"on 5/6/21 I went to a doctors appointment at the Prima Health Care Facility in Salem Ohio to see my PCP Dr Shivers, as a follow-up to my 2 previous visits to the emergency room in Alliance... I explained to Dr Shivers that my son had done a Google search of my symptoms and came up with Unstable Angina as a diagnoses... Dr Shivers stated to me that normally that would show up as pain on my left side, not my right, and said that he would schedule a x-ray at Salem Hospital for me... I told him that the 2nd ER doctor told me that an x-ray would not show anything and that she also said it would be a waste of time... Dr Shivers completely disregarded my symptoms of Unstable Angina, and insisted on me getting the x-ray, so I left the office and went straight to Salem Hospital to have the x-ray done... FYI, on 5/8/21 I was taken to University Hospital in Ravenna by an ambulance while having another heart attack and ended up having heart surgery within 45 minutes after arriving there!!! on Tuesday 5/11/21, I received a call from Dr Shivers nurse with the results of the x-ray, I blew up at her and told her what had happened to me, then I hung up on her... on the morning of 5/12/21 I received a phone call from Dr Shivers and he asked me what was going on, I explained to him what had happened, reminded him of me telling him about the Unstable Angina discussion we had, to which eh said we never talked about that... I called him a liar, told him to go f-himself, and I hung up on him... a few weeks later I received a letter from Prima Health Care informing me that Dr Shivers would no longer be working at their facility... since that time I reviewed my records and noticed that the records for my visit to Dr Shivers on 5/6/21 included medical procedures that were never done during that visit"
