Endometrial Ablation

Endometrial ablation is a procedure that destroys and removes a thin layer of the uterine lining. It is usually performed to treat heavy menstrual bleeding, especially if bleeding cannot be controlled by medications. After an endometrial ablation, periods often stop completely. If they do not, they are much lighter and easier to manage.

There are many different ways that the uterine lining can be removed, including radiofrequency, heat, microwaves, and cold. Ablations are usually performed with local anesthetic by an OB/GYN. A probe is inserted into the uterus through the vagina, and the end of the probe is expanded or moved around to access the uterine lining. Cramping and discharge afterwards are normal, but the procedure is not particularly painful.

Endometrial ablation is not for everyone. It shouldn't be done by women who have reached menopause, or women with uterine cancer or uterine infections. It is important to remember that an endometrial ablation does not remove the uterus or ovaries, so regular pelvic exams are still important after this procedure. Women who are considering endometrial ablation should also be done having children. Pregnancy is possible after an endometrial ablation, though not likely. If pregnancy does occur, an ablation increases the risks of miscarriage and other problems, so reliable birth control must be used after endometrial ablation all the way through menopause.

Urinary Tract Infection (UTI)

A urinary tract infection, or UTI, happens when bacteria enter the body through the opening where urine is normally released. The bacteria infect the lining of the urethra and bladder, turning them red and inflamed. This causes pain in the abdomen or pelvic area, a burning sensation during urination, a sense of urgency about going to the bathroom, frequent urination, and urine that smells bad and looks cloudy, or even contains traces of blood. If the urinary tract infection is severe, it may travel all the way to the kidneys, a more serious kind of UTI called pyelonephritis. Patients with this kidney infection might have blood in their urine, feel back pain, and develop a fever.

Urinary tract infections are extremely common: 12% of all men and 40-50% of all women will have a urinary tract infection during their lifetime. They are more common in women because women have shorter urethras, so bacteria have a shorter distance to travel from the outside of the body to the bladder and cause an infection. Some people also have urethras that are an unusual shape or have an obstruction in the urethra that makes getting a UTI more likely. Also, certain chronic illnesses like diabetes weaken the immune system, so any bacteria in the body are more likely to cause an infection.

A urinary tract infection can be diagnosed very quickly by a doctor. A sample of urine can be examined under a microscope for the presence of bacteria or white blood cells. There are also diagnostic strips that can be used to test a urine sample without the need for a microscope. Once a diagnosis is made, treatment is a course of oral antibiotics, and most patients feel better within just a few days. There are some things that patients can do themselves to help reduce the risk of getting a urinary tract infection in the future. Stay well hydrated, wipe from front to back after going to the bathroom, wear breathable cotton undergarments, and don't hold it in when patients feel the urge to go.

Urodynamics (Bladder and Urethra Function Test)

Urodynamics are tests that assess how well the lower urinary tract is functioning. Specifically, they evaluate its ability to hold and release urine, as well as show if any blockages or leaks are present. The lower urinary tract is made up of the following:

The bladder, which stores urine.
The urinary sphincters, which are muscles around the opening of the bladder that hold urine in.
The urethra, which is the tube where urine flows out of the body.

Urodynamics tests are normally performed together as one series, but depending on patients' symptoms, only one or some of them may be necessary. The most common tests are:

Cystometrogram, which measures the amount of urine the bladder can store and how full it is before someone has the urge to urinate. Doctors may order this test if she suspects their patient has a weak bladder.
Pressure flow study, which measures the pressure in the bladder while urinating. This test helps locate a blockage in the bladder.
Uroflowmetry, which measures how much and how fast urine flows out of the body. This test is usually recommended when patients have difficulty urinating or they have a slow stream.
Urethral sphincter electromyography, which measures how strong the muscles and nerves in the bladder and sphincters are. Doctors may request this test if the problem they are experiencing is linked to muscle or nerve damage in the lower urinary tract.
Urethral pressure profile, which measures the pressure along the urethra while the bladder is gradually filled with fluid. Low pressure could mean someone has a weak urethra, a contributing factor to urinary incontinence (loss of bladder control).

For a few hours following a urodynamics study, patients will likely feel some soreness or discomfort. It is recommended to drink at least one glass of water every half hour for two hours to relieve these symptoms. Holding a warm, moistened washcloth over the area where discomfort is felt or taking a warm bath may also help. Results for cystrometrograms and uroflowmetry are typically available immediately after the test, but results for the other tests may take a few days. When doctors have reviewed a patient's test results, he or she will discuss them with the patient, along with any treatments the patient may need.
