Angiogram (Angiography)

Angiography is the use of x-rays to examine blood vessels, especially in order to identify any blocked or misshapen vessels that may cause problems. The test itself is called an angiogram. Some of the disorders that angiograms can be used to diagnose include:

Blockages and narrowing
Weakened walls and bulging, called an aneurysm
Malformations and misshapen blood vessels
Kidney artery conditions
Aorta artery conditions
Vein problems, including deep vein thrombosis (DVT) and blood clots or embolisms

During an angiogram, the doctor inserts a very thin, flexible tube called a catheter into a blood vessel through an opening in the groin or arm. The catheter is threaded through the body to the site of the suspected problem, where a small amount of dye is injected into the bloodstream and x-rays are taken. The dye makes the surrounding blood vessels visible via x-ray.

If necessary, certain procedures can be performed after an angiogram while the catheter is in place. Angioplasty is the use of a catheter to widen narrowed or blocked arteries. Stents are like tiny coiled springs that hold blood vessels open, and they can be placed during a catheterization. Damaged heart valves can also be repaired or replaced.

A typical angiogram takes about an hour to perform, but it may be longer if the catheterization is used to perform procedures on the blood vessels. It may be uncomfortable at times but is usually not painful. Afterwards, patients may be monitored for up to six hours to make sure any bleeding from the insertion site has stopped. They are given fluids to help flush the dye from their body. For a day or two after an angiogram it is important for patients to drink plenty of fluids and keep strenuous activity to a minimum.

Attention Deficit Disorder (ADD/ADHD)

Attention Deficit Hyperactivity Disorder, or ADHD, is one of the most common disorders diagnosed in children. Nine percent of children in the USA have been diagnosed with ADHD. Symptoms, which include hyperactivity and difficulty maintaining focus or paying attention, can last into adulthood for some patients. Boys are four times as likely to be diagnosed as girls, although experts don't know why this is.

There are three main types of ADHD:

Predominantly hyperactive-impulsive types show symptoms such as constant fidgeting, talking non-stop, touching everything, having trouble sitting still and waiting their turn, and interrupting others.
Predominantly inattentive types tend to be distracted, get bored and wander off during tasks, forget things or instructions, daydream, and appear not to be listening. This type is sometimes misdiagnosed because they are not hyperactive and there are fewer behavior issues.
Combined types show symptoms from both categories. This is actually the most common type of ADHD.

When diagnosing ADHD, it is important to rule out other issues that may be causing the symptoms. Seizure disorders, hearing loss, anxiety, and domestic problems are some examples of problems that can cause behaviors similar to those seen with ADHD.

Treatment can include stimulant medications, behavioral therapy to teach patients ways to navigate their world and control symptoms better, and accommodations at school or work. A structured environment (with lots of organization and well-defined rules) seems to help most kids with ADHD function at their best.

ADHD is mainly thought of as a disorder that affects children, but symptoms can last into adulthood for a third to half of those diagnosed. In adults, ADHD has similar symptoms as when it is seen in children: impulsive behavior, difficulty maintaining focus, being easily distracted, or a tendency to fidget. These symptoms can cause problems in a patient's careers and relationships. Additionally, adults with ADHD are at increased risk for substance abuse. Medications can be an important part of treatment for adults, as well as stress reduction techniques and organization skills training. Specialized ADHD coaches can help adults with ADHD manage their symptoms and succeed in all areas of their life.

Chemodenervation

Chemodenervation is a type of treatment that stops unwanted or overactive sensations and movements. A very small dose of strong chemicals is injected into the affected nerve or muscle, blocking movement and pain in a small and specific area. Chemodenervation may use substances such as phenol or alcohol, but the most well-known agent is botulinum toxin, commonly in the form of Botox. Originally approved to treat wrinkles, Botox is now used to treat a wide variety of disorders.

Chemodenervation can be used to treat many different disorders that cause pain or unwanted movement. Some of these include:

Spasticity, or tight, stiff muscle movement
Cerebral palsy, which can cause exaggerated or uncontrolled movement
Dystonia, or cramping and involuntary motions
Migraine
Overactive bladder
Some kinds of chronic pain, including sciatica
Certain voice disorders
Excess salivation

Depending on the area being treated, a chemodenervation session may take only a few minutes or as long as an hour. In some cases, an ultrasound will be used to guide the injection to exactly the right spot. Although results may be felt immediately, it may take 5 to 7 days for full effect. Results usually last several months.

He cares about my total well-being and giving me the ability to keep my active lifestyle as much as possible given my chronic illness. Dr Amjad, leaves no stone unturned. Thoroughly reviews my treatment plan and symptoms. He hones in on symptoms and expertly figures out what changes may have triggered them or whether it is the disease itself. His problem solving abilities and his ability to ask the right questions are very impressive. I love the way he thinks. He thinks very quickly too! I am most humbly grateful to have Dr Fahd Amjad in my life. He works with me to give me quality of life, he cares that my pain level and discomfort is under control.
