Breast Biopsy

Breast biopsy is a procedure where a small sample of tissue is collected from the breast for testing. A physician may prescribe a breast biopsy for a variety of reasons, but they are most frequently prescribed for a lump found in the breast. Biopsy samples extracted are examined by pathologists in a laboratory setting and are tested to determine if they are cancerous or benign, or indicative of some other condition. Depending on the lab results, physicians may prescribe additional treatment. Other circumstances which may prompt a breast biopsy include:

  • Mammograms that detect other unusual features in the chest, such as a cyst or calcium deposit
  • Breast scans or ultrasounds that display abnormalities
  • Nipple changes, such as scaling, crusting, or bloody discharge

Breast biopsies are collected through several different procedures and the specific type of biopsy that a patient receives depends on the size, location, and features of a breast lump or irregularity. The three main methods of collecting a breast biopsy include:

  • Fine needle aspiration (FNA biopsy), where a thin needle is directed into a breast lump or abnormality, extracting a small sample of tissue or liquid
  • Core needle biopsy (CNB biopsy), where a hollow needle is directed into a breast lump or abnormality, extracting cylinders of tissue called "cores"
  • Open (surgical) biopsy, where a breast lump is removed through an incision in the breast

Fine needle aspiration (FNA) and core needle biopsy (CNB) are sometimes image-guided, where a mammogram, ultrasound, or MRI helps direct the physician performing the needle extraction.

Fine needle aspiration, the most common biopsy method, generally requires few patient preparations. It is recommended patients avoid using lotion, deodorant, and perfume prior to their biopsy procedure. FNA biopsy is performed with local anesthesia (topical numbing cream) and typically has a quick recovery, but patients may experience soreness for a few days. Core needle biopsy extracts slightly more tissue than an FNA biopsy.

Open (surgical) biopsy is more invasive and requires general anesthesia, where patients are put to sleep during the procedure. Patients must fast prior to the surgery. After the biopsy is performed, patients are monitored in a recovery room for a time before being discharged, when they will need someone else to provide transportation.

Although some may consider breast biopsy to be a simple procedure, it can catch critical issues - particularly breast cancer - that impact wellness. If a breast biopsy reveals an underlying condition like life-threatening cancer, patients can receive the diagnosis and medical treatment that is necessary for their long-term health.

Hepatectomy (Liver Resection)

Hepatectomy, or liver resection, is a surgical procedure that removes all or part of the liver to treat cancer or other diseases. The liver is divided into one left and one right lobe, and each lobe is divided into segments. The amount of liver removed is dependent on factors such as the size, location, and number of tumors or other lesions. The following are types of liver resection:

  • Wedge resection, which removes a small piece of the liver for small lesions
  • Segmentectomy, which removes a single segment of the liver, allowing the surgeon to treat tumors in multiple places without removing too much of the liver
  • Trisegmentectomy, which removes around 70% of the liver
  • Lobectomy, which removes the entire left or right lobe

Liver resection can be performed laparoscopically or through open surgery. Laparoscopic liver resection is a minimally invasive surgery that requires a few tiny incisions on the abdomen, through which the surgeon inserts a thin tube with a camera (laparoscope). Open hepatectomy is performed through a laparotomy, which makes one large incision on the abdomen. Although a laparoscopic liver resection is associated with less pain and quicker recovery, patients may have to undergo an open liver resection depending on factors such as the location of the tumor and previous abdominal operations they have had.

After the liver resection, patients will stay in the hospital for up to six days. The recovery period will vary based on how much of the liver was removed. Limiting consumption of alcohol and light physical activity, such as walking, can help patients to have a speedy recovery.

Pancreatectomy (Pancreas Removal)

Pancreatectomy is surgery to remove part or all of the pancreas to treat pancreatitis, tumors, or cancer. The pancreas is located behind the stomach and near the small intestine and spleen. There are three sections of the pancreas:

  • The head, which is the portion closest to the intestines
  • The body, which is the middle portion of the pancreas
  • The tail, which is the portion nearest the spleen

Pancreas removal procedures differ based on how much of the pancreas and surrounding organs are removed. There are three types of pancreatectomy:

  • Distal pancreatectomy removes the tail and body of the pancreas. This procedure treats tumors confined to these areas.
  • Pancreaticoduodenectomy, or the Whipple procedure, removes the pancreas, duodenum, bile duct, and, sometimes, part of the stomach. The Whipple procedure is used for pancreatic tumors that have not spread past the head of the pancreas and for other conditions affecting the pancreas, duodenum, and bile duct.
  • Total pancreatectomy removes the whole pancreas along with the duodenum, gallbladder, parts of the stomach and bile duct, surrounding lymph nodes, and, potentially, the spleen. Rarely performed, total pancreatectomy treats conditions that involve the entire pancreas, such as tumors found in multiple sites on the pancreas, or for rare tumors such as intraductal papillary mucinous neoplasms (IPMN).

