, I was concerned things would change, but they haven't. They stayed true to everything they were, and that I loved before Providence. They are WONDERFUL! And, for the record, I love Providence too! Thank you for the care you provide and for allowing all of the organizations to continue to offer premiere healthcare even after acquisition (that includes St. John's, which I have always loved too). That speaks loudly to the Providence organization on the whole. Thank you.

For context, this is for prenatal care. She is very knowledgeable but often skims over concerns and questions. When she walks in, she says "so you're good?" Which is a leading question and doesn't really invite discussion. Especially as a first time mom, I'm not really sure what is normal or how I should be feeling. So while I think I'm feeling normal for my stage of pregnancy, I really have no context. And ideally your doctor should be the one providing this context. Questions are technically answered, but briefly and without regard for your concern.

I have been visiting this practice for over 10 years and deeply appreciate Dr. Vogel. She Is incredibly knowledgeable and helpful. While I was able to get an appointment same day due to the reason I was calling, I had to wait 20-30 mins each time I called to speak to a receptionist. This never happened in all the years I've been going to this practice. I can't imagine having to call more frequently during a pregnancy. Hopefully there was just a glitch in the phone system that day.

Dr. Vogel received my uterine cancer results after she conducted my fibroid surgery. She promptly informed me and connected me with a stellar gynecological oncologist who performed my radical hysterectomy. I was impressed with the professionalism and felt that my best care was the her primary concern. She has been most recently and always quickly responsive to any needs or questions have. I feel lucky to have such a great physician especially in difficult times.

I was somewhat anxious going into this appointment because I was treated poorly during my previous appointment by the nurse who took my vitals and prepared me to meet with Dr. Vogel. However, the nurse who helped me this time was pleasant, friendly, and kind and took her time while doing my blood pressure, etc. She even took a second blood pressure reading using a different cuff size because the first one seemed off and told me what she was doing and why.

Dr. Vogel listens to me carefully and provides me with clear explanations and options for whatever problems I bring her. Her office is a safe space for me where I can ask her anything about my gynecological/sexual health and she will help me with the next step (whether it be a medication, a scan, or a blood test). I can rely on her to be there for me. Over the years, this has been such a comfort for me.

Dr Vogel has been my Gynecologist for the past 16+ years. She's always taken the time to explain procedures. Explain options and personally calls with results. On my visit there was a student Intern shadowing her with my exam. I was happy to let the student be In the room through my exam because she was learning from a very competent compassionate Dr. I am fortunate to have Dr Vogel as my Dr.

Dr Vogel is just the best. She is kind, calm, and incredibly intelligent. She is my regular OBGYN - I came in with a concern, she ordered the correct tests, made an accurate diagnoses, and quickly scheduled me in for an outpatient procedure. Always listens without rushing me, explains things clearly-no jargon-and actually remembers what we talked about last time. The staff's friendly too, and wait times are short.
