This doctor diagnosed me with clinical depression and then would not sign my paper work for FMLA leave after both the assistant and staff told me prior to the appointment my paper work would be signed but she then refused.  Then she also told me my next appointment would not cost as much as my first appointment and that was a lie. Don't use this doctor.
