Adrenalectomy (Adrenal Surgery)

Adrenalectomy is a surgical procedure to remove the adrenal glands, which lie above each kidney. The adrenal glands are responsible for producing hormones that are necessary for bodily functions, such as the regulation of blood pressure and metabolism. If the adrenal glands overproduce certain hormones, or if a tumor grows on them, removal of the glands is a treatment option.

Adrenal surgery can be open or laparoscopic. Laparoscopic adrenalectomy is a minimally invasive procedure that inserts a thin tube with a camera (laparoscope) through several small incisions on the abdomen. The surgeon uses the laparoscope to guide the procedure and is able to access and remove adrenal glands through the small incisions. Laparoscopic adrenal surgery lasts approximately 1-2 hours.

Laparoscopic adrenalectomy is associated with shorter recovery periods, reduced risk of infection, and smaller scars. Although these factors may be desirable, the location of the tumor, its size, and the severity of the condition may require that an individual instead undergo open surgery. An open adrenalectomy is typically done for larger adrenal tumors and requires a single, large incision on the abdomen. Open adrenal surgery takes around 2-4 hours.

Following the operation, patients will remain in the hospital for 1-2 days for a laparoscopic adrenalectomy or 3-5 days for an open adrenalectomy. Patients will likely have a liquid diet immediately after their surgery, but should be able to return to solid foods after a day. If both of the glands are removed, patients will be given drug therapy to replace the hormones they produced.

Colonoscopy

Colonoscopy is the use of a special tool called a colonoscope (a thin, flexible tube with a camera on the end) to examine the inside of the colon and rectum. The tube is typically inserted anally, and it allows the physician to examine the large intestine from the inside. It may be done at any time to diagnose bowel problems, but routine colonoscopies are advised after the age of 50 to screen for colon cancer.

A colonoscopy may be performed to diagnose:

  • Intestinal ulcers
  • Polyps
  • Bowel cancer
  • An unidentified source of pain or bleeding in the intestine

Patients may be asked to drink only liquids in the days before a colonoscopy, or they may be given an enema to remove residual fecal matter. Before the exam, patients are given medication to help them relax, and then lay on their side on a table. The scope is inserted into the anus and gently moved all the way through the large intestine. Air may be pumped into the intestine to improve the view for the physician. Any polyps that are found will be removed. Then the colonoscope will be withdrawn. A colonoscopy is not usually painful, but patients may feel some bloating or have some cramps right afterward. If patients have polyps removed, they might experience a small amount of bleeding. Any side effects should go away within a few hours.

Pancreatectomy (Pancreas Removal)

Pancreatectomy is surgery to remove part or all of the pancreas to treat pancreatitis, tumors, or cancer. The pancreas is located behind the stomach and near the small intestine and spleen. There are three sections of the pancreas:

  • The head, which is the portion closest to the intestines
  • The body, which is the middle portion of the pancreas
  • The tail, which is the portion nearest the spleen

Pancreas removal procedures differ based on how much of the pancreas and surrounding organs are removed. There are three types of pancreatectomy:

  • Distal pancreatectomy removes the tail and body of the pancreas. This procedure treats tumors confined to these areas.
  • Pancreaticoduodenectomy, or the Whipple procedure, removes the pancreas, duodenum, bile duct, and, sometimes, part of the stomach. The Whipple procedure is used for pancreatic tumors that have not spread past the head of the pancreas and for other conditions affecting the pancreas, duodenum, and bile duct.
  • Total pancreatectomy removes the whole pancreas along with the duodenum, gallbladder, parts of the stomach and bile duct, surrounding lymph nodes, and, potentially, the spleen. Rarely performed, total pancreatectomy treats conditions that involve the entire pancreas, such as tumors found in multiple sites on the pancreas, or for rare tumors such as intraductal papillary mucinous neoplasms (IPMN).

Pancreatectomy can be done by open or minimally invasive surgery. During a laparoscopic pancreatectomy, the surgeon will use tiny incisions to insert a thin tube with a camera, or laparoscope, into the abdomen. Open surgery, or a laparotomy, is performed through a larger incision on the abdomen. If additional organs are removed, some procedures may require that the small intestine be reconnected to the stomach.

The hospital for pancreatectomy stay may be between 1-3 weeks. Generally, laparoscopic surgeries require shorter recovery periods. Whether patients undergo an open or laparoscopic procedure is determined by factors such as their condition and the extent of their procedure. For example, open surgery tends to be more common for distal pancreatectomies. If patients have difficulty eating, they may require a nasogastric tube, inserted through the nose, to carry food to their stomachs. A catheter can be inserted into the bladder if patients have trouble urinating. Full recovery may take about two months.
