Deep Brain Stimulation (DBS)

Deep brain stimulation (DBS) is a procedure performed to help treat neurological conditions such as Parkinson's disease and epilepsy. Electrode leads are surgically implanted in the brain and connected to a device, called a neurostimulator (deep brain stimulator), that is placed underneath the skin (subcutaneously). The deep brain stimulator can send electrical signals through the electrodes to the brain to restore normal rhythms, or it can block electrical signals in specific areas of the brain to restore functioning. Although not a cure, DBS can treat symptoms of movement disorders such as pain and tremor when medication fails to work.

DBS surgery may be completed in one operating session, but it is more commonly performed in two parts. In the first stage of the procedure, the surgeon will map targets in the brain using CT or MRI imaging to determine where to place the electrodes. Then, by drilling holes into the skull, the surgeon will place the electrodes into specific areas in the brain. The holes will be closed, and patients will require 1-2 days to recover in the hospital. After about two weeks, patients will undergo another surgery to have the deep brain stimulator implanted. The stimulator is usually placed under the skin around the collarbone, chest, or abdomen, and it will be connected to the DBS electrodes through a subcutaneous wire. No component of the DBS system will be visible from outside. Following stimulator implantation, patients may need to recover in the hospital for 1-2 days.

Within 2-4 weeks of the operation, doctors will program the stimulator using a wireless device. Patients will work with their doctor to determine the most effective settings for the stimulator during follow-up visits. Patients will be given a handheld device so that they can turn the stimulator on or off and adjust its settings themselves. The stimulator may have a rechargeable battery, in which case patients will be given a charging unit. Stimulator batteries generally need to be replaced in 3-5 years.

Joint Injections

A joint is any area of the body where two bones connect. Due to injury or disease, the space between the two bones can sometimes become swollen and inflamed, which leads to pain and a loss of mobility. Injection and aspiration are two tools that physicians use to treat joint pain locally without needing to perform surgery.

Both injection and aspiration are techniques that involve inserting a needle connected to a syringe directly into the joint. Aspiration involves the removal of excess fluid, and injection is the placement of medication directly into the joint space via the needle. Both procedures may be performed at the same time. In both cases a local anesthetic may be used, the skin will be cleaned and disinfected, and then the needle will be inserted. In some cases, especially if the injection or aspiration is in a large and deep joint such as the hip or spine, ultrasound may be used to guide the needle to the exact location desired.

In some cases, irritation to the joint can cause fluid to build up so significantly that the joint hurts and can no longer move well. Aspiration removes some of the excess fluid and relieves the pressure. In addition, aspiration can be used to provide a sample of joint fluid if it needs to be examined microscopically for the presence of white blood cells, bacteria, or crystal formations.

Certain injuries and diseases that affect joints are inflammatory in nature, such as rheumatoid arthritis, tendonitis or gout. These diseases may be helped by the local injection of anti-inflammatory medications directly into the affected joint. In this case, corticosteroids such as methylprednisone are usually used. Relief from the pain may be felt right away and may last for weeks or even months.

In some cases, such as with osteoarthritis, the cartilage buffer between the two bones wears down and pain comes from bones rubbing against each other. In this case, injection of a lubricating agent such as hyaluronic acid may be beneficial. It provides a slippery cushion between the bones to relieve pain that can last for months.

Through several years of being a patient of Dr. Mayadev, I have consistently experienced exceptional care. Dr. Mayadev is always alert to the details of my medical and personal history, while also being very observant in-clinic. She has offered practical and practicable suggestions to benefit my strength, balance, and stamina for mobility. Dr. Mayadev is very responsive to my questions, graciously sharing her wealth of experience and expertise. I appreciate her collaboration with other practitioners at the MS Center to provide a truly holistic approach to care. On top of that, Dr. Mayadev is unfailingly pleasant and courteous.

Dr. Mayadev is an outstanding doctor! She is thorough, brilliant, and very compassionate! I get Botox injections every 3 months to help with my leg pain and spasticity. The length of her appointments to do this has been cut down and it puts a lot of pressure on her to hurry. That means that often I don't have time to ask her questions about my Multiple Sclerosis care. It would be wonderful if more doctors could be added to the practice so that the wait time for an appointment isn't so long. For example, this time around I had to wait 4 months to get the injections, instead of 3 because of lack of her availability.

Each time we meet, she remembers things we have discussed even when they are not in her notes. She is excellent about generating a variety of solutions to thorny problems. Dr Mayadev is a thoughtful, thorough, and wonderfully kind human being - all those qualities make her an excellent doctor. Answers all my questions thoughtfully and entirely. She is also good at adjusting my Botox doses on the fly, depending on what muscles are most spazzy, which helps immensely.

Dr Mayadev is thorough, detail oriented - but not so much as tolose sight of the bigger picture- and very caring. I see her every three months-ish. There's o to one of her to go around, so sometimes getting myPreferred appointment date is hard. But it's important that they're every three months, and they manage to get me an appointment around the every three months' timeframe.

Instead of just talking about some new symptoms that I have had, as other doctors have done recently, Dr. Mayadev Did a thorough exam manipulating my muscles in ways that helped come up with a diagnosis of the issue.
