Depression

Everyone knows what it feels like to get the blues once in a while. But depression is a serious illness that is more severe than a bad day and lasts much longer. Symptoms of depression stop a person from being able to function and enjoy daily activities for weeks or months at a time. It can happen to anyone, and it isn't something that people can control by force of will or "snap out of it."

Some common symptoms of depression include:

Feeling sad, guilty, empty or hopeless
Fatigue and a lack of energy and motivation
A loss of pleasure in activities that were previously enjoyed
Unusual sleep or eating habits
Mental fog -- trouble concentrating, remembering, or making decisions
Physical aches and pains
Suicidal thoughts or a preoccupation with death

We don't yet know what causes depression, but it's thought that it is a combination of genetic, biological, environmental, and social influences. Because of this, the most effective treatments for depression combine medication with psychotherapy. Therapy, especially cognitive-behavioral therapy, can be extremely helpful in resolving the negative thoughts and feelings that come with depression. It gives patients new tools that they can use themselves to cope when their depression is making them feel down.

Some of the common medications used to treat depression include antidepressants such as SSRI's (Prozac, Paxil, Zoloft) or atypical antidepressants (Cymbalta, Wellbutrin). It's important to remember that these medications have different effects on everybody, and no one medication works right for everyone. Patients may have to try a couple before finding the one that works just right for them. If the first medication they try doesn't work, they should talk to their doctor about trying something else. In extreme cases where medication is not enough, electro-convulsive therapy and hospitalization may be the answer to keeping a severely depressed person safe.

Depression is a difficult illness to deal with, but it is more common than believed and there are many people who can help. With the right treatment, individuals with depression can get back to fully enjoying life again.

Eating Disorders

Eating disorders are mental and physical illnesses that impact behavior, emotions, and thoughts about eating, food, body appearance, and weight. These disorders are characterized by preoccupations with food and physical size, resulting in dramatic lifestyle changes and adverse health effects. In the past, eating disorders were often diagnosed using BMI (body mass index), or by assessing someone's weight. Eating disorders specialists now recognize that eating disorders may occur in people of all sizes. The most common eating disorders are anorexia nervosa, bulimia nervosa, and binge-eating disorder (BED).

Anorexia nervosa, frequently referred to as anorexia, is a disorder where people severely restrict the types and quantity of food they eat. Those with anorexia have an extreme fear of eating and gaining weight and may repeatedly weigh themselves or engage in harmful behaviors such as excessive exercise or starvation. Those with anorexia often find themselves preoccupied with food, but unable to consume a healthy amount of it. This results in symptoms such as abnormally low BMI, vitamin deficiency, fatigue, fainting, and amenorrhea (lack of menstruation in women). Anorexia may cause bluish discoloration of the hands, brittle and falling out hair, yellowish skin, and lanugo (fine, downy hair). Left untreated, anorexia can lead to organ failure, brain damage, and even death.

Bulimia nervosa, typically called bulimia, is a disorder where people binge (eat large quantities of food) and purge the food that they just consumed by vomiting, laxative abuse, fasting, or excessive exercise. This behavior is usually motivated by a feeling of powerlessness over eating and a fear of weight gain or desire for weight loss. Bulimia can occur in underweight, normal weight, and overweight people. Symptoms of bulimia include acid reflux, intestinal distress, dehydration from purging, amenorrhea, and dental erosion from vomiting. Some people with bulimia have swollen parotid glands (salivary glands near the cheeks) and red scars and marks on the backs of their knuckles. Electrolyte imbalance from bulimia can be particularly severe and may lead to heart attack or stroke.

Binge eating disorder (BED) is the most common eating disorder. People with BED feel that they are unable to control the amount of food they eat and may continue to binge after reaching a point of fullness. BED binges (consuming large amounts of food) are often accompanied by feelings of sadness, guilt, and shame. Symptoms of BED include obesity, acid reflux, weight gain, and dissatisfaction with one's body. People with BED tend to experience cycles of negative emotions and binges. Binges or BED episodes can be provoked by stress, boredom, restrictive dieting, depression, and other triggers.

Other common eating disorders include:

Pica, eating things that are not food, such as paper, ice, or soil
Avoidant/Restrictive Food Intake Disorder (ARFID), disinterest in eating and avoidance of food
Other Specified Feeding or Eating Disorder (OSFED), abnormal eating that does not fall under any disorder

Many people with eating disorders do not consciously develop them, and have difficulty seeking treatment. For this reason, it's important to detect and treat eating disorders as early as possible. Treatment for eating disorders can vary from therapy visits to inpatient treatment, depending on the disorder and its severity. Treatment aims to not only restore physical health and correct negative behaviors, but to also promote psychological well-being and self-confidence.

