I would like that Dr. Smith would be listed as part of my "health care team" on My Chart. This survey is questions do not allow me to provide essential information about this care giver. Dr. Smith was very knowledgeable and caring. Mom definitely felt heard and was thankful for that. The fact that the upcoming MRI & MRA is 3 months away is very discouraging considering the amount of falls that are occurring .

It was my first visit to a neurologist.Dr.Smith answered all my questions and is doing follow up to assure all is OK.It took time understanding why I was there and was very thorough in his examination.