Pancreatectomy can be done by open or minimally invasive surgery. During a laparoscopic pancreatectomy, the surgeon will use tiny incisions to insert a thin tube with a camera, or laparoscope, into the abdomen. Open surgery, or a laparotomy, is performed through a larger incision on the abdomen. If additional organs are removed, some procedures may require that the small intestine be reconnected to the stomach.

The hospital for pancreatectomy stay may be between 1-3 weeks. Generally, laparoscopic surgeries require shorter recovery periods. Whether patients undergo an open or laparoscopic procedure is determined by factors such as their condition and the extent of their procedure. For example, open surgery tends to be more common for distal pancreatectomies. If patients have difficulty eating, they may require a nasogastric tube, inserted through the nose, to carry food to their stomachs. A catheter can be inserted into the bladder if patients have trouble urinating. Full recovery may take about two months.

Dr Welsh took my port out on this visit-explained in advance what to expect from beginning to end. She made the port removal very easy and I had very little bruising and no problems as a result of the port. Dr Welsh also did my bilateral mastectomies and I did not have any difficulties following the removal. She is kind, listens to your concerns and gives you helpful information. I had initially consulted with another woman surgeon at NW Cancer over by Sacret Heart and left there extremely discouraged and beat up. I found Dr Welsh, consulted her and immediately felt she understood me and was going to take good care of me. She did just that and myself and husband were extremely pleased with her skill and kindness!

This provider and her entire office has been the best experience I have had with Providence. The care from the moment I walked in the door my care was exceptional. I was treated with respect, empathy and kindness. The provider took the time to actually review my medical history prior to seeing her. She actually knew what she was talking about when it came to my care and made me feel very comfortable. The receptionists went out of their way to make me feel comforted in my care and they knew what they were talking about as well. Nice and refreshing compared to the other care I receive at Providence.

The appointment I had with Dr. Welsh was easily the best experience I've had with a doctor. She took enough time with me to carefully explain all of my test results and images. She patiently answered my questions and was knowledgeable about the care I'd receive from other doctors, such as my oncologist and plastic surgeon. She was amazing. Since my appointment, she's quickly added comments to all of my test results that show up on my chart. Her comments are helpful, thorough, and easy to understand. I am SO happy that she is the doctor I'm seeing during this difficult time.

Dr Welsh gave care and information above my expectations. Professionally, she seems to be the best! In addition, she was kind, gentle, and patient. She looked me in the eye as she talked with me. She allowed me time to ask all my questions. I was stressed because we were discussing possible breast surgery that could reveal cancer; Dr Welsh realized that and did her very best to put me at ease. She couldn't have been better!

Dr Welsh is very kind, understanding and empathic. She does a great job explaining options and the future outlook. And I am in a post Covid study which does a lot of bloodwork that posts to mychart. She looked and that and mentioned something's with that. I was surprised because IHave had some abnormal results which I don't think my pcp's office noticed. If they did they didn't contact me.

Dr.Welsh explained the medication I was taking post surgery and what it does to cancer particles in the body. That helped me to understand why it is good medicine to take it. She doesn't prescribe it for me, another one did. I so appreciate how is so very thorough with explanations for and about procedures Dr. Welsh is. She is a wonderful surgeon and very intelligent on many levels.

Dr Welsh is a breast surgical specialist. She is very knowledgeable, skilled, and experienced. Dr Welsh is personable and in the time I have been her patient she has gotten to know me she remembers the things I have gone through with her she remembers my history and reviews it with me. I could not ask for a nicer or more dedicated and knowledgable breast specialist.

Dr Welsh is the kindest, most compassionate yet thorough doctor in explaining technical medical issues I have ever seen. She is sensitive and yet does not minimize potential complications.
