Biopsy

A biopsy is a procedure performed to remove a small sample of cells for testing. The cells are examined in a laboratory to check for disease, or sometimes to see how badly a known disease is affecting them. Biopsy can be performed on any part of the body.

Although biopsy is most often associated with cancer, it can also be used to check for other diseases, such as infections. A biopsy sample can be scraped, cut, collected with a needle, taken with a machine that punches out a tiny piece, or removed with the tiny tools in an endoscope.

In most cases, a biopsy is a simple outpatient procedure. Depending on the procedure, patients may need to stop taking certain medications beforehand, such as blood thinners. It is important for patients to tell doctors if they are pregnant, as certain biopsies require the use of x-rays to guide the needle to the right location. There is usually very little pain associated with a biopsy. Patients will have to wait a few days to hear the results.

Kidney Stones

Kidney stones are hard deposits that form in the kidneys, made up of minerals that are normally present in urine. They can vary in size, from as small as a grain of sand to as large as a nickel, occasionally even larger. Sometimes they lodge in the kidney, and sometimes they break free and make their way out through the urinary tract, which can be extremely painful.

Kidney stones can be smooth or jagged and are yellow to brown in color. They are mostly comprised of the minerals calcium, oxalate, and phosphorus. Examining the stones to see what they are made of can show what caused the stone to be formed in the first place. For example, a stone made of mostly calcium, which is the most common type, can happen any time the urine becomes too concentrated due to dehydration or a blockage in the kidney. A uric acid stone forms when acid levels in the urine get too high, usually due to excessive consumption of animal protein such as meat and fish. A struvite stone is a sign of certain infections, and a cystine stone can be due to a genetic disorder that raises the risk of kidney stones.

The most common symptom of kidney stones is pain, either in the back or lower abdomen, or severe pain when urinating. There may also be blood in the urine. Treatment for kidney stones depends on how large the stone is. Very small stones can pass out of the body on their own, and they do not require treatment other than drinking adequate water and taking pain killers. Larger stones need to be broken apart and removed. The main treatment options are:

Shock wave lithotripsy, which uses sound wave vibrations to break apart the stone
Percutaneous nephrolithotomy, or the use of a very tiny tool (like a wire inserted through the back) to break apart and remove the stone
Ureteroscopy, a thin tube inserted through the urethra and bladder to the stone, where tiny tools can grasp the stone and remove

People who have had one kidney stone are at risk of developing another. To reduce this risk, patients are given instructions specific to the type of stone they developed. Generally the instructions will include drinking more water to dilute the urine, but it may also involve lowering sodium intake or eating less meat.

Urodynamics (Bladder and Urethra Function Test)

Urodynamics are tests that assess how well the lower urinary tract is functioning. Specifically, they evaluate its ability to hold and release urine, as well as show if any blockages or leaks are present. The lower urinary tract is made up of the following:

The bladder, which stores urine.
The urinary sphincters, which are muscles around the opening of the bladder that hold urine in.
The urethra, which is the tube where urine flows out of the body.

Urodynamics tests are normally performed together as one series, but depending on patients' symptoms, only one or some of them may be necessary. The most common tests are:

Cystometrogram, which measures the amount of urine the bladder can store and how full it is before someone has the urge to urinate. Doctors may order this test if she suspects their patient has a weak bladder.
Pressure flow study, which measures the pressure in the bladder while urinating. This test helps locate a blockage in the bladder.
Uroflowmetry, which measures how much and how fast urine flows out of the body. This test is usually recommended when patients have difficulty urinating or they have a slow stream.
Urethral sphincter electromyography, which measures how strong the muscles and nerves in the bladder and sphincters are. Doctors may request this test if the problem they are experiencing is linked to muscle or nerve damage in the lower urinary tract.
Urethral pressure profile, which measures the pressure along the urethra while the bladder is gradually filled with fluid. Low pressure could mean someone has a weak urethra, a contributing factor to urinary incontinence (loss of bladder control).

For a few hours following a urodynamics study, patients will likely feel some soreness or discomfort. It is recommended to drink at least one glass of water every half hour for two hours to relieve these symptoms. Holding a warm, moistened washcloth over the area where discomfort is felt or taking a warm bath may also help. Results for cystrometrograms and uroflowmetry are typically available immediately after the test, but results for the other tests may take a few days. When doctors have reviewed a patient's test results, he or she will discuss them with the patient, along with any treatments the patient may need.

Dr. Evans is truly one of the most compassionate and empathetic doctors I've ever seen. He really takes the time to listen, and the visit was not rushed. He is great with eye contact, and I felt he listened to every word I said. At the appointment, he recorded his summary while I was there, and asked if there was anything I wanted to add. He is great at explaining medical things in a normal person way! But let me tell you one thing, that has never happened to me before, at any doctor's office. Dr. Evans walked me out and held the door to the lobby for me. Every step of the way, he wanted to make sure I understood, and that all my questions were answered. That really blew me away, not that I needed it, but that he did that.

I was very satisfied with my first visit with Dr. Evans. I was referred to him to investigate concerns and results of an ultrasound exam. He was extremely warm and friendly upon meeting me. He had prepared very well for our visit. He explained in detail what he'd observed and correlated it with history going back more than ten years. He then clearly and succinctly outlined a treatment strategy and scheduled a follow-up. I left his office smiling! I am used to excellent care and service from my team at Swedish, but Dr. Evans exceeded even those high expectations. Thanks!

Dr. Evans is outstanding. He is kind, patient, professional and he really knows his stuff. This was my first visit with him. I'm grateful that I will be in his care. Scheduling an appointment with Swedish Urology is super hard. It's hard to even get through to talk to someone. Reaching the clinic takes a long time, even if one is just needing directions. So I'd say you're in serious need of more support staff. But the medical care, so far, has been outstanding.

Great experience! Dr. Evans was very professional and caring. He did an excellent job of reviewing the scans before we met and did a fantastic job of walking through the scans so we understood what we were looking at. We have worked with Dr Evans before on a very complicated and difficult kidney stone. He did an exceptional job on that. Would highly recommend Dr Evans. He is a very talented Dr. And we are blessed to have him as a Dr.

This was my first visit with Dr. Evans. He gave me information and help of understanding what was going on with me and immediately started a game plan as to how to treat my condition. This is more in one visit with Dr. Evans than I've received in the last seven years with Harborview. It was a breath of fresh air and I have a new feeling of hope again.

Doctor Evans has a very good manner about himself. It is also evident he is very good at his trade but still is able to communicate to me in a way I understand. He has followed me through tests that were not positive to now better health and made sure I had proper testing for many years. I hope to keep him as my urologist.

Dr Evans was very helpful in explaining my cancer risks and pointing out that if I elect to start taking Testosterone treatments that I would need to continue with them into the future, as the body will no longer provide it if it's given manually. He has a great personality, good energy, and was a pleasure to work with.
