Headache

Headaches are dull or sharp pains that occur in regions of the head and face. Headaches occur in many different forms and vary in location, severity, and duration. They are not necessarily a sign of an underlying illness and often resolve on their own. However, headaches can present significant day-to-day discomfort. The most common forms of headaches include:

  • Migraines: one of the most prevalent types of headache, migraines typically last from two hours to three days, causing a throbbing pain on one side of the head. Nausea, sensitivity to bright lights, fatigue, and blurred vision all may accompany migraines. While migraines have no known cure, medications and lifestyle modifications may ease symptoms. Avoiding behaviors that may trigger migraines such as lack of sleep or loud, repetitive noises can improve headaches.
  • Tension headaches: also known as stress headaches, these headaches are characterized by pain in the upper top part of the head and behind the eyes. Tension headaches are common and have no exact cause; however, certain practices may increase the likelihood of developing them. For example, wearing one's hair up in a tight hairstyle or wearing a restrictive hat can put pressure on the scalp, leading to a tension headache. Other causes of tension headaches may include alcohol or nicotine use, caffeine, teeth grinding, stress, overexertion, and eye strain. Some people find posture improvement, alternating sleeping positions, and neck exercise can alleviate tension headaches. Over-the-counter pain relievers such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve) may also reduce tension-headache pain.
  • Sinus headaches: this type of headache causes pain in the sinuses, but may progress to pain and pressure in the eyes, forehead, and cheeks. These headaches are often accompanied by nasal congestion, fatigue, and ache in the upper teeth and may or may not be caused by sinus infections or sinusitis (sinus inflammation). Some sinus headaches are allergic headaches, which arise from allergic triggers such as seasonal allergies (pollen allergies) or hay fever, causing sinus pressure and facial pain. Other symptoms of allergic headaches include nasal congestion, irritated or watery eyes, runny nose, and sneezing. Allergic headaches may be treated with antihistamine medications such as cetirizine (Zyrtec). Allergic headaches are common during pollen season, which is usually around springtime.
  • Cluster headaches: these headaches occur suddenly and cause stabbing, sharp pain from one eye down to the one side of the neck. They are often referred to as the most aggravating and painful type of headaches. Other symptoms of cluster headaches include nasal congestion, droopy eyelid, and enlarged pupil on one side of the face. In the past, it was widely believed that cluster headaches were caused by a bevy or "cluster" of multiple types of headaches, such as a combination of an allergic headache and a tension headache. Medical experts now recognize that cluster headaches are a separate and unique form of headache. Cluster headaches last for around twenty minutes to two hours. They may recur throughout the period of a day or several weeks, but usually dissipate within a month. Treatment for cluster headaches includes over-the-counter pain medications such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve). Prescription Prednisone can help with swelling and Verapamil may reduce pain by relaxing the blood vessels.

Living with headaches is challenging; headaches can prevent people from fully enjoying life. Medical treatment and care in avoiding certain triggers can help those with headaches begin to feel normal once again.

Mononucleosis (Mono)

Mononucleosis, frequently called ''mono,'' is a viral infection that causes fatigue, fever, swollen lymph glands, and sore throat. The Epstein-Barr virus (EBV), spread through saliva, is the primary virus that leads to mono. Mono is most common in teenagers and young adults, where EBV is often transmitted through kissing or sharing drinks. For this reason, mono is colloquially referred to as ''the kissing disease.'' Mononucleosis usually resolves on its own; however, recovery from mono can take from several days to several weeks. Some young adults may continue to experience extreme fatigue from mono for months, often requiring a semester home from school.

Mononucleosis happens in three stages:

During the initial stage, those with mono may experience few symptoms as EBV replicates in the body. Symptoms such as headaches and body aches are often misattributed to allergies or stress. People with mono may continue performing their daily activities and attending school, leading to increased transmission of the virus to others. The initial stage lasts for about five to fourteen days.

