After sending me to another Dr for evaluations, paperwork was filled out and signed by myself forbidding the sharing with anyone including my personal physician of any information. When my next appointment came up with Dr Reynolds it was revealed to me that confidential information was indeed revealed leading to a violation of several laws as well as my personal rights. I was extremely upset by this and informed them I would be seeking legal counsel. I have now received a letter from Henry County Hospital banning me from there and removing me as a patent after being one ther for almost 15 years with no other issues."
