Headache

Headaches are dull or sharp pains that occur in regions of the head and face. Headaches occur in many different forms and vary in location, severity, and duration. They are not necessarily a sign of an underlying illness and often resolve on their own. However, headaches can present significant day-to-day discomfort. The most common forms of headaches include:

Migraines: one of the most prevalent types of headache, migraines typically last from two hours to three days, causing a throbbing pain on one side of the head. Nausea, sensitivity to bright lights, fatigue, and blurred vision all may accompany migraines. While migraines have no known cure, medications and lifestyle modifications may ease symptoms. Avoiding behaviors that may trigger migraines such as lack of sleep or loud, repetitive noises can improve headaches.
Tension headaches: also known as stress headaches, these headaches are characterized by pain in the upper top part of the head and behind the eyes. Tension headaches are common and have no exact cause; however, certain practices may increase the likelihood of developing them. For example, wearing one's hair up in a tight hairstyle or wearing a restrictive hat can put pressure on the scalp, leading to a tension headache. Other causes of tension headaches may include alcohol or nicotine use, caffeine, teeth grinding, stress, overexertion, and eye strain. Some people find posture improvement, alternating sleeping positions, and neck exercise can alleviate tension headaches. Over-the-counter pain relievers such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve) may also reduce tension-headache pain.
Sinus headaches: this type of headache causes pain in the sinuses, but may progress to pain and pressure in the eyes, forehead, and cheeks. These headaches are often accompanied by nasal congestion, fatigue, and ache in the upper teeth and may or may not be caused by sinus infections or sinusitis (sinus inflammation). Some sinus headaches are allergic headaches, which arise from allergic triggers such as seasonal allergies (pollen allergies) or hay fever, causing sinus pressure and facial pain. Other symptoms of allergic headaches include nasal congestion, irritated or watery eyes, runny nose, and sneezing. Allergic headaches may be treated with antihistamine medications such as cetirizine (Zyrtec). Allergic headaches are common during pollen season, which is usually around springtime.
Cluster headaches: these headaches occur suddenly and cause stabbing, sharp pain from one eye down to the one side of the neck. They are often referred to as the most aggravating and painful type of headaches. Other symptoms of cluster headaches include nasal congestion, droopy eyelid, and enlarged pupil on one side of the face. In the past, it was widely believed that cluster headaches were caused by a bevy or "cluster" of multiple types of headaches, such as a combination of an allergic headache and a tension headache. Medical experts now recognize that cluster headaches are a separate and unique form of headache. Cluster headaches last for around twenty minutes to two hours. They may recur throughout the period of a day or several weeks, but usually dissipate within a month. Treatment for cluster headaches includes over-the-counter pain medications such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve). Prescription Prednisone can help with swelling and Verapamil may reduce pain by relaxing the blood vessels.

Living with headaches is challenging; headaches can prevent people from fully enjoying life. Medical treatment and care in avoiding certain triggers can help those with headaches begin to feel normal once again.

Joint Injections

A joint is any area of the body where two bones connect. Due to injury or disease, the space between the two bones can sometimes become swollen and inflamed, which leads to pain and a loss of mobility. Injection and aspiration are two tools that physicians use to treat joint pain locally without needing to perform surgery.

Both injection and aspiration are techniques that involve inserting a needle connected to a syringe directly into the joint. Aspiration involves the removal of excess fluid, and injection is the placement of medication directly into the joint space via the needle. Both procedures may be performed at the same time. In both cases a local anesthetic may be used, the skin will be cleaned and disinfected, and then the needle will be inserted. In some cases, especially if the injection or aspiration is in a large and deep joint such as the hip or spine, ultrasound may be used to guide the needle to the exact location desired.

In some cases, irritation to the joint can cause fluid to build up so significantly that the joint hurts and can no longer move well. Aspiration removes some of the excess fluid and relieves the pressure. In addition, aspiration can be used to provide a sample of joint fluid if it needs to be examined microscopically for the presence of white blood cells, bacteria, or crystal formations.

Certain injuries and diseases that affect joints are inflammatory in nature, such as rheumatoid arthritis, tendonitis or gout. These diseases may be helped by the local injection of anti-inflammatory medications directly into the affected joint. In this case, corticosteroids such as methylprednisone are usually used. Relief from the pain may be felt right away and may last for weeks or even months.

In some cases, such as with osteoarthritis, the cartilage buffer between the two bones wears down and pain comes from bones rubbing against each other. In this case, injection of a lubricating agent such as hyaluronic acid may be beneficial. It provides a slippery cushion between the bones to relieve pain that can last for months.