Sexual Health Issues

Sexual health is a broad and loosely defined term that encompasses several extremely different areas of medicine. Sexual health not only includes physical health related to the act of sex, but also emotional, mental, or identity issues that can interfere with healthy sexuality. Because this scope is so broad, a huge number of health care professionals can accurately say they work in the sexual health specialty. Some of the many varied conditions included in sexual health include intimacy disorders, sexually transmitted diseases, reproductive health, LGBTQ issues, and sexual violence.

Sexual intimacy disorders are those that cause problems with the act of sexual intercourse, such as erectile dysfunction or vulvodynia. Erectile dysfunction, the inability to get or maintain an erection hard enough for sexual intercourse, is extremely common. It affects fully half of men over 40 to some degree. It is treated with medications, devices, or surgery. Vulvodynia, burning pain in the vulva that can be made worse with sexal activity, has no known cause. It is treated with oral or topical medications.

Sexually transmitted diseases, including HIV/AIDS, herpes, gonorrhea, and chlamydia, are bacterial or viral infections that are contagious in the bodily fluids exchanged during sexual contact. Barrier methods of contraception, such as condoms, help prevent the spread of STDs. HIV is a virus that can be caught from an infected person's sexual fluids or blood. When the virus infects the body, it interferes with the immune system and causes the disease known as AIDS. There is no cure, but anti-viral medications can help people with AIDS live a long and healthy life. Herpes is also a virus found in sexual fluids, called HSV-2. It causes blisters that come and go on the genitals. There is no cure for herpes, but anti-virals can minimize outbreaks. Gonorrhea and chlamydia are both bacterial infections. They are extremely common and often have no symptoms. The most common signs of an infection are burning pain and discharge. Gonorrhea and chlamydia can be treated with medications.

Reproductive health refers to both contraception and infertility treatment. Contraceptives prevent and plan the timing of pregnancy. Some, such as a vasectomy or tubal ligation, are extremely effective at preventing pregnancy. Others, such as spermicides, are less reliable. Popular contraceptives include condoms, birth control pills, IUDs, and diaphragms. On the other side of reproductive health, infertility is the inability to carry a pregnancy to term after one year of trying. It can be due to problems in either the man or the woman, and both genders are affected equally. In total, about 15% of the population suffer from infertility. Infertility treatments include medications and surgery.

Gay and transgender people have a few specific health care needs that may sometimes fall under the sexual health umbrella. First, many LGBT people still face discrimination from healthcare providers, so providing supportive and appropriate care is an issue. Because of social attitudes surrounding sexual orientation and gender identity, many gay and transgender people struggle with their identities. Rates of depression, substance abuse, and suicide are all higher in this group. Providing mental health support and counseling to those in transition is critical. Finally, transgender patients who are undergoing gender affirmation require sensitive medical health care, including surgical and hormonal treatment.

Sexual violence can be defined as any unwanted or non-consensual sexual activity. It ranges from sexual harassment to touching to rape. It affects women more than men and is widespread: 1 in 4 women report being victims of sexual violence at the hands of a partner, and as many as 1 in 3 girls report their first sexual contact as being violent. Sexual violence has lasting physical and emotional consequences for victims, but educating both girls and boys can prevent violence. Care for those who have endured sexual violence includes counseling and mental health support, medical care, and legal support.

Sexuality is more than a reproductive requirement -- it is a form of expression and identity. Sexual health blends all these aspects of experience together and helps people lead sexually fulfilling lives.

Dr. Shuhart was attentive to all of my concerns relating to my rare disease (HPP) symptoms and treatment. I have been ill for over fifteen years, and misdiagnosed several times. Prior to using mobility aids, It was often my experience that providers didn't take my symptoms seriously because of my young age and the fact that I looked normal. I wasn't sure what to expect with Dr. Shuhart, all I knew about him going into my appointment was that he is only one of three providers in WA state that treat hypophosphatasia. I waited over a year for this appointment...which I would gladly do again, it was worth the wait! He knew the ins and outs of hypophosphatasia concerning the symptoms throughout the lifespan and treatment. He was a great listener, never cutting me off, and made me feel heard. I brought my family member also with HPP Into the appointment to help me remember and write down Information. My extreme brain fog makes appointments tricky sometimes, but he was very patient. He was fully willing to answer all of my family member's questions as well. If he didn't know the answer to a question, he would say that the studies were inconclusive or that there was a lack of research on that particular topic. The candid replies made me feel at ease knowing that he wasn't just guessing with his answers. He ordered a grip test device specifically for me, which made me feel like he was going above and beyond. The nurse doing my Intake was very compassionate and personable. At many appointments I feel like all the staff are rushing through appointments and not taking the time to meet my concerns and needs so thoroughly. This was not the case with the Swedish bone clinic. The office staff after my appointment have been great at staying in touch via my patient portal to help get my treatment approved by insurance. All the team members have made a stressful time in my life much more pleasant and bearable! Please thank them for me.