During the acute stage, mono symptoms intensify and may include the enlargement of the spleen, a sore throat, fatigue, liver issues, and skin rashes. The mono infection is at its worst in this stage.

Lastly, the convalescent stage is when recovery begins. This period of healing can last from two to six months.

Mono can be detected by a physical exam; physicians can diagnose mono by checking for inflamed lymph nodes, tonsils, or spleens. Blood antibody tests may also help diagnose mono by revealing antibodies in the blood that were formed as a response to the Epstein-Barr virus. Elevated levels of white blood cells can also suggest that a patient has mono.

There are no medications specifically for mono or the Epstein-Barr virus, and patients are advised to rest and avoid strenuous exercise. Drinking plenty of fluids, eating a healthy diet, and taking anti-inflammatory ibuprofen (motrin) may also help people who are afflicted with mono feel better.

People with mono should take precautions to avoid spreading the condition to others. They can prevent infection by not sharing glasses, food, and utensils with others until they fully recover.

Thyroid Problems

Thyroid problems are a group of conditions that alter hormone regulation and metabolism. The thyroid is a small, butterfly-shaped gland located in the lower neck, just below the Adam's apple (laryngeal prominence) and larynx (voice box). Despite its small size, the thyroid has an outsized effect on the body by regulating hormones that control metabolism. Metabolism is the body's process of converting consumed food into fuel to power the entire body. Heart rate, weight, cholesterol, body temperature, and even physical growth and development are all affected by the metabolism.

When the thyroid malfunctions, the body is unable to properly process and allocate energy. Thyroid problems, such as hyperthyroidism (overactive thyroid) and hypothyroidism (underactive thyroid), cause irregular metabolism. In some cases, the body produces antibodies that attack the thyroid cells for unknown reasons. The damaged thyroid responds by releasing either too little or too much T3 and T4 thyroid hormones. Thyroid malfunctions may also arise from genetic causes or iodine deficiency. Thyroid problems may include:

  • Hashimoto's thyroiditis, an autoimmune disease that causes hypothyroidism
  • Graves' disease, an autoimmune disease that causes hyperthyroidism
  • Goiter
  • Thyroid nodule
  • Thyroid cancer

Hashimoto's is the leading cause of hypothyroidism, which is when the thyroid does not produce enough hormones to stimulate adequate metabolism. The condition is diagnosed by blood tests that measure TSH (thyroid stimulating hormone) and detect Hashimoto's antibodies. Symptoms of hypothyroidism include sluggish metabolism, inflammation, fatigue, weight gain, brittle hair, dry skin, and sensitivity to cold. To supplement impaired thyroid function, patients with Hashimoto's are typically prescribed synthetic thyroid hormone pills. Such patients may need to consume synthetic thyroids for the rest of their lives. Other treatments may include dietary changes, such as the avoidance of certain foods and medication to reduce inflammation. Frequent blood tests can inform patients if their TSH levels are within a normal range.

Grave's disease is most frequently the cause of hyperthyroidism, which is when the thyroid produces too many hormones, overstimulating one's metabolism. The condition is also diagnosed by blood tests that measure TSH and detect Grave's antibodies. Elevated heart rate, high blood pressure, sweating, bulging eyes, and weight loss are common symptoms of hyperthyroidism. Treatments for hyperthyroidism include oral radioactive iodine, which slowly shrinks the thyroid, slowing the production of excess thyroid hormone. Other anti-thyroid medications such as methimazole (Tapazole) also reduce the production of thyroid hormones. Blood pressure medications known as beta blockers can ease the increased heart rate caused by hyperthyroidism. If treatments are unsuccessful, patients may be prescribed a thyroidectomy, or surgery to remove part or all of the thyroid. Some patients with hyperthyroidism may require ophthalmological (eye medicine) treatments such as eye drops, prednisone, and surgery to reduce eye swelling and bulging.