It quickly came to lite that Dr David had reviewed my records going back at least to 2018. There he saw my previous Dr had requested a bone scan in 2018, which I took. The previous Dr informed me all was good. However, Dr David saw previous Dr incorrectly informed me and that the bone density test actually showed I had osteopenia and needed treatment which I never received. Dr David surmised, had I been accurately informed and given the opportunity for treatment, the four fractured vertebrae (from recent fall) would not have been as severe as they are and likely fewer fractures. This assured me Dr David was vested in my health and what was best for me. I'm looking forward to the Kyphoplasty he'll perform within the next few weeks to stabilize my spine and significantly reduce my pain. He also referred me to another Dr to start treatment for the osteopenia and physical therapy. Looking forward to working with Dr David.

I have complete confidence in Dr David. On three occasions in the past 3 1/2 years I have literally put my life in his hands. He is a competent Medical provider, caring, and a good communicator. He has many tools in his medical arsenal. He reviews each patient's case, presents possible alternatives, and assists the patient in making appropriate decisions. I always have many questions and when facing surgery on my back I am very very nervous. Dr David is understanding, patient, and tells me what he is doing every step of the way. I trust him completely. I have shared my experiences with Dr David with several friends who I feel might benefit from his expertise and have encouraged them to make an appointment for a consultation. I highly recommend Dr David in matters related to pain management involving the spine.

Dr Glen David and his staff have been very helpful in setting up appointments both at the hospital and the pain clinic as well as via Zoom. They keep appointments in a timely fashion. They listen attentively and with concern, explain diagnoses and procedures clearly and in easy to understand terms. I feel confident of their knowledge, their expertise and their desire to help me keep my abilities and independence as long as I can. I appreciate that they take the problem of pain seriously. This is the third time I have encountered Dr. David. The first time was for a consultation were I found him to be very friendly and attentive. The second time was for a follow up after the first procedure where I found him to be preoccupied (possibly in a bad mood) and dismissive of me. The third time was for a repeat of the first procedure where he only briefly interacted with me. I don't doubt that he is excellent at his medical profession but I feel he needs to work on his bedside manner' and realize that his patients are real people with valid feelings.

I n addition to being an excellent Physiatrist Dr David is very personable and fully engaged. He always takes the time to explain any procedure and what to expect. His interpersonal skills always put me at ease and enhances the confidence I have in care I receive. I would also like recognize the support staff especially the person who deals with the insurance. My procedures require a peer to peer review at a time when my pain level is highest. This is always handled in a seamless and expedient way. Way to go y'all.

Dr. David was helpful in some ways with determining next step actions to figure out my medical situation, but his bedside manner needs improvement. When a patient is seeing you because of chronic pain, telling them they're "lucky" because X other thing isn't wrong is not helpful. He also did not ask me many questions to understand my pain better and started my appointment out questioning why I was seeing him - when I had been referred by another doctor. The appointment was stressful and I left feeling overwhelmed.

I had never met Dr. David before. I found him on my own as I wanted "the best". I needed a kyphoplasty procedure "URGENT" as time was of the essence. I was contacted within a week of my primary care physician's referral and was seen the very next day. The staff was personable and very efficient; Dr. David was the same. Although I was unhappy with some of the suggestions regarding lifestyle changes (due to my medical conditions) I respect and appreciate his concern and forthright manner.

After meeting Dr. David for the first time, I do have complete trust in his ability to give me the care I need. ( The only problem is that I've already waited over a week for the person who schedules nerve blocks to call me, which I'm sure is no reflexion on Dr. David. I'm posiitive that my insurance does not take that long to authorize procedures.) I do know that Dr. David is tops in his field and comes highly recommended. He also is a very kind person and extremely easy to talk to.

My appointment was recommended by my neurologist team to address pain from bursitis in left hip- cortisone injection. I was under the impression that Dr David would explain the issue and help me understand where it was coming from. I was also hoping to discuss a treatment plan to address any future flare ups. Unfortunately, I didn't get a chance to do this. He performed the injection and the appointment was over, it felt rushed. I was hoping to have some of my questions answered.

I felt like I had a really engaged and informative conversation with Dr David Glenn, he made sure I understood the complicated medical situation that I've been experiencing and he left me with a tremendous amount of hope for my future care with with his medical team at swedish. He said my prior authorization could take from 2 weeks to 2 months for insurance to approve, but that he would advocate for me on my behalf to insurance and that made me feel really well taken care of.
