My experience was not good. The intake history form took me several hours to fill out because it was very thorough. During my first (and last) appointment she got my name wrong. She asked 
the intake questions all over again. It was obvious that she hadn't even read it. Her affect was dull and flat and lacked compassion. She made me feel very uncomfortable.
