Depression

Everyone knows what it feels like to get the blues once in a while. But depression is a serious illness that is more severe than a bad day and lasts much longer. Symptoms of depression stop a person from being able to function and enjoy daily activities for weeks or months at a time. It can happen to anyone, and it isn't something that people can control by force of will or "snap out of it."

Some common symptoms of depression include:

Feeling sad, guilty, empty or hopeless
Fatigue and a lack of energy and motivation
A loss of pleasure in activities that were previously enjoyed
Unusual sleep or eating habits
Mental fog -- trouble concentrating, remembering, or making decisions
Physical aches and pains
Suicidal thoughts or a preoccupation with death

We don't yet know what causes depression, but it's thought that it is a combination of genetic, biological, environmental, and social influences. Because of this, the most effective treatments for depression combine medication with psychotherapy. Therapy, especially cognitive-behavioral therapy, can be extremely helpful in resolving the negative thoughts and feelings that come with depression. It gives patients new tools that they can use themselves to cope when their depression is making them feel down.

Some of the common medications used to treat depression include antidepressants such as SSRI's (Prozac, Paxil, Zoloft) or atypical antidepressants (Cymbalta, Wellbutrin). It's important to remember that these medications have different effects on everybody, and no one medication works right for everyone. Patients may have to try a couple before finding the one that works just right for them. If the first medication they try doesn't work, they should talk to their doctor about trying something else. In extreme cases where medication is not enough, electro-convulsive therapy and hospitalization may be the answer to keeping a severely depressed person safe.

Depression is a difficult illness to deal with, but it is more common than believed and there are many people who can help. With the right treatment, individuals with depression can get back to fully enjoying life again.

Eating Disorders

Eating disorders are mental and physical illnesses that impact behavior, emotions, and thoughts about eating, food, body appearance, and weight. These disorders are characterized by preoccupations with food and physical size, resulting in dramatic lifestyle changes and adverse health effects. In the past, eating disorders were often diagnosed using BMI (body mass index), or by assessing someone's weight. Eating disorders specialists now recognize that eating disorders may occur in people of all sizes. The most common eating disorders are anorexia nervosa, bulimia nervosa, and binge-eating disorder (BED).

Anorexia nervosa, frequently referred to as anorexia, is a disorder where people severely restrict the types and quantity of food they eat. Those with anorexia have an extreme fear of eating and gaining weight and may repeatedly weigh themselves or engage in harmful behaviors such as excessive exercise or starvation. Those with anorexia often find themselves preoccupied with food, but unable to consume a healthy amount of it. This results in symptoms such as abnormally low BMI, vitamin deficiency, fatigue, fainting, and amenorrhea (lack of menstruation in women). Anorexia may cause bluish discoloration of the hands, brittle and falling out hair, yellowish skin, and lanugo (fine, downy hair). Left untreated, anorexia can lead to organ failure, brain damage, and even death.

Bulimia nervosa, typically called bulimia, is a disorder where people binge (eat large quantities of food) and purge the food that they just consumed by vomiting, laxative abuse, fasting, or excessive exercise. This behavior is usually motivated by a feeling of powerlessness over eating and a fear of weight gain or desire for weight loss. Bulimia can occur in underweight, normal weight, and overweight people. Symptoms of bulimia include acid reflux, intestinal distress, dehydration from purging, amenorrhea, and dental erosion from vomiting. Some people with bulimia have swollen parotid glands (salivary glands near the cheeks) and red scars and marks on the backs of their knuckles. Electrolyte imbalance from bulimia can be particularly severe and may lead to heart attack or stroke.

Binge eating disorder (BED) is the most common eating disorder. People with BED feel that they are unable to control the amount of food they eat and may continue to binge after reaching a point of fullness. BED binges (consuming large amounts of food) are often accompanied by feelings of sadness, guilt, and shame. Symptoms of BED include obesity, acid reflux, weight gain, and dissatisfaction with one's body. People with BED tend to experience cycles of negative emotions and binges. Binges or BED episodes can be provoked by stress, boredom, restrictive dieting, depression, and other triggers.

Other common eating disorders include:

Pica, eating things that are not food, such as paper, ice, or soil
Avoidant/Restrictive Food Intake Disorder (ARFID), disinterest in eating and avoidance of food
Other Specified Feeding or Eating Disorder (OSFED), abnormal eating that does not fall under any disorder

Many people with eating disorders do not consciously develop them, and have difficulty seeking treatment. For this reason, it's important to detect and treat eating disorders as early as possible. Treatment for eating disorders can vary from therapy visits to inpatient treatment, depending on the disorder and its severity. Treatment aims to not only restore physical health and correct negative behaviors, but to also promote psychological well-being and self-confidence.

Shoulder Arthritis

Shoulder arthritis is a very common condition that causes pain, stiffness, or a grinding sensation in the shoulder. Some patients may hear a clicking or popping noise, and all symptoms are more noticeable when moving. Arthritis occurs when the cartilage that cushions joints becomes damaged.