Untreated thyroid disorders frequently lead to goiters or thyroid nodules. A thyroid goiter is a large lump that can be felt near the base of the neck and is made of swollen thyroid tissue or nodules (excess cell growth). Nodules and goiters are diagnosed by CT scan or ultrasound. Obstructive goiters can cause pain, coughing, and abnormal breathing. The most common cause of goiters is hyperthyroidism (overactive thyroid). However, Hashimoto's is also known to cause goiters or nodules. Goiters develop from continued damage from antibodies to the thyroid over time. At times, a goiter can result from a cancerous thyroid tumor. Most goiters and nodules are themselves benign.

The exact causes of thyroid cancer are unknown. People who develop thyroid cancer may or may not have another thyroid problem such as Hashimoto's or Grave's disease. Symptoms of thyroid cancer include a nodule that can be felt on the neck, hoarseness, difficulty swallowing, swelling in the neck, and neck and throat pain. CT and MRI scans are used to diagnose thyroid cancer. There are four types of thyroid cancer which vary in intensity. Thyroidectomy (thyroid removal surgery) is usually prescribed to treat thyroid cancer.

Thyroid problems are often long-term conditions that require some form of treatment. With regular monitoring and medication, many people with thyroid problems are able to lead active and fulfilling lives.

Dr. Frugone is a very caring, sincere, kind doctor. He never rushes me, listens well to my concerns, and is attentive throughout. While being earnest about providing good medical care for me, he is also a very pleasant person and he smiles easily. I trust him. His medical assistants make the visit to this office even better. A great team! Dr. Frugone and his staff don't miss a beat - very proficient at what they do and totally pro-active in addressing what I need to be doing. Dr. Frugone leads the way by setting a good tone in his office. Glad I found them! They do a great job, and they set a good example of how a medical office should be run. 10/10.

This was an unscheduled virtual visit. I called my provider's office at 0830 and received a virtual appointment for 1420 the same day. Using ZOOM is simple and made easier by the office assistant. I met with the doctor as scheduled, explained my complaint and discussed the treatment options as recommended by Dr. Larrea. He prescribed two medications, a Zpack and a cough suppressant. It was a very positive experience. Dr. Larrea is very professional and personable. He exhibited real concern for my well-being for which I was grateful. I would recommend him highly to anyone in need of medical care.

Outstanding physician and medical assistants who are a 10+ on kindness and their ability to listen to me, their patient. They never make me feel rushed or that they need to get on to the next thing. Dr. Frugone is super nice and patient, and I can tell that he wants to provide me with the best care he can. The medical assistants are delightful and they just get the job done! No complaints! Very good office!

I was very impressed with my first meeting with Dr. Frugone. He listened to me and asked excellent questions . I am having knee surgery in a few weeks. My regular doctor canceled my appointment to clear me for surgery. Dr. Frugone saw me and did a thorough exam clearing me for surgery. He was also easy to talk to. I felt he cared about me as a person! Thank you Doctor Frugone!

My husband is the patient. Due to his complications from encephalitis R/O stroke I have been communicating with all of his providers. I am completing this survey for him. This office has been very supportive and follow up regarding his ongoing cognitive issues is still being evaluated. Our family is grateful for the support and due diligence in his ongoing care.

It has been very hard for me getting used to a new primary after losing the best one I've ever had after 6 years. I have only seen Dr Frugone 2 times now. But I feel, especially after "losing it" in his office last week, I am becoming more comfortable with him and look forward to seeing him in the future. I just hope he stays around for a while!

I am new to the Senior Clinic. I like the doctor and staff. They seem to care about senior citizens. My last doctor had lost interest in me. I have an appointment scheduled in Nov. And feel that if I need to, I can call and get friendly service. Thank you for this clinic and Dr. Fergone and staff.

The clinic is very well run. The staff is very attentive to my requests and when they say they will call me back they mean It. The nurses are te best ever and doctor Frogone is a very kind person. I am very blessed to have this clinic to take care is me at this point in my life.
