Gynecologic Surgery

Gynecologic surgery is surgery performed on a woman's pelvic region. It is usually performed by an OB/GYN and can involve the bladder, rectum, or reproductive organs. Surgery in this area may be performed for many reasons, but the most common procedures include:

Hysterectomy
Fibroid removal
Treatment of endometriosis or other heavy bleeding
Cancer treatment, including cervical, uterine, and ovarian cancers
Removal of ovarian cysts
Infertility treatment
Treatment for incontinence
Sterilization or 'tube-tying'
Surgery for pelvic organ prolapse
Repair of vaginal tears

More and more, gynecologic surgery is moving towards the use of minimally invasive surgery. Minimally invasive surgery uses several small incisions and tiny tools and scopes to perform the operation, rather than one large opening. While it is not appropriate in every case, minimally invasive surgery can lead to less scarring, less blood loss, faster recovery, and a lower chance for infection. Minimally invasive gynecologic surgery may be performed with a laparoscope, a thin tube with a camera on the end that allows the surgeon to see inside the body. It may also be performed robotically, using even smaller tools and cameras that a surgeon controls from a computer. Robotic surgery allows even better vision, precision, and control than laparoscopic surgery.

Over the course of a lifetime, many women need pelvic surgery to stay healthy. Qualified gynecologic surgeons will have the right tools at their disposal to provide care with a minimum of discomfort.

Pelvic Organ Prolapse

Pelvic organ prolapse is the bulging of pelvic organs into the vaginal canal due to a weak pelvic floor, causing symptoms like discomfort, pain, urinary problems, and constipation. The pelvic floor is a group of muscles that support the pelvic organs, which are made up of the bladder, rectum, small bowel, uterus, and vagina. Vaginal childbirth, menopause, pelvic surgery, radiation treatments, or being extremely overweight may cause the pelvic floor to weaken. Pelvic organ prolapse is a very common disorder among women, particularly for those who are over 50.

The choice of treatment for pelvic organ prolapse depends on the severity of the condition. Mild to moderate prolapse may be managed by doing pelvic floor strengthening exercises, taking hormone replacement therapy, or using pessaries. Pessaries are small devices inserted into the vagina to help support the pelvic organs. Pessary fitting is a quick procedure done in a doctor's office. For severe prolapse, surgical treatment may be necessary. Pelvic organ prolapse surgery repairs the following:

Cystocele or bladder prolapse, where the bladder sags into the vagina. The surgeon repairs a cystocele by strengthening the front wall of the vagina using stitches so that the bladder will no longer sag.
Enterocele or small bowel prolapse, which occurs when the small bowel descends towards the vagina. To correct an enterocele, the tissues between the small bowel and the vagina are sewn together, allowing them to provide more support.
Rectocele or posterior prolapse, which is the bulging of the front wall of the rectum into the back wall of the vagina. Surgery to repair a rectocele removes the excess stretched tissue between the rectum and vagina.
Uterine prolapse, which occurs when the uterus slips down to the vagina. To treat a uterine prolapse, the surgeon may secure the neck of the uterus (called cervix) to a ligament in the pelvis using stitches. Another option for this type of prolapse is a vaginal hysterectomy, which is the removal of the uterus.

Each of these procedures is performed through a vaginal incision, but an enterocele repair may use the abdominal approach as an alternative. If two or more pelvic organs have prolapsed, a combination of any of these procedures may be performed during the same surgical session.

For women who no longer wish to have intercourse, an operation called colpocleisis

may be a surgical option as well. By closing the vaginal canal, colpocleisis treats pelvic organ prolapse and eliminates any risk of its recurrence.

In most instances, pelvic organ prolapse surgery only requires a hospital stay of one day. Patients may experience vaginal bleeding for the first few weeks following surgery. If this happens, patients should use sanitary pads rather than tampons, as a higher risk of infection is associated with the use of tampons. To further help with recovery, patients should begin doing gentle pelvic floor strengthening exercises a few days after surgery. Doing these exercises at least three times a day as a routine tightens the pelvic floor muscles and prevents the recurrence of prolapse.

