Autoimmune Diseases

An autoimmune disorder happens when the immune system mistakenly attacks the tissues of its own body, causing symptoms of illness. There are more than 80 different types of autoimmune disorders. While some are very rare, others are fairly common. Combined, autoimmune disorders are one of the leading causes of death and disability in the United States, affecting approximately 24 million people.

A properly working immune system identifies foreign substances in the body that might cause illness, such as bacteria and viruses. The immune system then creates antibodies which attack the foreign substances, neutralizing them and keeping the body safe. In people with autoimmune disorders, something goes wrong with this process. For reasons that are not fully understand, the immune system creates antibodies to attack the patient's own tissues.

Symptoms of an autoimmune disorder depend on which tissue is being attacked by the immune system, but common symptoms of autoimmune disease include fever, fatigue, and a general feeling of just not being well. Autoimmune disorders are more common in women than in men, and they may run in families. Autoimmune disorders can affect various parts of the body such as blood vessels, connective tissue, endocrine glands, joints, muscles, red blood cells, skin, and many others.

It is common to have more than one autoimmune disorder at a time. Most are chronic, or life-long illnesses, although they may come and go in flares. Treatment for autoimmune disorders depends on which part of the body is being attacked. For example:

A type 1 diabetic whose pancreas has been damaged will need insulin.
A person with Hashimoto's whose thyroid has been damaged will need replacement thyroid hormones.
Someone with Sjogren's syndrome will need eye drops and mouth rinses to replace tears and saliva.

Many autoimmune disorders of all kinds are treated with immune-suppressing medications, such as corticosteroids (e.g. prednisone) to reduce the effect of the immune system.

Heart Attack

A heart attack is a serious medical event that happens when the blood supplying oxygen to the heart is cut off, usually because of a blockage in an artery. This results in damage or death of part of the heart muscle. The heart can heal, but like any part of the body that was injured it leaves a scar. This scar tissue does not contract effectively, so the ability of the heart to pump is lowered after a heart attack.

A heart attack, sometimes called a myocardial infarction, can happen without warning. However there are certain factors that raise the risk of heart attack. Some of these are:

Age
Gender (most heart attack sufferers are male)
A family history of heart attacks
Smoking
Obesity
High cholesterol
High blood pressure
Diabetes
Unhealthy lifestyle choices (lack of exercise, poor diet, too much alcohol or stress)

Heart attacks typically feel like pain or pressure in the chest that can radiate out to the back, arm, or jaw. Some other symptoms can include fatigue or weakness, shortness of breath, sweating, and nausea.

Treatment aims to increase blood flow to the heart muscle by opening up blocked blood vessels. Medications are sometimes used to dissolve clots in the arteries. Aspirin and other medications such as Plavix can thin the blood and make clots less likely to form. Dilators can widen blood vessels to make room for more blood to flow. Doctors can also use a procedure called catheterization to look at the blood vessels around the heart directly, place stents, or widen vessels using angioplasty. In extreme cases bypass surgery can be used to help blood flow better. After recovery, it is important to lead a healthy lifestyle and take any prescribed medications in order to protect the heart.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

Dr. Medvedev: A Cardiologist ExtraordinaireI have been under the care of Dr. Medvedev for over two years and without hesitation, I can say he is the absolute best doctor I have ever encountered. I have concerns about the providers being stretched so thin and the stress, unnecessary as it is, it places on them when additional healthcare providers can be brought on board to help alleviate this issue. Dr. Medvedev was so busy we ended up waiting in an office for approximately an hour after our scheduled appointment time, but Dr. Medvedev appologized politely and proceeded with the appointment professionally and in a most pleasant manner despite the pressure placed upon him. Dr. Medvedev is a wonderful, caring Doctor . Thank you.

The whole experience was amazing, I didn't know what to expect, since I hadn't been there before, and I have not always had amazing visits with Specialist, but I felt at ease and impressed by the amount of time the doctor took to sit with me and answer all my questions And make sure I was on board with the plan. The gal who checked me in and did my EKG was also super friendly and put me at ease immediately. I will recommend this practice to anyone who needs to see a cardiologist.

I was very impressed with this doctor. I am 73 years old and people are always asking me when am I going to retire. I get very upset and explained to people that I have a purpose, a reason to get up In the morning. I always ask what else am I'm going to do? Sit around and watch telvision all day. He was the only person who complemented me and told me to keep working..... In addition, he walked with me and we talked about food. We both like lamb.

Dr Medvedev is an outstanding cardiologist. Knowledgeable and patient. His grasp of my history and current issues is extraordinary. He offered concrete and constructive interventions for my AFIB and heart pain and answered all of my questions and concerns. I am so grateful for his support band guidance. Dr Medvedev called me after a fall and a visit to Swedish Edmonds ER and suggested that I have a Watchman device placed in my heart. He also contacted Swedish Heart and Vascular about me. I was able to get the Watchman procedure done very quickly because of his intervention. Initial contact with Swedish Heart and Vascular was last Tuesday. I had the procedure on Friday. That's amazing! Thanks to all involved with my care!

Dr. Medvedev inspires my confidence. He addresses my condition candidly and yet kindly, even when discussing the effects of my obesity on my heart. I also want to call out Dr. Medvedev's assistant, who was exceptionally cheerful and attentive, as well as the office staff. Even as they were dealing with an unwieldy computer system to schedule the tests I needed, they were considerate of my comfort and time.

Dr Medvedev medical treatment has helped me to enjoy a very active energetic life. At 72 years old, I am excited about my future. I am getting stellar medical care. Dr Medvedev put the stints in for my 3rd heart attack and stretched 3 undersized stints previously installed by other cardiologist previously. Since that time, no short of breath or oxygen When I work or go on long walks or climb hills.

This was a first appointment with Dr. Medvedev. I was the most efficacious medical appointment I have ever had. He was totally prepared and was familiar with my previous cardiology treatment and testing. I learned a great deal about my heart issues and so appreciated a clear path forward in my treatment. I'm new to the area so comparing my care here to my care previously, in Billings, MT. This past appointment with Dr. Ivan was excellent. I find him informed, highly capable (seems that way to me) and seems to care very much. He treats me like a patient he cares about - NOT like a number. I would recommend him to anyone looking for a heart doctor.
