Hepatectomy (Liver Resection)

Hepatectomy, or liver resection, is a surgical procedure that removes all or part of the liver to treat cancer or other diseases. The liver is divided into one left and one right lobe, and each lobe is divided into segments. The amount of liver removed is dependent on factors such as the size, location, and number of tumors or other lesions. The following are types of liver resection:

Wedge resection, which removes a small piece of the liver for small lesions
Segmentectomy, which removes a single segment of the liver, allowing the surgeon to treat tumors in multiple places without removing too much of the liver
Trisegmentectomy, which removes around 70% of the liver
Lobectomy, which removes the entire left or right lobe

Liver resection can be performed laparoscopically or through open surgery. Laparoscopic liver resection is a minimally invasive surgery that requires a few tiny incisions on the abdomen, through which the surgeon inserts a thin tube with a camera (laparoscope). Open hepatectomy is performed through a laparotomy, which makes one large incision on the abdomen. Although a laparoscopic liver resection is associated with less pain and quicker recovery, patients may have to undergo an open liver resection depending on factors such as the location of the tumor and previous abdominal operations they have had.

After the liver resection, patients will stay in the hospital for up to six days. The recovery period will vary based on how much of the liver was removed. Limiting consumption of alcohol and light physical activity, such as walking, can help patients to have a speedy recovery.

Musculoskeletal Problems

The musculoskeletal system refers collectively to the bones, joints, ligaments, muscles, and nerves. It is what gives the body structure and movement. Because this system encompasses so much of the body, musculoskeletal problems are extremely varied and can happen almost anywhere. There are musculoskeletal problems that affect only the joints, those that affect the bones, those that affect tendons and ligaments, and those that can happen in any area of the body but cause pain and numbness.

Joint problems include bursitis and arthritis. Bursitis is the inflammation of a fluid-filled sac cushion on the outside of a joint, causing symptoms including pain and swelling. Arthritis is the inflammation and damage of a joint due to wear and tear or disease. There are several treatments for arthritis, but if the damage progresses far enough, joint replacement may be necessary. In joint replacement, a metal or plastic implant is surgically placed within the joint to make movement easier.

Bone problems include scoliosis, fractures, and osteoporosis. Scoliosis is a curvature of the spine, making the normally straight spine look like a 'C' or 'S.' It is treated with braces or surgery. Osteoporosis is a disease that causes bones to weaken and break easily. It's most common in women, especially older women. Fully half of all women over 65 have osteoporosis. Fractures are any break in the bone. They can be a simple crack or a severe and complicated shatter. Osteoporosis causes fractures in people who have it, but other causes are trauma (such as a fall) or overuse.

Tendon and ligament problems most often result from injury or overuse. Two good examples are ligament tears and tendonitis. The most common ligament tear is the ACL tear in the knee. This ligament supports and stabilizes the knee and is most often torn during sports activities. Tendonitis happens when a tendon, which connects muscles and bones together, becomes irritated and inflamed. This happens most often in older patients who push their bodies too far, leading to pain and swelling. Treatment for both tendon and ligament problems usually includes rest, ice, and supporting the area to let it heal.

Pain and numbness problems can be caused by overuse, disease, an injury, or a compressed nerve. Lower back pain and carpal tunnel syndrome are two of the most common conditions patients encounter. Lower back pain may be caused by sore muscles that have been overworked, or an injury to the disks separating the vertebrae in the spine. Lower back pain usually goes away on its own within a few days, but it may require medical treatment depending on the cause. Carpal tunnel syndrome occurs when the nerve at the base of the palm becomes trapped or pinched. Symptoms include tingling, pain, numbness, or weakness in the hand. Treatment involves rest and sometimes steroid injections.

Because the musculoskeletal system involves so much of the body, problems here can be incredibly varied in their type and severity. Some musculoskeletal problems will go away on their own or only require rest, while others may require medication, physical therapy, or even surgery.

Pancreatectomy (Pancreas Removal)

Pancreatectomy is surgery to remove part or all of the pancreas to treat pancreatitis, tumors, or cancer. The pancreas is located behind the stomach and near the small intestine and spleen. There are three sections of the pancreas:

The head, which is the portion closest to the intestines
The body, which is the middle portion of the pancreas
The tail, which is the portion nearest the spleen

Pancreas removal procedures differ based on how much of the pancreas and surrounding organs are removed. There are three types of pancreatectomy:

Distal pancreatectomy removes the tail and body of the pancreas. This procedure treats tumors confined to these areas.
Pancreaticoduodenectomy, or the Whipple procedure, removes the pancreas, duodenum, bile duct, and, sometimes, part of the stomach. The Whipple procedure is used for pancreatic tumors that have not spread past the head of the pancreas and for other conditions affecting the pancreas, duodenum, and bile duct.
Total pancreatectomy removes the whole pancreas along with the duodenum, gallbladder, parts of the stomach and bile duct, surrounding lymph nodes, and, potentially, the spleen. Rarely performed, total pancreatectomy treats conditions that involve the entire pancreas, such as tumors found in multiple sites on the pancreas, or for rare tumors such as intraductal papillary mucinous neoplasms (IPMN).

Pancreatectomy can be done by open or minimally invasive surgery. During a laparoscopic pancreatectomy, the surgeon will use tiny incisions to insert a thin tube with a camera, or laparoscope, into the abdomen. Open surgery, or a laparotomy, is performed through a larger incision on the abdomen. If additional organs are removed, some procedures may require that the small intestine be reconnected to the stomach.

The hospital for pancreatectomy stay may be between 1-3 weeks. Generally, laparoscopic surgeries require shorter recovery periods. Whether patients undergo an open or laparoscopic procedure is determined by factors such as their condition and the extent of their procedure. For example, open surgery tends to be more common for distal pancreatectomies. If patients have difficulty eating, they may require a nasogastric tube, inserted through the nose, to carry food to their stomachs. A catheter can be inserted into the bladder if patients have trouble urinating. Full recovery may take about two months.

Im grateful for the compassion and care from care provider. Very thorough and direct about care needed. Did not dismiss anything I had to say and asked me questions as he took my answers seriously. I had been struggling with my health concern for a while and when I was done with my appointment I felt relieved and like a huge weight had been lifted off my shoulders. Dr. Howlett is great! He made me understand the importance of waiting until January or maybe even February to get my knee replaced. I didn't think about the importance of getting my dental work finished and reduce the chance of infection. Dr. Howlett replaced both of my husbands knees, one in 12/2015 and the other in 03/2016. We appreciated his work, PA's and staff and felt he is the best. I'm very privileged to have him do my surgery.

Upon receiving my cancer diagnosis my world stopped for a moment. Then learning I'd have to travel a good distance to receive the care I need added more stress. Now having met all the staff and especially my direct care team that stress level has subsided a bit. I feel like I am in great hands with very caring, compassionate, knowledgeable and straightforward people who strive to do their best for those in their care.

Dr Howlett was dismissive, rude and seemed annoyed. I don't know if this is normal for him or he was just having a bad day but I'm getting another consult because I sure don't want someone like this operating on me. I also waited one hour to see him. Which may or may not have been something he could control. But overall, a very unpleasant experience.

Very prompt walked in and introduced himself with nice hand shake. Explained thedmages to us and checked to make sure we understood d asked if we had any questions or him stood up and shook our hands again and told us to have a safe trip home as we had two hours and it was snowing hard. Very professional and knowledgable.

This visit was a referral from my Dr. In Walla Walla. I was so impressed by Dr. Howlett and his team! They did x-rays and explained the findings and gave options on my treatment. It was worth the trip to finally get answers on the extreme pain I have been having!
