Called for an appointment and asked for instructions.
Office staff asked for a FULL medical record to be faxed to a number with no indication of who would receive it. 
had to inquire what would happen to my information and what the next steps would be.

Did not make me feel comfortable - mediocre customer service manner. Never got to speak to someone about my issue in the first place.