I have nothing but appreciation and praise for Dr. Shuhart, who has been treating me for osteopenia since 2017. I am a nurse and proactive in my care. I love our time together. He is the best doctor I've ever had when it comes to collaborating with me and explaining things to me at the level I like. I feel like he treats me like a colleague. I always learn something very pertinent to my concern, curiosity and care. Every decision he makes is based on research and takes my health priorities into account. He brings a personal touch like walking me down to the clinic lab to get me a specimen cup for a future lab test. I only see him once a year, but he seems to remember everything about me. I was losing none density at a rapids pace and having to drive my own screening and even Dexa results until I found him. Since then, my dexa scan has improved and kept stable for 7 years with our treatment plan. Thank you Dr. Shuhart!

Dr Shuhart was unusually kind and respectful. Unfortunately, this is extremely rare in people in his profession. The amount of time he spent with me was much longer than I expected, and he took time to listen to me. He was even willing to admit that the abuse I have encountered from other providers was wrong, and should not have happened. I saw him for a rare condition I have had for many years, and have not been adequately treated for, simply because the research on my condition has not been done. I appreciated Dr. Shuhart's willingness to admit to his (and other providers') lack of knowledge, rather than attempting to gaslight me into believing that he knows more than I do about my own experiences. That said, I don't know anyone else with my condition, so it's unlikely I will recommend him to anyone else. If he was a general family practice physician, I might.

He's "a keeper" - how I refer to those who live up to my expectations in all categories and have earned my continued trust. Osteoporosis came out of the blue for me / not genetically obvious. It's upsetting when your view of your heath is flipped. His natural warmth soothes me.; I always leave feeling that he cared about me, a unique individualnever just a cattle call appointment. My too-sensitive gag reflex to large pills eventually stopped my taking his prescribed vitamins and was clearly obvious in my blood work. The VISUAL proof, his focus on finding the causeand a solution that fit me = haven't missed a pill since. This year's my blood work Is Impressive! Why? He LISTENED and switched me to chewable vitamins! So, I am grateful to have him as my doctor to walk this path with me.

I have been seeing Dr Shuhart for several years, and feel completely confident in his amazing knowledge of Osteoporosis using the best treatments that will assist in keeping my bones healthy. He is very detailed in explaining the best personal treatment for my Osteoporosis. He listens to my health & treatment concerns taking time to explain in detail so that I may understand, and he takes time in answering my questions. I appreciate his passion In providing care for his patients, and his extreme knowledge in the current & future health of his patients with Osteoporosis. New patient visit. Extremely impressed with Dr Shuhart's knowledge and ability to easily present his findings in layman's terms. He could relate to my MCAS, putting me at ease that he understood my health concerns were more than just his specialty. With the labs he ordered I know I am in the best hands possible to determine whether there is a need to be concerned about other health conditions. I needed to wait 9 months to see him as a new patient. If I had a choice to see another doctor sooner or wait for Dr Shuhart I still would have waited. His expertise in his field is exceptional. You are fortunate to have him on your staff.

Dr Shuhart spent the time to explain the particulars. He looked at all of my test results ordered by other doctors to try to get a handle on my condition. He answered all of my questions clearly and respectfully. I got the feeling that he actually cared about finding the best way to treat my condition and provided options for care. Many doctors have not reviewed my history before I come in and talk to me as if I am a two year old. I think this was the most informative and care centered appointment I have ever had with a doctor. The only negative is the waiting time for an appointment was at least four months.

Dr. Shuhart is an exceptional doctor and has always been very thorough in understanding my conditions and answering my questions; making time to review my CT scans and recommend the best treatments. His retirement and the loss of his practice for specialized care for osteoporosis and bone health will be huge for me and all the rest of his patients. He has had a great enthusiasm for his work and his patients care. He will be greatly missed and I (along with the rest of his patients, I'm sure) will greatly miss him and the loss of thus care at Swedish! I wish him much happiness in his retirement.

Dr Shuhart is a very knowledgeable & caring physician. As a widow & cancer survivor, I've faced some serious life challenges on my own.It has been extremely comforting & reassuring to have a medical professional dedicated to overseeing my treatment for osteoporosis - knowing that Dr Shuhart has been dedicated to this field of patient need. Though I am happy for him to be taking a well-earned retirement, I will deeply miss the care that he and his team have provided me - and so many others - over these past 10 years.

Dr. Shuhart was awesome. He had clearly read my medical file before seeing me and knew my concerns. He took the time to explain my condition, necessary tests, and possible treatment options and answered all my questions. He was also friendly, funny, and established a good rapport. Only quibble is it took about 6 months to get an appointment with him, but that's not his fault. Just a bummer as he said my bones likely deteriorated more during that time and it would have been better to have initiated treatment earlier.