Urodynamics (Bladder and Urethra Function Test)

Urodynamics are tests that assess how well the lower urinary tract is functioning. Specifically, they evaluate its ability to hold and release urine, as well as show if any blockages or leaks are present. The lower urinary tract is made up of the following:

The bladder, which stores urine.
The urinary sphincters, which are muscles around the opening of the bladder that hold urine in.
The urethra, which is the tube where urine flows out of the body.

Urodynamics tests are normally performed together as one series, but depending on patients' symptoms, only one or some of them may be necessary. The most common tests are:

Cystometrogram, which measures the amount of urine the bladder can store and how full it is before someone has the urge to urinate. Doctors may order this test if she suspects their patient has a weak bladder.
Pressure flow study, which measures the pressure in the bladder while urinating. This test helps locate a blockage in the bladder.
Uroflowmetry, which measures how much and how fast urine flows out of the body. This test is usually recommended when patients have difficulty urinating or they have a slow stream.
Urethral sphincter electromyography, which measures how strong the muscles and nerves in the bladder and sphincters are. Doctors may request this test if the problem they are experiencing is linked to muscle or nerve damage in the lower urinary tract.
Urethral pressure profile, which measures the pressure along the urethra while the bladder is gradually filled with fluid. Low pressure could mean someone has a weak urethra, a contributing factor to urinary incontinence (loss of bladder control).

For a few hours following a urodynamics study, patients will likely feel some soreness or discomfort. It is recommended to drink at least one glass of water every half hour for two hours to relieve these symptoms. Holding a warm, moistened washcloth over the area where discomfort is felt or taking a warm bath may also help. Results for cystrometrograms and uroflowmetry are typically available immediately after the test, but results for the other tests may take a few days. When doctors have reviewed a patient's test results, he or she will discuss them with the patient, along with any treatments the patient may need.

Dr. Lindo and her staff were amazing! I walked right in to my exam room at the scheduled appointment time. The nurse was thorough in reviewing my reasons for the visit and helping set the expectations of what would come next. During the entire visit and exams, Dr. Lindo explained everything and answered all of my questions. The after-appointment follow up has been fantastic as well. No one wants to talk to a stranger about her pelvic disfunction, but Dr. Lindo and her staff made it a comfortable, safe place to be open and honest. I feel like she's my partner in helping me get this part of my life back under control!

Her notes in my medical records misstate the facts and dishonor me as a person. She claimed I wanted to do laparoscopy while awake, when in fact I was not talking about being awake during laparoscopy but about a different procedure. She got it mixed up and made it sound like I was insane.
She insists that a nurse comes to the room to witness a gynecological exam and listen to the discussion. Her nurse told me it was "for legal reasons". In my records I read that the Dr called me "semi-cooperative" because I objected a nurse walking in for comfort reasons. I had no problem with Drs examining me without a "witness" in the past and there's nothing strange about being modest and asking for privacy during the exam. There are many women like that and many are afraid to speak up.
She also made objectionable remarks about me in my records and said I appear "anorexic" I've never suffered from anorexia and did not complain to her about any eating problems. I have normal weight for my height. She even made a comment in the notes about a small scar revision I did for cosmetic reason on an entirely different part of the body.
She could not do the type of procedure I read about other Drs do and offered to fix a prolapse by suspending the top only and not fixing all the defects. She also offered to use synthetic materials for a surgery.
This Dr made it sound like I was unreasonable and misstated what I said about my wishes and produced the kind of records on me that dishonor me as a person and could potentially prevent me from getting care from other providers. She should be ashamed for doing that to a person who trusted her and came to her for help. At the very lease she should have avoided doing harm and I feel like she did harm to me with the kind of opinionated notes she left. I cannot show this kind of records to other providers. On the other hand, she was praising herself all over the records for how much time she spent with me and for giving me "reassurance", making herself look good as compared with all the negative remarks about me.
I'm leaving a review about her online so there's also a record for her other potential patients and I'm hoping she will change her ways and treat her patients with more kindness, understanding and respect and would ask questions if something is not clear before putting incorrect statements in the records.
