I was a patient at ATS Detox and requested my personal file after discharge to verify the accuracy of services provided versus billed. Upon reviewing my file, I discovered a 16-page report containing an interview and assessment supposedly conducted with me. This report included my full name and date of birth on each page, along with Courtney Shulick's name. However, this interview and assessment never occurred. The report featured graphs, charts, and responses to questions that I was never asked, all stemming from an interview that never took place. It appears either another client was mistakenly assessed under my identity, or this document was fraudulently completed. Signed off and completed by Courtney Shulick This raises serious concerns about the integrity and accuracy of care provided. Imagine receiving treatment based on another client’s information and needs—it’s not only negligent but could also be a violation of HIPAA regulations or an instance of outright fraud. This level of oversight is deeply troubling and unacceptable. I have reached out to several members of ATS and have no answers.
