I don't have the exact date but it was July 2014. First I had an odd question from the receptionist who verified my insurance and said, we will receive you but your insurance needs you to urgently called them. I called later and said they never asked to speak to me. Doctor's manners are not the best. I will never forget he said "you have too many issues" and I responded, doctor I'm not here to have coffee, I'm here because I have issues... Never went back.