Normally, when two bones come together inside a joint, they are separated by a layer of tough, smooth cartilage. The cartilage allows the bones to glide across each other, which makes movement easier. Cartilage can be damaged by an injury, by an illness such as rheumatoid arthritis, or by simple wear and tear (osteoarthritis). In the shoulder, the two joints most commonly affected are the AC joint (where the shoulder blade and collarbone connect at the outside edge of the shoulder), and the glenohumeral joint (where the arm bone meets the shoulder blade).

Treatment for shoulder arthritis may include:

Ice and rest
Exercise and physical therapy
Over-the-counter pain medications
Corticosteroid injections
Surgery

Surgical treatments include the removal of inflamed tissue and joint reconstruction. The type of arthritis and severity of the damage will determine which treatment is most appropriate.

Dr. Yuen has been my rheumatologist for the last eleven years and he is fantastic. He was the FIRST rheumatologist to EVER take me seriously, listen to me, and create a plan of care that involved and respected my input and opinions. I would NEVER change rheumatologists because he truly cares about me and my condition. Prior to meeting Dr. Yuen, I was dismissed and even told I was FAKING my condition by countless other doctors, including other rheumatologists at Pacific Medical Center. Dr. Yuen is an incredible doctor with excellent bedside manner. He cares not only about making my life easier to live while coping with my condition but he cares about me as a person, which I find to be extremely rare these days.

The doctor acts like he does not know my unique problems every visit. He worked hard to avoid providing me extra diagnosis for a tough situation. When I really pushed for help he pushed a condition which causes people to feel hot, when I explained that being hot causes my problem, which is backwards and he hoped that I would be clueless enough to accept that. I don't know what I get from going to him other than proving I have a doctor. The only thing that has helped my condition has been pain meds, and he blew me off when I asked for help finding a pain doctor. Most of the time he's trying to get out of the office quickly.

The doctor seemed thorough and did run lab work, but I waited over three months for this specialty appointment and then after the nurse/MA did her part and walked out to the doctor I heard the doctor through the door complaining about them scheduling a "new patient" right before lunch. When the doctor came into the room, I was closed off from even wanting to be there at that point. I have had a rash and swelling since getting COVID vaccines in 2020 and he was the first doctor to admit that he has seen this condition before tied to COVID & Vaccine.

I loved Dr. Yuen. He was extremely attentive and very kind. His manner put me at ease. I felt like he took my concerns seriously. I do feel that he was rushed, but I attribute that to the number of patients that are seen at PacMed daily, and not to anything that Dr. Yuen did. On the contrary, despite the fact that the scheduled appointment was brief, Dr. Yuen made time to converse with me which I appreciated. I just wish appointments were scheduled to last longer and doctors saw fewer patients per day.

I sought out Dr. Yuen at the advice of a colleague who was very satisfied with him. Dr. Yuen the provides the best care, expertise, and kind service of my former rheumatologists. I have complete confidence in him. He uses the allotted appointment time quite wisely and manages to address my questions and concerns. May I suggest, however, that the Dr. Yuen establish some eye contact during visits. I feel it would make for even more meaningful communication. Nevertheless, he's still the best.

Dr Yuen, is great. In the beginning it was hard to figure out my medication combination to help with my allergies because sometimes they are really bad. He took my allergy concerns and questions seriously and listen to me. We tried different things until we found what worked.This comment isn't related to Dr Yuen but I just want to say how disappointed I was to have to wear a mask when I walk in there. Wasn't already determined that masks don't work especially the kind that we use.

He is kind. I can tell that he cares and is genuinely concerned when he hears about my conditions.It all feels very fast when we're doing things and when he speaks, like we're trying to fit as much as possible into not enough time.He's always trying to be productive.I also really appreciate him wanting to take blood tests after our check ins when I change medications to make sure that there are no hidden adverse affects that we would've missed otherwise.

Shortly after I made an appointment through the scheduling department, Dr. Yuen's office called and moved my appointment up from March to December so I could get Insurance approval for a new medication sooner. They keep in regular contact with my other doctors and, even though my illness is fairly rare, I feel like everyone knows a lot about me and my condition. Everyone in Dr. Yuen's office was kind, supportive, and knowledgeable.

I was not given proper directions regarding my appointment and therefore was not able to get tests done after waiting 6 months for this appointment so I will need to return. This is exceeding frustrating. The provider was alright, albeit a little distracted, but the pre appointment communication was seriously lacking, especially with such a vast amount of time in between scheduling and the actual appointment

Care provider was very polite, informative, and listened intently to everything I brought up during the appointment. I felt calm the entire time we interacted. The only negative thing I experienced during the appointment was having to wait longer than expected, but it wasn't a big deal since I didn't have to be anywhere else that day. It was understandable that the doctor was busy with other patients.
